Dental problems can be very painful and take the smile off your face completely. Those who have experienced sensitivity would vouch for it. It just will not allow you to enjoy the hot coffee or a favourite sweet or a cold smoothie.  The pain that shoots down the tooth after any of these would leave the person shuddering.

The tooth has 3 layers, from the outside in these are the enamel, the dentin and the pulp.  The outermost enamel is mineralized and the hardest structure in the body. The next layer, dentin, has fine tubules and when the enamel is worn off exposing the dentin, sensitivity sets in. The enamel is thinner in some portions of the tooth like the neck and the roots, therefore sensitivity onset is quicker in these areas.

Some of the common reasons for sensitivity are listed below:

  1. Tooth decay:  As caries progresses from enamel to dentin, sensitivity sets in, especially to hot and cold foods.
  2. Wear and tear:  Excessive brushing of teeth can lead to wearing of teeth, especially near the neck areas, leading to loss of enamel and resulting in sensitivity.
  3. Dental damage:  Chipped or broken tooth can lead to sensitivity.
  4. Gingival disease: This leads to loss of gum cover over the tooth, leading to exposure of dentin and therefore sensitivity.
  5. Bruxism: Clenching of teeth, common in people with high levels of stress, can lead to enamel wearing and subsequent sensitivity.
  6. Dental treatment: Some dental procedures like scaling, crowns, root planing, and some fillings can cause transient sensitivity.
  7. Highly acidic food items: Citrus fruits, tomatoes, tea, etc. can lead to tooth sensitivity.
  8. Bleaching agents:  Most tooth whitening agents result in sensitive teeth.
  9. Excessive use of mouthwash: The mouth rinses contain high amount of alcohol which can also lead to tooth sensitivity.

Management: As with all health conditions, the first step in management is to identify the problem.  Whether it is dental decay or recent dental treatment, food habits or tooth whitening agents, the cause needs to be identified and then treatment begun accordingly.

  1. For lost enamel, be it decay or damage, the tooth again needs to be restored to its original form to cover the dentinal tubules and reduce sensitivity.
  2. For habits, be it food related or bruxism, in addition to treating the tooth, the habit per se needs to be addressed to prevent recurrence.
  3. For associated dental products like whitening agents or mouth rinses, talk to a dentist to switch to a less harmful product.
  4. De-sensitizing pastes are also available that can be used on a regular basis to reduce the problem.

Tooth sensitivity is annoying but there are ways to manage it effectively.

Pain Even After Root Canal - What To Do

When a small cavity isn't treated in time, the tooth decays badly that normal filling cannot solve the problem. In such cases, your dentist will probably advise you to undergo, a root canal. When performing a root canal, the dentist will remove the nerves and pulp within the tooth, clean the inside of the tooth and seal it. After this procedure your tooth will not sense anything and should be pain free.

However, in some cases you may still experience pain after root canal and there are four main causes of this pain:

  1. Swelling of ligament around the tooth: One of the signs that you need a root canal is swelling of the gums. Even after the nerves and pulp within the tooth are removed, the ligaments around the infected tooth may still be swollen. This can take some time before the tissue is normal again. In most cases this is the cause for pain after a root canal procedure.
  2. Damaged tissue: Part of the root canal procedure is to clean the insides of the tooth. Here the dentist must be very careful to not go beyond the tooth. In some cases the file used to clean the tooth may go beyond the root and damage the tissue there. Another possibility is that the sealant used to fill the tooth may go beyond the root, thus aggravating the tissue. This can take some time to heal and may cause pain.
  3. Excess filling: After the tooth has been cleaned, the dentist fills the tooth with a sealant. If excessive sealant is filled in the tooth, it may become taller than the surrounding teeth, which puts extra pressure on the tooth and results in soreness and pain. The dentist will have to remove the excess sealant in order to resolve this issue.
  4. Phantom pain: Phantom pain is rare when it comes to root canal. This occurs when the nerve leading up to the tooth still behaves like it were connected to the tooth despite the nerve within the tooth being completely removed. The peripheral nerves will need to be treated to resolve this issue.

In most cases, these events cannot be prevented. There is also no reason to expect that this is a symptom of your root canal being a failure. Stay in touch with your dentist and brush and floss your teeth regularly. You will soon notice the pain subsiding.

Kindly apply orahelp gum paint twice daily for 15 days. Take multivitamins for a month. Also notice that you are not constipated, stressed or having lack of sleep, having acidity problem as these are few of the reasons for ulcers. Avoid hot and spicy food as this may irritate the ulcers. Brush your teeth twice daily using correct brushing technique to maintain your oral hygiene.

I have got a very bad ulcer in my mouth on my lips and tongue. May I know how can I get rid of the pain as soon as possible. I tried antibiotics. But the pain is gain more and more.

I have stains in my teeth. I had it cleaned every 3months basis. Bt it keeps on appearing. Doc advice me not to smoke r drink neither. Bt I am a tetottler I hv never drank alcohol r smoke cigrate in my life. Pls suggest something. I brush twice a day.

you try to get scaling done aling with polishing. use a mouth wash ,rinse after each meal,use a anti plaque tooth paste and soft bristles tooth brush

I am 23 year male, I have problem of black spot at the base of front 2 tooth, I don't use to smoke or chew any thing. I tried a lot to remove it, but didn't work. I should I do?

We need more investigations to decide upon treatment. You may need cleaning and artificial enamel filling or root canal treatment. Advance procedures can be done with laser. Dental tips: - visit a dentist every six months for cleaning and a thorough dental check-up. Limit sugary food to avoid tooth decay. Gargle your mouth thoroughly after every meal. Scrub gently to clean your tongue with a tongue cleaner. Floss all your teeth inter dentally & brush twice daily, morning & night, up & down short vertical strokes, with ultra-soft bristles, indicator brush. Tooth brush to be changed every 2 months.

For protrusion you need orthodontic treatment ie. Braces. Consult an orthodontist. After examining your teeth he will suggest the braces accordingly.

Hello, as per your query a cold sensation after fracture of the teeth is commonly due to exposure of the dentine to the oral cavity. You need to pay a visit to the dentist so that the necessary remedy can be carried out.

FOR BLEEDING GUMS: We need more investigations with clinical examination to decide upon treatment. You may need deep cleaning along with surgical curettage or gum strengthening procedure. Rinse your mouth thoroughly with a mouth wash after every meals. Use pepsodent tooth paste & gum paint for 2 to 3 weeks. FOR YELLOW TEETH: Advise deep cleaning, polishing, whitening (bleaching) procedures, artificial enamel filling or capping. Advance procedures can be done with laser. Use Snowdent tooth paste for whitening until then. Apply SNOWDENT paste on the teeth. Wait for 10 to 15 min .Then brush your teeth with the same paste. Applying it overnight for better results.
