DM - Gastroenterology, MD - General Medicine, MBBS
Gastroenterologist, Hyderabad
Gastroesophageal reflux disease, or gerd, is a digestive disorder that affects the lower esophageal sphincter (les), the ring of muscle between the esophagus and stomach. Many people, including pregnant women, suffer from heartburn or acid indigestion caused by gerd. Doctors believe that some people suffer from gerd due to a condition called hiatal hernia. In most cases, heartburn can be relieved through diet and lifestyle changes; however, some people may require medication or surgery.
What is gastroesophageal reflux?

Gastroesophageal refers to the stomach and esophagus. Reflux means to flow back or return. Therefore, gastroesophageal reflux is the return of the stomach's contents back up into the esophagus.
In normal digestion, the lower esophageal sphincter (les) opens to allow food to pass into the stomach and closes to prevent food and acidic stomach juices from flowing back into the esophagus.

Gastroesophageal reflux occurs when the les is weak or relaxes inappropriately, allowing the stomach's contents to flow up into the esophagus.
The severity of gerd depends on les dysfunction as well as the type and amount of fluid brought up from the stomach and the neutralizing effect of saliva.

What is the role of hiatial hernia in gerd?

Some doctors believe a hiatal hernia may weaken the les and increase the risk for gastroesophageal reflux. Hiatal hernia occurs when the upper part of the stomach moves up into the chest through a small opening. Many people with a hiatal hernia will not have problems with heartburn or reflux. But having a hiatal hernia may allow stomach contents to reflux more easily into the esophagus.
Coughing, vomiting, straining, or sudden physical exertion can cause increased pressure in the abdomen resulting in hiatal hernia. Obesity and pregnancy also contribute to this condition. Many otherwise healthy people age 50 and over have a small hiatal hernia. Although considered a condition of middle age, hiatal hernias affect people of all ages.

Hiatal hernias usually do not require treatment. However, treatment may be necessary if the hernia is in danger of becoming strangulated (twisted in a way that cuts off blood supply, called a paraesophageal hernia) or is complicated by severe gerd or esophagitis (inflammation of the esophagus). The doctor may perform surgery to reduce the size of the hernia or to prevent strangulation.

What other factors contribute to gerd?

Dietary and lifestyle choices may contribute to gerd. Certain foods and beverages, including chocolate, peppermint, fried or fatty foods, coffee, or alcoholic beverages, may trigger reflux and heartburn. Studies show that cigarette smoking relaxes the les. Obesity and pregnancy can also play a role in gerd symptoms.

What are the symptoms of heartburn?

Heartburn, also called acid indigestion, is the most common symptom of gerd and usually feels like a burning chest pain beginning behind the breastbone and moving upward to the neck and throat. Many people say it feels like food is coming back into the mouth leaving an acid or bitter taste.

The burning, pressure, or pain of heartburn can last as long as 2 hours and is often worse after eating. Lying down or bending over can also result in heartburn. Many people obtain relief by standing upright or by taking an antacid that clears acid out of the esophagus.

Heartburn pain can be mistaken for the pain associated with heart disease or a heart attack, but there are differences. Exercise may aggravate pain resulting from heart disease, and rest may relieve the pain. Heartburn pain is less likely to be associated with physical activity.

How common is heartburn and gerd?

More than 60 million adults experience heartburn at least once a month, and more than 15 million adults suffer daily from heartburn. Many pregnant women experience daily heartburn. Recent studies show that gerd in infants and children is more common than previously recognized and may produce recurrent vomiting, coughing, and other respiratory problems.

What is the treatment for gerd?

We recommend lifestyle and dietary changes for most people needing treatment for gerd. Treatment aims at decreasing the amount of reflux or reducing damage to the lining of the esophagus from refluxed materials.
Avoiding foods and beverages that can weaken the les is often recommended. These foods include chocolate, peppermint, fatty foods, coffee, and alcoholic beverages. Foods and beverages that can irritate a damaged esophageal lining, such as citrus fruits and juices, tomato products, and pepper, should also be avoided if they cause symptoms.

Decreasing the size of portions at mealtime may also help control symptoms. Eating meals at least 2 to 3 hours before bedtime may lessen reflux by allowing the acid in the stomach to decrease and the stomach to empty partially. In addition, being overweight often worsens symptoms. Many overweight people find relief when they lose weight.

Cigarette smoking weakens the les. Stopping smoking is important to reduce gerd symptoms.

Elevating the head of the bed on 6-inch blocks or sleeping on a specially designed wedge reduces heartburn by allowing gravity to minimize reflux of stomach contents into the esophagus. Do not use pillows to prop yourself up; that only increases pressure on the stomach.
Along with lifestyle and diet changes, doctor may recommend over-the-counter or prescription treatments.

Antacids can help neutralize acid in the esophagus and stomach and stop heartburn. Many people find that nonprescription antacids provide temporary or partial relief. An antacid combined with a foaming agent helps some people. These compounds are believed to form a foam barrier on top of the stomach that prevents acid reflux from occurring.

Long-term use of antacids, however, can result in side effects, including diarrhea, altered calciummetabolism (a change in the way the body breaks down and uses calcium), and buildup of magnesium in the body. Too much magnesium can be serious for patients with kidney disease. If antacids are needed for more than 2 weeks, a doctor should be consulted.

For chronic reflux and heartburn, the doctor may recommend medications to
Reduce acid in the stomach. These medicines include h2 blockers, which inhibit acid secretion in the stomach.

What if heartburn or gerd symptoms persist?

People with severe, chronic esophageal reflux or with symptoms not relieved by the treatments described above may need more complete diagnostic evaluation. Doctors use a variety of tests and procedures to examine a patient with chronic heartburn.

An upper gi series may be performed during the early phase of testing. This test is a special x-ray that shows the esophagus, stomach, and duodenum (the upper part of the small intestine). While an upper gi series provides limited information about possible reflux, it is used to help rule out other diagnoses, such as peptic ulcers.

Endoscopy is an important procedure for individuals with chronic gerd. By placing a small lighted tube with a tiny video camera on the end (endoscope) into the esophagus, the doctor may see inflammation or irritation of the tissue lining the esophagus (esophagitis). If the findings of the endoscopy are abnormal or questionable, biopsy (removing a small sample of tissue) from the lining of the esophagus may be helpful.

Esophageal manometric and impedance studies -- pressure measurements of the esophagus -- occasionally help identify low pressure in the les or abnormalities in esophageal muscle contraction.

For patients in whom diagnosis is difficult, doctors may measure the acid levels inside the esophagus through ph testing. Testing ph monitors the acidity level of the esophagus and symptoms during meals, activity, and sleep. Newer techniques of long-term ph monitoring are improving diagnostic capability in this area.

Does gerd require surgery?

A small number of people with gerd may need surgery because of severe reflux and poor response to medical treatment. However, surgery should not be considered until all other measures have been tried. Fundoplication is a surgical procedure that increases pressure in the lower esophagus. Endoscopic procedures that involve making the les function better or using electrodes to promote scarring of the les are newer options in treatment.

What are the complications of long-term gerd?

Sometimes gerd results in serious complications. Esophagitis can occur as a result of too much stomach acid in the esophagus. Esophagitis may cause esophageal bleeding or ulcers. In addition, a narrowing or stricture of the esophagus may occur from chronic scarring. Some people develop a condition known as barrett's esophagus. This condition can increase the risk of esophageal cancer.

Although gerd can limit daily activities and productivity, it is rarely life-threatening. With an understanding of the causes and proper treatment, most people will find relief.

I have red eyes whenever I sleep late at night and have white eyes whenever I sleep in day.

DHMS (Diploma in Homeopathic Medicine and Surgery)
Homeopath, Ludhiana
I have red eyes whenever I sleep late at night and have white eyes whenever I sleep in day.
Eyes appear red because the vessels at the surface of the white portion of the eye (sclera) become swollen. Vessels may swell due to: • Eye dryness • Too much sun exposure • Dust or other particles in the eye • Allergies • Infection • Injury Eye infections or inflammation can cause redness as well as possible itching, discharge, pain, or vision problems. These may be due to: • Blepharitis: Swelling along the edge of the eyelid. • Conjunctivitis: Swelling or infection of the clear tissue that lines the eyelids and covers the surface of the eye (the conjunctiva). This is often referred to as "pink eye." • Corneal ulcers: Sores on the cornea most often caused by a serious bacterial or viral infection. • Uveitis: Inflammation of the uvea, which includes the iris, ciliary body, and choroid. The cause is most often not known. It may be related to an autoimmune disorder, infection, or exposure to toxins. The type of uveitis that causes the worst red eye is called iritis, in which only the iris is inflamed. Other potential causes of eye redness include: • Colds or allergies . • Acute glaucoma: A sudden increase in eye pressure that is extremely painful and causes serious visual problems. This is a medical emergency. The more common form of glaucoma is chronic and gradual. • Corneal scratches: Injuries caused by sand, dust, or overuse of contact lenses. Sometimes, a bright red spot, called a subconjunctival hemorrhage, will appear on the white of the eye. This often happens after straining or coughing, which causes a broken blood vessel on the surface of the eye. Most often, there is no pain and your vision is normal. It is almost never a serious problem. Because the blood leaks into the conjunctiva, which is clear, you cannot wipe or rinse the blood away. Like a bruise, the red spot will go away within a week or two. So,visit a local eyespecialist. The diagnosis of your problem in eyes can only be made by local examination of eyes

IUI: This is Important for ALL Women to Know

What all women should know about IUI

Intrauterine insemination or IUI is a form of artificial insemination that involves the sperm being placed inside the uterus of a woman for bringing about fertilization. The aim of this procedure is to enhance the number of sperms that successfully reach the fallopian tubes and fertilize an egg, thereby increasing your chances of getting pregnant. Depending on the cause of your infertility, the procedure can be synchronized with fertility drugs or your ovulation cycle.

When should you go for IUI?

While reduced sperm mobility or a low sperm count may be the reason this procedure is opted for, there are certain conditions that may cause you and your partner to opt for IUI:

  • If the cause of your infertility doesn't have any medical explanation
  • Problems with ejaculation affecting the release of semen
  • Problems associated with cervical mucus can hinder the journey of the sperm from the vagina to the fallopian tubes
  • Scarring of the cervix due to past surgical procedures, rendering the sperm unable to enter the uterus

At the same time, the procedure is not recommended for women who suffer from:

  • A disease of the fallopian tubes
  • Past episodes of pelvic infections
  • Mild to severe endometriosis (a condition wherein the womb's lining grows outside it)

What happens after IUI?

The procedure takes up only a few minutes of your time and causes very little discomfort. Following the procedure, you'll be told to lie on your back for a short period of time, after which you can get back to your normal routine the same day. You can expect instances of light spotting, which will go away after two days.

Can Beer Be a Healthy Drink?

PG Diploma in Clinical Research, Diploma in Acupuncture, MBBS, College Of Physicians & Surgeons
General Physician, Mumbai
Can Beer Be a Healthy Drink?

Can beer be a healthy drink? Despite the common myths surrounding alcohol consumption and the adverse effects that it has on a person in the long run, a controlled and well-balanced intake of beer is not as bad as it would seem.

Here are a few reasons proving that beer can be healthy for you if taken in the right amount and quantity:

  1. In comparison to other alcoholic beverages, beer is relatively healthier for your kidneys. As a matter of fact, studies show that a bottle of beer can actually reduce the risk of acquiring kidney stones by up to 40%.
  2. Fiber, as we all know, plays a vital role in digestion. The presence of up to one gram of soluble fiber in just a 30ml glass of beer (dark beer, in particular) makes the beverage very beneficial for digestion.
  3. Fiber present in beer can also help reduce the level of LDL cholesterol (a harmful proponent of cholesterol).
  4. It has been proved that beer is a rich source of various B vitamins like vitamin B1, vitamin B2, vitamin B6 and vitamin B12.
  5. The presence of both nicotinic acid and Lactoflavin in beer makes it an effective cure for insomnia.
  6. Beer is also helpful in the prevention of blood clots.
  7. The presence of high levels of silicon in beer is responsible for bestowing a stronger bone density.
  8. Studies have concluded that beer can also reduce the risk of suffering a heart attack.
  9. Unlike their non-drinking counterparts, beer drinkers are less vulnerable of experiencing dementia or Alzheimer's disease.
  10. Because of the presence of certain vitamins, beer has a ton of healthy nourishing benefits for the skin.
  11. Beer is also useful in fighting against stress.

What are the recommended safe limits of Beer?

  • Men should drink no more than 14 units of Beer per week, these units should be spread out through the week and they should have at least two alcohol free days a week.
  • Women should drink no more than 14 units of Beer per week, these units should be spread out through the week and they should have at least two alcohol free days a week.
  • Pregnant women or women trying to conceive should not drink alcohol at all. If they do choose to drink, to minimise the risk to the baby, they should not drink more than 1-2 units of beer once or twice a week and should not get drunk.

Despite the above people with increased Triglyceride, Uric Acid should be restricted for consumption of alcohol. People with fatty liver patient with other medication for hypertension, etc. also should refrain from drinking.

People with diabetes who drink should follow these alcohol consumption guidelines:

  1. Do not drink more than two drinks of alcohol in a one day period if you are a man, or one drink if you are a woman.
  2. Drink beer only with food.
  3. Drink slowly.
  4. Avoid "sugary" mixed drinks, sweet wines or cordials.

I feel that if you went for counseling too a lot can be done and resolved. So apart from the medication you need the help of a counselor who will equip you with coping skills and techniques to handle the daily pressures in life. Relying only on the chemical intervention is grossly insufficient. By just popping in pills how do you hope to develop skills and tools to manage life?! You can follow these simple rules too: Have a good night’s sleep, have a good breakfast of more proteins, meditate often, remain free of stress, eat a lot of fiber, nuts, avocado, exercise regularly, eat dark chocolate, do Yoga meditation exercises, etc. I suggest you do the opposite of what this depression makes you feel like doing (actually, not doing): you will need to fight this condition. You must become active; stay upright during the daylight time; meet people; never sleep during the day, wake up by 6 am every day, play some active games, especially contact games, do physical exercises, talk to people and join some social clubs, attend Yoga classes etc. Watch sitcoms on TV or comedies and cheer yourself up. Go for excursions in groups, for outings, camps, conferences, and religious conventions. Get a pet dog and spend time training it, exercising it and relating to it. Expose yourself to some sunlight every day, at least 30 minutes but not in the scotching heat. Whatever happens, please incorporate these three important adaptations in your life: always be responsible, be respectful, and be functional. If you did these three, lots of things will go well in life. Please pray and have faith in God to alleviate your sufferings. Don’t wait for others to help. Use your own motivation, which might be at its lowest, but persevere and win this battle. Above all to be really happy, you need to live in love and for love: find someone to love and to love you back. A counselor is there only to facilitate you, all the hard word must come from you, and your cooperation with that person is very critical for your success. Be positive everyday and learn to be contended with what you have. Do some left brain exercises: it is the happy brain. Here are a few suggestions: shut your left nostril and breathe, move your eyes from right to left and vice versa for at least half a minute at a time, and do callisthenic exercises with some form of counting, regularly.
