How to Improve Your Libido Post Menopause

MBBS, F R S T & H London , Fellowship of College of General Practice (FCGP)
Sexologist, Alwar
How to Improve Your Libido Post Menopause

Menopause automatically brings about a dip in testosterone and estrogen levels in a woman’s body. It leads to certain changes in the body and also reduces libido. A drop in libido means that sexual arousal becomes a cause of concern. This can bring in certain problems in your sexual/conjugal life. Lower estrogen levels reduce supply of blood to the vagina, thus resulting in lubrication problems. A dry vagina during sexual intercourse can lead you to experience pain during sex.

There are some other factors that can affect a woman’s libido after menopause:
1. Problems in sleep
2. Anxiety and depression
3. Problems of the bladder
4. Certain medications
5. High stress levels

To remedy lubrication problems, use water soluble lubricants to lubricate your vagina. Avoid using non-water soluble products such as petroleum jelly as it can weaken the latex. Non water soluble lubricants are also known to aggravate bacterial vaginosis. You may also use vaginal moisturizers to address the issue. There are oral medications that are available, which cause the tissues of the vagina to thicken, thus causing less pain during sex.

Improving your libido
You can opt for estrogen therapy to reduce pain during sex as this therapy helps in vaginal lubrication. The main objective of estrogen therapy (Hormone Replacement Therapy) is to ease the discomforts associated with a deficiency of estrogen secretion in the body. You should consult a professional who will be able to guide and counsel you.

Certain other measures to rev up your libido have been listed below:

  1. Indulge in foreplay: Try and include activities such as oral sex and erotic massages in your agenda. Experimentation with foreplay can help spice things up between you and your partner.
  2. Try out new things: Change your sex poses, read erotic books or watch erotic videos as these may help in boosting your libido.

Bear in mind that menopause does not prevent STDs; you still need to take proper precautions before engaging in sexual intercourse. If you wish to discuss about any specific problem, you can consult a gynaecologist and ask a free question.

Exercise for Pregnant Women

Walking during pregnancy is the best cardiovascular exercise. A pregnant women can do it at home or anywhere else. Walking is a great, safe exercise for pregnant women. It’s an ideal way to make sure that you’re getting the exercise you need in pregnancy. Brisk walking works your heart and lungs.

Start with a short, comfortable stroll and gradually increase the duration of the walk.
Once you’ve got into the habit of walking regularly, you can build up to faster, 30-minute walking sessions, four or more times a week.
You can walk for longer than that. Slow down or stop if you feel tired or feel any pain.

Swimming is safe in pregnancy.

Swimming helps to keep you fit, which will make it easier to adapt to pregnancy.
Swimming is a particularly good exercise in pregnancy, as the water helps to support your extra weight.
It’s usually safe for you to swim throughout your pregnancy.

Benefits of swimming:
• improve circulation
• boost heart and lung function
• increase muscle tone and strength
• build endurance
• reduce swelling and fluid retention
• burn calories, which helps to manage weight gain
• promote good sleep.
Yoga

Yoga is a way of life, opt for a beginner’s yoga class that uses a gentle form of yoga.
Wear loose, comfortable, light clothing. Yoga is easier to do if you’re barefoot, so you don’t need any special footwear.
Yoga should be practiced daily. Join regular aerobic exercise too, such as brisk walking or swimming. This will complement your yoga practice and help to develop muscle tone, strength and stamina.

Pilates

What is pilates?

Pilates makes your body strong, flexible and balanced. It involves a series of movements and positions that help to improve your strength. The movements focus on your tummy, pelvic floor muscles and back muscles, which are all key to good posture, balance and strength. They also help your back and pelvis.
How can pilates help me in pregnancy?
Pilates strengthens your tummy, back and pelvic floor muscles without straining other joints, so it’s a great exercise to do when you’re pregnant. Some research suggests that doing pilates regularly can be as effective as doing pelvic floor exercises.
The main benefit of pilates is that it targets the exact muscles and functions that can be a problem during pregnancy and after birth, in a safe way.

Wanting to look young is not exactly a new idea

MBBS, DDV
Dermatologist, Mumbai
Wanting to look young is not exactly a new idea – the search for the fountain of youth has been going on for centuries.

There are many treatments available to combat the aging effects of the sun, from creams to peels to lasers. Plus, reducing the signs of sun damage can reduce your risk of developing skin cancer.

The sun and the years take their toll on your skin. Photo ageing is often the first sign that your skin has been damaged, but such repeated exposure can also lead to skin cancer. Luckily, many of the treatments for photo ageing not only restore skin youthful appearance, they also reduce your risk for developing skin cancers and pre cancers.

There are many non-surgical options offered to people who would like to look younger.
1) Creams – Sunscreens, Retinoids.
2) Injectables – Such as Dermal Fillers, Muscle Relaxers.
3) Skin Surface Treatments – Including Chemical Peels and Dermaroller / Micro Needling.
4) Laser, IPL and other heat treatments.
Sunscreens

If you don’t wear a sunscreen every day, today is the day to start. Daily application of sunscreen with an SPF of 15 or higher- whether on its own, or in a moisturizer – will prevent additional damage to your skin. Also, studies have shown that daily use of sunscreen can actually reduce the existing number of actinic keratoses – abnormal skin eruptions or pre cancers that can lead to skin cancer.

Retinoids
Retinoids have been a breakthrough discovery for the treatment of photo aged skin. Synthetic derivatives of Vitamin A, they can improve discoloration of the skin, degeneration of elastic tissue, and fine wrinkling by enhancing naturally occurring production of collagen and elastic fibers. In addition to giving skin back its youthful appearance, retinoids can inhibit tumor growth, decrease inflammation, and enhance the immune system. Continuing use of retinoids can reduce the number and size of actinic keratoses. Currently, two retinoids are available for treatment of photo ageing: (1) Tretinoin (2) Tazarotene. Note that retinoids can irritate the skin and cause dryness and photo sensitivity – an extreme sensitivity to the sun. They are available by prescription only.

Lunch Time Lifts
The “Lunch Time Face Lift” promises a younger, better-looking you with no surgery, no general anaesthetic and no down time. While “Lunch Time” and “Face Lift” may be a bit of a stretch, these days it’s increasingly possible to take a few years off your face in a day clinic without the expense, pain and temporary disappearance from public life associated with surgery.

INJECTABLES Dermal / Skin Fillers
Filling substances are injected under facial skin to round out contours and correct wrinkles, furrows and hollows. A number of substances are currently used as fillers, including bovine collagen, hyaluronic acid, polymer implants, and the patient’s own (autologous) fat. To maintain the effects of bovine collagen and autologous fat, the procedure needs to be repeated periodically. Many of these treatments can be used in conjunction with one another to produce younger-looking. It is important to consult with a licensed dermatologist to discuss treatment options, as many of these treatments can cause damage in inexperienced hands

Rather than just smoothing or tightening skin, they can change the facial profile to a more youthful one.

Hyaluronic acid is now more popular. Hyaluronic acid lasts 6-12 months – although treating areas of the face that move less will last longer those that move more, such as the lips – and you tend to get better results with each treatment. There’s evidence that hyaluronic acid injections can also have a more permanent anti-ageing effect by stimulating collagen growth.

Muscle Relaxers
Botulinum toxin A temporarily paralyses muscles when injected. This prevents the skin above from creasing up and causing wrinkles. It only works on so-called expression lines, such as frown (vertical) lines and surprise (horizontal) lines on the forehead, smoothing them and preventing them from becoming more pronounced. The effects take a few days to kick in, and initially last a few months – longer after continued treatment.Read more…
Liquid Face Lift / Liquid Facial
Combined use of botulinum toxin A and dermal fillers is called a Liquid Face Lift / Liquid Facial.

SKIN TREATMENT

Chemical Peel
Acid, such as glycolic acid, lactic acid or trichloracetic acid, is applied to the skin and exfoliates the top layer of skin, causing dead skin cells to peel off. At stronger concentrations, new tighter skin forms as collagen production is stimulated in response to the wounding. It helps reducing fine lines, small scars, discoloration and sun damaged skin. The stronger the peel, the more dramatic and longer-lasting the results – stronger peels (concentrations of more than 30%) are better left to medical professionals rather than beauticians.
The benefits can last for two years for stronger peels, although it depends on the strength of acid used (which determines how deep the peel is). These days a course of several milder peels, rather than one strong peel, is common. Milder peels may require top-up peels every few months.Read more…

Dermaroller / Micro Needling
Dermarollers are used for a treatment process called micro-needling or skin needling. It is a form of collagen induction therapy. The actual dermaroller is made up of a small, solid wheel attached to a little stick that propels it backwards and forwards across your face in small sections. The roller is covered with hundreds of minuscule ‘micro-medical’ needles that penetrate the skin, creating lots of tiny micro injuries.Read more…

Intense Pulsed Light
Intense pulsed light, commonly abbreviated as IPL, is a technology used by medical practitioners to perform various skin treatments including hair removal and photo rejuvenation. The technology utilizes a broad spectrum light source, with a general spectral range of 515 to 1200 nm. Various cutoff filters are commonly used to filter out lower frequencies and narrow this spectrum to target specific structures and chromophores. Because of the broad spectrum of the IPL light source they are unique, as opposed to lasers, in the ability to filter the light spectrum to target various chromophores in the skin

Dear user. Vagina is a storehouse of many germs which could cause many diseases. You should understand that vagina cannot be cleaned completely. Therefore it is not advised to lick or suck vagina. Though you can always do that if you can ensure that it is perfectly clean and hygienic. Take care.

Hi there ~ please have your mother consult a gynecologist. Usually the use of a vaginal lubricant cream is all that is necessary to have painless enjoyable sex at that age. However if there is another gynecological issue even that will be addressed. Hope you do so as soon as possible. I hope this helps. Take care and have a lovely day! sincerely

Chikungunya Fever - Causes and Treatment

MBBS, MD - General Medicine
General Physician, Gurgaon
Chikungunya Fever - Causes and Treatment

The tiny mosquito can pose a lot of health issues. While malaria was the only dreaded disease until some time ago, the incidence of chikungunya is constantly on the rise. In fact, the nonspecific symptoms and absence of vaccination make it extremely difficult to prevent and diagnose this in the early stages.

Causes: First described in the African continent (Tanzania), the word chikungunya means “to walk bent over” in the Makonde or Kimakonde language. This appearance is because of the effect on bones, leading to severe bone and joint pains. More than 90% of the people who are bitten by the infected mosquito develop symptoms.

Chikungunya is a member of the Togaviridae family and is transmitted by bites of the infected female mosquito. In rare cases, the infection can spread through blood transfusions of the infected person.

After entering the bloodstream, it divides and multiplies rapidly in different types of cells including epithelial cells, endothelial cells, and connective tissue cells. These produce the symptoms including the rashes and the joint pains.

Symptoms: The challenge with chikungunya is that there is no specific, telltale symptom indicative of the condition. The patient develops nonspecific symptoms as below, somewhere between 4 to 7 days after the infected mosquito bite.

  1. High fever (as high as 40 degrees Celsius)

  2. Pain involving multiple joints which can be debilitating

  3. Headache

  4. Nausea and vomiting

  5. Muscle pain

  6. Skin rashes

  7. Swelling around the joints

  8. Conjunctivitis

Though the disease is not fatal, it can be quite limiting. The fever and other symptoms may disappear within a week, but the joint pains can last up to 6 months to a year.

Diagnosis: A blood test is the most diagnostic test for confirming the disease. Dengue fever and Zika virus need to be ruled out, as their presentation is very similar.

Treatment: There is no definitive treatment for this condition and it is mostly symptomatic.

  1. The patient is advised rest to recover from the fever and fatigue

  2. Pushing fluids, including juices and electrolytes is recommended to prevent dehydration

  3. Fever needs to be managed with antipyretic medications

  4. Joint pains are managed with nonsteroidal painkillers

  5. Avoid aspirin as it increases the tendency to bleed

  6. Physiotherapy is recommended to improve severe bone/joint pains to improve mobility and reduce swelling

  7. Check with your doctor to see if your regular medications need to be revisited

There is no vaccine to prevent chikungunya, though virus like particles that can be a potential vaccine are in the final stages of trials. Prevention of mosquito bites is also highly recommended as an important step in controlling the disease. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

You just go and get ultrasonography done. Sometimes because of some retained products of conception you may have bleeding. If there is no problem in ultrasonography, you just will have to take medicines and you will be alright. Give rest to your uterus and your body for at least 3months. Then you can conceive.

The most common early signs and symptoms of pregnancy might include: Missed period. If you're in your childbearing years and a week or more has passed without the start of an expected menstrual cycle, you might be pregnant. Tender, swollen breasts. Nausea with or without vomiting. Increased urination. Fatigue. Pregnancy tests check for the presence of the pregnancy hormone, human chorionic gonadotrophin (HCG) in your urine. After you conceive (when the sperm fertilises the egg), your body begins to produce HCG. Around two weeks after you conceive, there's enough HCG for most pregnancy tests to detect it.

As the periods come on proper time but the amount of menstrual flow is very low and do not last for 2-3 days. It means that your hormones that control the the periods (menstruation) have normal cycle but the amount of hormones are less than normal that's why the flow is scanty. *for normal & adequate menstrual flow get up early in the morning and take a fast walk for atleast 15 -20minutes. If you can run, then run atleast for 3-5 minutes. * practice bhujangasana (cobra postion) & pawanmuktasana everyday only under the guidance of yoga expert. * check out your thyroid profile & in case of abnormal reports get the proper treatment of it. * monitor your weight & if you are overweight then do more physical activities & change lifestyle to reduce the weight so that the peroids get normal. * eat more green leafy vegetables & avoid fast foods having high refined starch & fat. * you can take dashmoolarisht for your scanty periods only under the supervision of ayurvedic doctor. It can make your hormones level normal and make your periods regular.

Menstrual periods are often irregular during the first few years after menstruation starts. It may take several years for the hormones that control menstruation to reach a balance. Menopause or peri-menopause is the phase when a female again faces irregular menses. Pregnancy is the most common cause of a missed period. If you might be pregnant, treat yourself as if you are pregnant until you know for sure. Use a home pregnancy test as the first step to finding out whether you are pregnant. If you are not pregnant, other causes of missed or irregular periods include: Excessive weight loss or gain. Although low body weight is a common cause of missed or irregular periods, obesity also can cause menstrual problems. Eating disorders, such as anorexia or bulimia. For more information, see the topic Anorexia Nervosa or Bulimia Nervosa. Increased exercise. Missed periods are common in endurance athletes. Emotional stress. Illness. Travel. Medicines such as birth control methods, which may cause lighter, less frequent, more frequent, or skipped periods or no periods at all. Hormone problems. This may cause a change in the levels of the hormones that the body needs to support menstruation. Illegal drug use. Problems with the pelvic organs , such as imperforate hymen, polycystic ovary syndrome, or Asherman's syndrome. Breast-feeding. Many women do not resume regular periods until they have completed breast-feeding. Other diseases such as irritable bowel syndrome, tuberculosis, liver disease, and diabetes can cause missed or irregular periods, although this is rare. Any nutritional deficiency can also lead to Menstrual problem/ irregularity. Take more of green vegetables, nuts, salads, seasonal fruits, yellow coloured veggies, milk & milk products, plenty of water and enough of sleep & rest. Take homoeopathic medication – Schwabe's Five Phos. And Alfaalfa Tonic – both thrice daily for 1 month. *****also get an Ultrasound (pelvis) done****
