Madam five monthsplus baby may or maynot satisfy with breast milk alone. This can be best judged by weight gain of baby and demand of breast feeds. If twenty minutes of sucking should keep child playingfor four hrs. Means breast milk is enough. Same is fact for feed baby. 5-6 months is crucial to start semi liquid soup type of feeds two three times a day breast feed is continued at five hrs interval. Frequency of breast feed should never increased nor contd sucking beyond twenty minutes is advocated.

I have a girl baby who is 40 days old.Usually she is getting severe stomach pain and gas trouble Dr advised me to give her colicaid 2 to 5 drops according to the pain even though sometimes she is normal sometimes she is struggling a lot.Is it advisable to use gripe water for pain or can u give alternative medicine.

If baby is gaining weight about 500 gms per month & passing clear watery urine, your baby is absolutely fine. Just give him comfort, love & support. Be along with him during distress,i.e.Crying. We can not always stop a crying child, we can't know the cause every time. World over all babies cry for no reason, at some or other times. Colicaid may or may not help. Don't blame yourself for inability to stop cry. Things will settle in matter of few months. That is a part of life, we can't avoid. Being with the child supporting, comforting, showering love during distress is the only way. That is optimum & enough.

Diagnosis and Treatment of Valvar Aortic Stenosis in Children

Valvar aortic stenosis, commonly called aortic stenosis, is a disorder which occurs when the aortic valve of the heart becomes narrow (know Main Indicators of a Heart Disease in a Child). The narrowing of the valve prevents its full opening, and hence, blood flow from the heart to the aorta is restricted. When the aortic valve gets obstructed, the heart has to exert much more effort to pump the blood. The heart muscles get weakened as a result. This condition is more common in children.

Diagnosis of Valvar Aortic Stenosis:
Before diagnosis, we should know about the symptoms of valvar aortic stenosis in children. They include:

  • Feeling breathless
  • Angina or chest pain with a feeling of pressure or tightness
  • Syncope of fainting
  • Palpitations and enhanced heartbeats
  • A steady decline in regular activities and energy levels
  • Fatigue due to little exertion
  • Not gaining weight
  • Poor eating patterns
  • Problems in breathing

The wall of the left ventricle also thickens muscularly, and the thick wall occupies more space in the lower chamber of the heart and hence, the room for adequate blood supply is reduced. This may lead to heart failure.

Echocardiogram: The initial test that is recommended for patients with symptoms of valvar aortic stenosis is Doppler Echocardiography. This test enables the doctor to estimate the aortic valve region, peak or mean transvalvular gradients and the maximum aortic velocity. These primary measures are required to assess the severity of the disease. Echocardiography provides important information on the valve function, left ventricular filling pressure and disruptions in other valves.

Other major tests which help in the diagnosis of valvar aortic stenosis include:

  • MRI or magnetic resonance imaging, which provides images of internal body structures with great detail.
  • CT scan or computed tomography where three-dimensional images are extracted.
  • Chest X ray
  • Cardiac catheterization where a dye is used to highlight blockages, if any, in the heart.

Treatment: Usually, there are no medicines for fixing valvar aortic stenosis as the disease is not reversible. Medicines can be used to treat the problems, which are caused by the condition.

Replacement of the damaged valve by surgery is the best treatment for valvar aortic stenosis. The surgery performed is called valvuloplasty. This is a cardioscopic surgery where a tube with a small balloon is inserted into a vein. The tube is guided into the heart, and the balloon is inflated. The balloon and tube are removed after the valve is opened. The damaged valve is replaced by mechanical valve or the valve of a cow or pig by an open-heart surgery.

Valvar aortic stenosis is a severe heart condition in children, and the only permanent remedy for this disease is a valve replacement surgery.

My child (20 month old) suffering from loose motion, greenish with traces of blood, what should I do?

Stool mixed with blood is suggestive of infection. An antibiotic treatment is needed. Give plenty of fluids to avoid dehydration. Giving probiotic and zinc helps in early recovery.

Hello friends! I’m Dr. Usha. M. Kumar. I’m Senior Consultant and Gynae Endoscopic Surgeon at Max Super Speciality Hospital. I want to discuss one very common problem is fibroids. Because when I sit in OPD, I see many patients of fibroids every day and this is a problem which usually happen to 40+ of women but it can happen to the young girls as well and you will be very surprised to know sometimes the fibroids are there in your womb and you don’t even come to know and you are going to the doctor with some other problem and when we advise ultrasound and all of a sudden we see there are the fibroids. It can be totally asymptotic as well but sometimes, in the young girls, if they are having fibroids, they can have the problems.

First of all, I would like to tell you very important thing, if you have fibroids, there is nothing to be scared of. Because fibroids, these are non-fleshy and basically non-cancerous tumour. So, fibroids if you have, it can cause problems in the terms suppose if it is inner line of uterus, this can cause difficulty in conception and if it is in the outer wall and it is very big in size , it can cause some pressure symptoms like you may have only the back ache or might you have to go many times to pass urine so, frequency of urine is more because of pressure of that fibroid and sometimes, if it is in the inner lining of uterus or in the middle layer of uterus, it can cause heavy periods which is a very common problem I’m usually facing and most of the women are facing they come to my OPD so if, the young girls or married young girls say 22-30 , 22-40 if they are having fibroid and they are having problem in the conception, treatment is very simple.

Now, the medical technology has become so much advanced so with the very small equipment , we can take out the fibroids without making any cut in the body, so this we call endoscopic surgery and basically I’m expert of endoscopic surgeries and if this fibroid is big in size and it’s causing heavy periods and it’s more than 3cm, you have taken medicine, nothing is helping you then, the option is , if you want to save your uterus, we can only remove fibroid again with the help of key hole surgery which we also call minimally invasive or laparoscopic surgery. So, it hardly makes , 0.5cm or 1cm of cut, we can take out any size of fibroid.

Only thing we have to use morcellator which can cut down the big size of the fibroid in the small pieces and I mean it’s a very wonderful surgery and people are surprised to know up to 8cm or 10 cm of fibroid can be removed with laparoscopic surgery. I would like to give you very important tip, fibroids, most of the time, they are genetically inherited and if your sister has got fibroid, or your mother has got fibroid then, you have little higher possibilities to have fibroid but, of course, the most important thing for all girls, all women, is taking balanced diet, taking more and more antioxidants in your food and do lots of exercise. My daughter is 40 days old. She is not sleeping properly. She sleeps for hardly half n hr n then wakes up crying. She keeps on putting pressure downwards even while sleeping. She's passing urine n stools properly. She's also throwing milk our of her mouth even after burping. Kindly suggest what to do?

Hi I guess her sleep is interrupted during night only. You just check whether her nostrils are blocked or not, passing urine or stool and become wet or not, because these are the common cause of discomfort during sleep. Regarding pressure this is normal if she is on exclusive breast feeding. Regurgitation of feed 2-3 times a day is normal. Proper burping can reduce the freguency of regurgitation.

My daughter was born on 7-12-17. At the time of birth her calvical born got fractured. Now after a month she is moving her arm from shoulder once in a while but she is not moving her arm from below elbow.

Dear madam, your baby's fracture will settle by itself. But she is likely to have nerve palsy additionally. So immediately consult an orthopedic or neurosurgeon doctor. Confirm the diganosis and start physical therapy. Wish her speedy recovery.
