Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are experiencing inflammation at site of attachment of tendo-achilles. My advise is: - intensive ice packs 3-4 times in a day at home - try restrict your physical activity till your inflammation goes down - take an anti inflammatory medication for 5-7 days whichever suits your bode - wear well cushioned sports shoes for ambulation - intensive physical therapy, initially with modalities and then exercises, gradual training of muscles and short increments per day. Let me know in 2-3 weeks time how you are feeling. Do not hesitate to contact me if you need any further assistance.

10 Ways to Manage Chronic Back Pain

FIPP ( UK ), DA, MBBS
Pain Management Specialist, Navi Mumbai
10 Ways to Manage Chronic Back Pain

Chronic back pain may occur due to a variety of reasons. Sometimes, it may be due to conditions like osteoporosis or arthritis and at other times, it may be due to an injury or a slip disc caused by such an injury. Injuries to the spinal cord can also trigger chronic back pain. There are numerous ways of dealing with chronic back pain, which may be debilitating, if not treated properly.

10 ways of managing this pain.

  1. Medication: One of the ways of managing chronic back pain is with the help of anti-inflammatory drugs as well as pain killers like ibuprofen. However, do keep in mind that these medicines must be consumed only if they are prescribed by the doctor. Anticonvulsants and anti-depressants can also help in preventing pain that emanates from the nerves of the region.
  2. Surgery: Surgery can be done by the doctors in case the pain is extreme and severe. This kind of surgery usually aims at implanting devices and tools that can support the slipped disc, which may cause pinched nerves.
  3. Electrotherapy: This kind of therapy can help in reducing pain by releasing low-voltage electric stimulation that helps in dealing with the sensory nervous system.
  4. Exercises: Muscle strengthening exercises will help in a better balance. Also, other physiotherapy exercises can help in reducing back pain significantly by targeting the points that create pain and by pinching of the nerves.
  5. Relaxation techniques: There are a number of relaxation techniques that one can turn to so as to reduce the pain. This pain can also be managed with the help of meditation and breathing exercises, which can serve as an alternative means of treatment.
  6. Rest: With proper rest, one can do away with chronic pain. Yet, if it is a nerve-based issue, then the doctor may ask you not too lie down for long stretches and to walk regularly so that inflammation and pain does not set in.
  7. Heat: Heat therapy or heat compresses as well as hot water bags can be used for pain relief.
  8. Prolotherapy: In this method, an irritant solution is injected into the back so as to stimulate better blood circulation. This process also aids in faster ligament repair.
  9. Opioid fusion pump installation: These pumps are surgically implanted so that opioid agents are delivered straight to the spinal cord, which can treat and manage chronic back pain.
  10. Injections: These are usually considered as blocks that deliver steroids to the ligaments and joints. Also, one may have to take corticosteroid injections in order to deal with a very painful case. These epidural injections are usually best for temporary relief. One will need more long term treatment or management methods in order to effectively deal with and reduce pain.

I am suffering with back bone pain and body pains for that pains even I can't sleep in the night perfectly.

MBBS, Diploma in Psychological Medicine, M.D Psychiatry

Hi Siva, most common cause of low back pain and body aches are postural pain. Try to seat in a correct posture while doing your works. Do stretching exercise regularly, specially back muscle exercises. It will strengthen your back muscle to relieve the pain. If not then consult with a physical medicine specialist, If not improved you consult with a psychiatrist. But you have to rule out any underlying pathology in the bones, muscles n nerves. Thanks.

My husband is 33 years old he is suffering from backace for the lats 3 months. What it could be regarding?

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist, Delhi
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned backache as your problem. If you have only localized backache with no other associated symptoms like radiating leg pain, numbness, paresthesias etc, and no systemic symptoms like fever, loss of appetite, loss of weight etc, my advice to you for initial 2-3 weeks will be: - to maintain proper posture while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or even tab paracetamol which suits you - intensive physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied 3-4 times in a day - analgesic spray for local application like dynapar qps spray can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc backache can be due to multiple reasons like degenerative disc issue, weak back muscles, mechanical back pains, spinal stenosis etc. If you do not improve in 2-3 weeks with this conservative management, we will need to evaluate you further in form of dynamic x rays to look for any instability as a cause for pain and some blood investigations to rule out any deficiencies etc. Do not hesitate to contact me if you need any further assistance. Thanks & regards Dr. Akshay kumar saxena consultant orthopaedics fortis hospital, new delhi.

I have done a surgery in my left hand wrist in August 2013 due to a fracture in bone in an accident. There is a titanium plate placed in the wrist area. Now 2 years have been passed. Is it compulsory to remove that plate thru a surgery? I don't feel any difficult in functioning & daily work using my left hand. Please guide.

MS - Orthopaedics, MBBS
Orthopedist, Pune
Hello lybrate user, I need to see the fracture x-ray and the plate applied x-ray a recent one to comment on your question generally plates or for that matter any implant in the body are a foreign body and a potential source of infection and a landing pad for bacterias that may cause infections anywhere else in the body. Titanium plates or implants are having a lower risk in this respect as compared to stainless steel implants. Follow up with your x-ray for a second opinion god bless you.

I have too much of dandruff, I have tried different shampoos but again it cmes back. Is there any natural therapy to reduce the dandruff. Please suggest.

BHMS
Homeopath, Faridabad
Hello, dandruff causes hair loss so its necessary to treat dandruff first. Take thuja 200, 5 drops in the morning, once in a week. Take arnica q and cochleria arom. Q in equal amount, mix with equal amount of coconut oil, and apply this on your scalp, thrice a day. Wash your hair with b&t anti- dandruff shampoo. Revert me after 1 month. Home remedies are as follows:- coconut oil helps eliminate dandruff due to its antifungal properties. It also moisturizes dry scalp and provides relief from itching. Take some coconut oil and mix half the amount of lemon juice in it. 1. Rub it on your scalp and massage for a few minutes. 2. Wash your hair after at least 20 minutes. 3. Follow this remedy two to three times a week.

Hello doctor good evening. I am 50 years old woman and diabetic patient. I came to know about it just three months back because my shoulder was paining. Right now I am taking me dicine. I have few questions. Will you please answer them. 1. How long will it take to cure my shoulder pain. I am regularly doing some exercises. 2. I have had my blood test done from two different labs but both are showing different results. So which is more accurate.

Normally without clinical examination its difficult to tell you how many days for you to be alright with your shoulder pain that too without any x ray or mri reports generally if you have severe pain stop doing any exercise to that particular joint regarding lab tests it would be more accurate if is by nabl certified laboratory.
