Dear Lybrate User, *Diabetes Mellitus is a chronic metabolic disorder in which the body is unable to make proper use of glucose, resulting in the condition of hyperglycemia (high blood sugar). Excess glucose in the blood ultimately results in high levels of glucose being present in the urine (glycosuria). This increases the urine output, which leads to dehydration and increased thirst. *Glucose comes from the food we eat and is also made in liver and muscles. The blood carries glucose to all the cells in our body. Insulin, a chemical (or hormone) produced in the pancreas, is responsible for the uptake of glucose into cells for energy. Decreased levels of insulin affects this mechanism leading to increased glucose in the blood stream. •Include whole grains in the diet, such as wheat bread/pasta and brown rice. •Cheese and yogurt prepared with skimmed (nonfat) milk may be taken. •Use garlic, onion, bitter gourd, spinach, raw banana, and black plum. •Make a flour mixture of 1 part barley, 1 part black chickpeas, and 4 parts whole-wheat flour and use this to form pancakes and bread. •Avoid sweet, sour, and salty foods, potatoes, sweet potatoes, colocasia (taro), yams, fresh grains and pulses (legumes), whole yogurt (high in fat), and heavy, oily and spicy foods. •Avoid sweet fruits like pineapple, grapes, mangoes, etc. •Start doing some light exercise, such as brisk walking. Build up to a brisk walk of 30-40 minutes in the morning and again in the evening. •Avoid sleeping in the daytime.

It is very difficult to give a clear answer why it happened. But it appears the earlier treatment of giving two sulfonylureas (SU) together was a wrong approach. Ideally one of the SUs is given along with metformin most often. Secondly diabetes itself damages glomerular filtering membrane in the kidney, this leads deterioration of kidney function. That leads to increase in creatinine level, protein excretion etc. In such situation Insulin is the best medication for blood glucose control. Ideally fasting glucose should be closer to 100 mg, PP 160 to 170 mg & HbA1c%<7, then only all the long term complications of diabetes can either be postponed or prevented. Thanks.

If you have hypothyroidism then getting pregnant is difficult unless the condition is excellent control. When in good control tsh level should be between 3 to 4 milli units/ml. But for conception to occur tsh las to be between 0.3 to 2 milli units. Plus throughout the pregnancy also levels should be in the same range for fetal development and proper out come of pregnancy.

If your tsh levels are normal now, you have to concentrate on doing regular exercise for more than 1 hour daily. Brisk walking could be a good one or if you can join a gym do that and exercise under instructions. Secondly reduce your food intake by at least 20 % of whatever you are taking now. This schedule has to be followed meticulously. You can aim at reducing about 2 kg per month. Eat a balanced diet, avoid sweets, sugar, deep fried foods, if non vegetarian avoid red meat. Take more green salads. Hypothyroidism is a condition which has no cure except taking thyroxine tabs for life.

Causes and Symptoms of Blocked Fallopian Tubes

Certification in IVF & Infertility, Diplomate Gynae Laparoscopy, Royal College of Obstetricians and Gynaecologists (MRCOG), MD - Obstetrics & Gynaecology, MBBS
Gynaecologist, Ghaziabad
Causes and Symptoms of Blocked Fallopian Tubes

Blockage in the Fallopian tubes accounts for 40% of infertility in women, which is called Tubal Factor Infertility. Fallopian tubes are two thin tubes on each side of the uterus that aid the movement of mature eggs from the two ovaries to the uterus. Any kind of blockage in these tubes can prohibit the eggs from reaching the uterus and prevent pregnancy.

Symptoms

There are very few symptoms to detect blocked fallopian tubes. Some of the most common are :

  • A particular type of blocked fallopian tube is known as Hydrosalpinx, which causes pain in the lower abdominal or pelvic region with abnormal vaginal discharge
  • Painful periods can be a warning sign of blockage in the fallopian tubes
  • Painful intercourse is also a potent indicator
  • Spotting or heavy periods
  • Feelings of extreme tiredness
  • Pre-menstrual abdominal cramps

Once you know the symptoms, it is imperative to know the cause behind blocked fallopian tubes in order to get proper treatment.

Causes

  • Pelvic inflammatory disease (PID) is the most popular cause of blockage in the fallopian tubes. It can be sexually transmitted, but not all cases of PID are linked to STDs.
  • Prior abdominal surgery
  • Earlier ectopic pregnancy
  • Previous occurrence of ruptured appendix
  • Endometriosis
  • STD infections like gonorrhea and Chlamydia
  • Uterine infection due to abortion or miscarriage

There are precautionary measures that can be adopted to prevent blocked fallopian tubes, which can be opened with a laparoscopic surgery or a Hysteroscopic procedure. If you are diagnosed with PID, be sure to start your treatment as soon as possible. The most important advice is to look for symptoms of blockage and get professional help if you find any.

Can honey b taken by the blood sugar patient whose blood sugar often remains neat 150 mg?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Can honey b taken by the blood sugar patient whose blood sugar often remains neat 150 mg?
Mr. Lybrate-user singh, please explain the level mentioned 150 mg is it fasting sugar or pp or random. What is the hba1c%? because honey is another form of sugar, as it contains 50% fructose and 50 % glucose, like sucrose (sugar). Like sugar it also increases blood glucose level and fairly rapidly. A little bit of honey may be taken if blood sugar is in full control, with fasting around 100 mg and pp 150 to 160 mg, hba1c% <7. Thanks.

I'm loosing wait because m diabetic and using tablets how can I gain mussels and weight again?

MSc

I'm loosing wait because m diabetic and using tablets how can I gain mussels and weight again?
Hi lybrate-user to gain weight and balance your diabetes you need to combine a balanced eating pattern with regular physical activity. You should be eating at least 6 small meals a day, rather than sticking to 3 square meals. You could divide it as 3 main meals and 3 snacks. You need to be eating every 2-3 hours. Space out snacks during the day so you don't spoil your appetite. Bread, cereal, rice, whole wheat, dalia and pasta: biscuits, and corn-bread are good sources of carbohydrates. Fruits: dried fruits, fruit nectars, and large quantities of fresh fruits vegetables: include, potatoes, peas, corn in your diet. Meat, poultry, fish, dry beans, eggs, and nuts: beef, pork, lamb, poultry, salmon, omelets, nuts, peanut butter, kidney beans, chickpeas will help you gain weight. Include dairy products like milk, yogurt, and cheese: milk, fruited yogurts, hard cheeses, ice cream, puddings, custards, milkshakes. -nuts and seeds make a healthy option for snacks. -roasted channas and peanuts are good -peanut butter on the sandwiches are a great mid-meal choice. -you must know that cereals actually contain some healthy calories. Stay hydrated: drinking plenty of water and other fluids like juices, milk, soups etc, will work in your favour. Do not drink water before a meal and in between the meal as it may make you lose your appetite, thus calorie intake suffers. Sleep: at least 8 hours of sound sleep is a must. Since your body is actually building up muscles, while you rest. Weight gainers have long term side effects.

Mr. Lybrate-user, peanut chutney may not increase the blood sugar to that level. It is the other items that are taken during breakfast which are responsible. Peanuts give protein and fat, no sugar. You have to look at your diet in total and exercise done daily which should help in controlling blood sugar better.

If there is swelling it can be from uric acid deposition called gout and check your serum uric acid level.
