Good Morning Doctor, My question is to an Endocrinologist. My daughter has been diagnosed for hypothyroidism for the last 15 years and she is 27 now, Her Tsh is 3.6, T3 1.17 and T4 14.3. She is taking thyronorm 125g daily. Can the elevated T4 cause any problem Does she has to go for 150 from 125? Thanking you in advance. Ravikumar.

Mr. G lybrate-user, since she has hypothyroidism, thyroxine supplementation has to continue for life time. Slightly elevated t4 is fine. Main parameter for control of hypothyroidism is tsh level. This has to be between 3 to 4 milli units / ml. When on treatment. However, during pregnancy (if hypothyroid patient wants to have a child,) recommended tsh levels are between 2 to 2.4 milli units. In such a situation there may be a need to increase the does of thyroxine at that time. Some times if tsh levels show any upward level then there may be a need to increase. There are patients taking dose up to 200 mcg per day.

Don’t worry. You can lose weight easily if you are patient and determined to shed those extra kilos. You just have to alter your daily routine, watch what you eat and be physically active. This requires you to be on your weight loss commitment for quiet long. As soon as you start seeing the results, you will be surprised as well as motivated to go beyond your weight loss and adopt and a lifelong healthy attitude. For a start please follow: 1.Watch what you eat – Are you eating too much junk, drink excessive alcohol and eating too much white food and sugar? Please cut it down to as low as you can from tomorrow. Cut down tea/coffee if you are too addicted to it. It is creating havoc inside your body. 2.Lifestyle – Are you a late sleeper and late riser? You can’t imagine what this schedule of yours is doing to your metabolism. It is degrading your digestive system from inside and your looks from outside. 3.Physically active – If you are a couch potato who loves to watch tv whole day then throw that couch of yours in the street and get yourself a treadmill tomorrow. Yes it is as simple as that. Who is stopping you? You have to be physically active all day long if you want to burn those extra calories. Do daily brisk walk, Pranayam, atleast 20 surayanamaskar everyday to start seeing the results. Take break between office work to walk a little or rather just climb some stairs to get that food you ate in lunch have space and heat to digest and absorb. What’s the harm in using your washroom to do a little middle of the day squats. Do that – No one is watching you. The key is to stay physically active all day long, no matter what. 4.Eat healthy and eat on time: Not only eating healthy is important but eating at right time is equally imp too. Have an early breakfast and early dinner and have 4 small meals between breakfast and dinner. To guide you a bit, here you go: A.Breakfast (Between 8-10 a. M) – Have oats/porridge or brown bread with milk and nuts. B.At 11 a. M - Cup of Green Tea c.Mid day meal (around 12 p. M) – Have a bowl of fruits. Citrus fruits are best taken during this hour and absorb well in the body. Have oranges, amla, pineapple etc during this hour. D.Lunch (between 2-3 p. M) – Daal, roti, mix veg, buttermilk and salad. You can also take brown rice with daal in lunch. Try to get your food cooked in olive oil. Lace your salad with apple cider vinegar, black pepper and seeds like flaxseeds or chia seeds to make it nutrition packed lunch. E.Between 4 p. M – 5 p. M – Cup of Green Tea F.Evening (4-6 p. M) – Have some nice sprouts with tomatoes and cucumber. G.Dinner (2 hours before sleeping, preferably before 9 p. M) – Paneer & veggies salad with olive oil, Roti or brown rice with Daal. H.Have a glass of warm milk. Weight loss is 80% about what you eat and 20% about exercises. That said, both the things go hand in hand. If you want to lose weight in a more healthy way than be patient and just follow the regimen everyday. Slow and steady wins the race.

DEAR Lybrate USER. You have to follow some basic things so that you get better results in a short period of time. 1) take solid diet two times in a day and liquid diet two times in a day 2) drink only 100-150 ml of lukewarm water during meals . 3) don’t drink water before meal for one hour and one hour after meal 4) avoid oily, spicy, junk, fermented and stale foods. 5) avoid late night sleeping habits 6) avoid coffee and tea A) take 30ml of triphala juice and 10 ml of honey early in the morning on empty stomach and at night before sleep. B) take 2-2 tablets of lakshyadi guggul and vasantkusumakar rasa early in the morning on empty stomach with triphala juice C) take 10 gm of black cumin seeds along with 10 gm of jiggery after meals D) apply equal quantity mixture of fresh lemon juice and aloe vera juice on the roots of hairs Leave it for 30-45 minutes…. After that wash hairs with ROOTZ SHAMPOO (rajah ayurveda) do all things for 30 days. These things are much more effective. Consult to me for further suggestion. THANKS. REASON behind hair loss is increased pitta dosha (heat) in a rakta (blood) due to this roots of the hair get burned and hair fall started. To stop hair fall we have to remove increased pitta dosha from rakta by the root of STOOL, URINE AND SWEAT. And after that we have to increased strength of hairs by three ways. You are only 18 years old as of now. Don't worry your facial hair will grow. Beard is purely genetic make up. It could be a pituitary issue, hair follicle or their lack of, or could possible even be genetics. To make hair grow you need to stimulate, moisturize, and cleanse. Use a soft bristle scrub brush for the face and use circular motions using a eucalyptus, menthol, and/or peppermint type cleanser or lotion. The invigoration of the brush and cleanser/moisturizer will wake up that hair follicles and gives them a signal to produce. Make a mixture of Argan oil, Jojoba oil, sesame oil and olive oil and massage your beard and moustache daily to grow and maintain black and smooth hair.

Dear , this condition is known as planter fascitis. Just do 1 thing at home, take lukewarm water in a small tub and put a small teaspoon of salt to it. Dip your heel inside for 30mins. After that dry it properly n put a thin filament of any ointment like moov, volini etc. N wear socks so that the heat remains for a longer period n keep your foot under the blanket. Do it before sleeping for 7 days and then tell me how it feels.

Signs of Gynecological Cancer

Being diagnosed with cancer is something no one looks forward to and Gynaecological Cancer is every woman’s worst nightmare. However, if diagnosed in time, it can be treated. Cancer in any part of a woman’s reproductive system is termed as gynecological cancer. It is of five types:

  1. Cervical cancer

  2. Ovarian cancer

  3. Vaginal cancer

  4. Vulvar cancer and

  5. Uterine cancer

Paying attention to your body and understanding its natural rhythm can help you recognize signs of gynecological cancer. This is because signs of gynecological cancer can be difficult to identify unless you know your body. Here are a five signs to watch out for-

  1. Abnormal Vaginal Bleeding: Bleeding in between periods is okay once in a while but if it happens continuously over a period of 2 to 3 months, do not ignore it. Vaginal bleeding after menopause should also never be ignored. Abnormal bleeding could be triggered by a number of conditions including cancer of the lining of the uterus. Sudden changes in your menstrual blood that persist for over 2 cycles such as heavy bleeding can also be a symptom of uterine cancer. Heavy bleeding can be defined as a period that lasts for over 7 days.

  2. Changes in the Vulva: Any change in colour of development of bumps, sores of thickened skin should be immediately shown to a gynecologist. Any form of vulvar itching or burning should also not be ignored. These conditions could be symptoms of vulvar cancer. Hence, it is important to know what your vulva looks like normally.

  3. Bloating: Persistent bloating that lasts for over a fortnight can be a sign of ovarian cancer. This is especially true in cases of bloating accompanied by sudden weight loss or between-period bleeding.

  4. Change in Bathroom Habits: Due to the proximity of the bladder to the reproductive organs, a tumour or swelling in the reproductive organs can trigger symptoms similar to those of a urinary tract infection. This includes pain while urinating, difficulty passing urine, diarrhea or constipation. Urinary incontinence could also a sign of gynecological cancer.

  5. Abdominal or Back Pain: In rare cases, lower back pain could be a sign of ovarian cancer or uterine cancer. Pain in the lower abdomen or pelvic area can also be a sign of ovarian cancer.

The above could also be signs of other health disorders. The only way of knowing for sure is by consulting a doctor. Hence, understand your body and if you notice any of these symptoms consult your gynecologist at the earliest.

Dear lybrate-user, It is always better to do Free T3 and Free T4 rather than Total. The decision to increase or decrease Thyroid dosage depends not just on the lab report but on the status of symptoms. Other factors relate to status of other metabolic imbalances and the presence or absence of comorbidities. Management of Hypothyroodism should be comprehensive through an elaborate protocol. Prima facie I would say, if your energy leves are good, weight under control and moods quite positive, there is no need to change your present dose of Thyroxine.
