Yeh chaar ligaments hotey hain hamare knee mein, do bahar ki taraf aur do andar yaney agey ki taraf ek ligament jise hum ACL kahte hain aur pichhe ki taraf ek ligament jisey hum PCL kahte hain. Sath hi sath humare ghutne mein do meniscus yaney shock absorbers hotey hain ek andar ki taraf aur ek bahar ki taraf. To yeh ligament injuries usually accidents, siriyo se girne se ya phir koi aur injury ki wajah se yeh ho sakte hain. Jab hum doctor ke paas jate hain to X-rays normal hote hain phir bhi ghutne mein sujan bani rehti hai, chalne mein taklif hoti hai, lymph hota hai aur ghutna kabhi kabhi locked rehata hai. Aise injuries ko hum log MRIs pe detect kar sakte hain. 

Ye ligaments injuries ko hume treat karne ke liye orthscopic surgeries ki jaroorat kabhi kabhi par sakti hain. Kuch ligament injuries hote hain jo apne aap hi heal ho jate hain. In mein se sabse important injury ACL ligament injury jo agey ki taraf yani anteriorly rehati hai. ACL ligament injuries ko usually young patients mein surgery se treat kiya jata hain. Kuch patients ko physiotherapy se bhi yeh thik kiya ja sakta hain. Lekin unhe bahut lambi mehnat ya physiotherapy ki jarurat par sakti hain. ACL surgery jo hoti hain wo orthoscopic surgery yane durbin ki surgery se hota hain. Usme humlog andar ki taraf se ek snayu nikalte hain jise humlog hamstring kehte hain. Hamstring graft ka hum log artificial ACL banate hain aur usse ghutne ke andar hum log orthoscopic surgery ke through daal sakte hain. Yeh ek bohot hi common surgery hai. Aur ye surgery ke baad mein jo recovery hoti hai bohot hi fast aur achhi hoti hai.

Kabhi kabhi ACL ligament ke saath baki ke tino ligament mei se ek ya do ligament bhi injured ho sakte hain. Isse complex injury bhi kaha jata hain aur in complex injuries mein bhi surgery ki jarurat par sakti hain. Kabhi kabhi ACL ligament injuries ke saath mei yeh jo meniscus hote hai yani shock absorber jin ka bhi injury ho sakta hai. Orthoscopic surgery se ye jo meniscus ki injuries hoti hain inka bhi illaj bohut achhi tarah se ho sakta hai. In sab injuries ke baad mein physiotherapy ka bohot mahatwapurna role hota hai jismei physiotherapy karne se dhire dhire dhire recovery aati hai aur patients apni normal zindagi mei phir se laut sakte hain.

Anterior Cruciate Ligament Injury - Everything You Should Know!

Anterior cruciate ligament is one of the important ligaments joining the thighbone (femur) to the leg bone (tibia). In general, ACL injuries can vary from minor injuries, such as small ligament tears, to more serious cases like complete tears or fracture of bone attached to ACL.

These injuries usually occur during sports activities, like soccer, basketball, football, gymnastics, tennis, and volleyball or due to road traffic accidents or severe fall/twisting injury etc. An untreated ACL injury can lead to a condition called an ACL deficiency. In some cases, the cartilage covering the ends of the bones may get damaged or tearing and trapping the cushion pads (meniscus) supporting the knee joints. This eventually leads to damaged knee joints.

Symptoms:
Signs and symptoms of anterior cruciate ligament may include:

  1. You experience feelings of instability in knee or loss of balance while running or jumping.
  2. You experience immense pain while walking up and down stairs.
  3. You are unable to engage much in physical activities like running and jumping.
  4. Your knee begins to swell gradually.
  5. You can sense a loud popping sensation when moving your knee.

Causes:
Anterior cruciate ligament injuries generally occur during sports. Here are some common possible instances that might happen during a game:

  1. Sudden arrest in movement like running.
  2. An unsteady landing from a jump
  3. Direct blows to the knee from tackles
  4. Sudden shifts in direction

Treatment:
Treatment of ACL injuries normally involves the following:
1. Immediate medical attention along with intense first aid care.
2. Sincere adherence to the R.I.C.E model of self-care:

  1. Rest: proper rest for a couple of weeks or months depending upon the severity of the injury
  2. Ice: using ice packs to help cool the nerves
  3. Compression: ice treatment further helps in compression
  4. Elevation: when lying down in bed, it is strongly recommended that you keep your affected leg at a slightly higher position, through the use of pillows or cushions

3. Anterior cruciate ligament surgery or reconstruction

  1. The surgery is done arthroscopically. It involves taking a hamstring tendon (tissue) around the knee and creating a new ligament in the knee. The new ACL is fixed with the help of bio-absorbable implants outside the knee joint.
  2. Patients walk from the next day and are independent. With the help of new age techniques, it is almost a painless surgery with prompt recovery.
  3. They have to undergo rehabilitation under a trained physiotherapist for 3-6 months. Usually, patients can join their respective jobs at the end of 2 weeks.

Usually, sportspersons can resume sports activities within 6 to 9 months from the date of surgery.

Rotator Cuff Tears - Know The Options Available!

Rotator cuff is the name given to four muscles and their tendons, which keep your arms in your shoulder sockets, allowing you to lift your arms and do day-to-day stuff like combing your hair, cutting vegetables etc.

The shoulder joint is a very important joint of the body and this is why any tear that happens in the rotator cuff can be devastating.

Causes

Tears can happen in rotator cuff due to a number of factors like injury and degeneration due to age. Tears that happen due to an injury to the shoulder are called acute. Degenerative tears are tears that happen due to aging of the tendon over time.

Common causes of degeneration are lack of proper blood circulation, which impedes the rotator cuff’s ability to repair itself. Repetitive stress is another cause where we stress the tendon by repeating the same shoulder motions again and again while playing games like tennis, cricket etc. and doing jobs that require us to extend our arms upwards a lot. Lifting heavy objects can also lead to tears.

Symptoms

Chronic shoulder and arm pain are good reasons for you to see your doctor. If you suffer from pain in your shoulder at rest, while sleeping on the affected shoulder or pain while lifting and lowering your arm, then there is no time to wait.

The doctor will do a physical examination by moving your arm in different directions. He may follow it up with X-rays and MRI to check how big the tear is in your rotator cuff.

Treatment

Once confirmed that you indeed have a rotator cuff tear, your doctor will start the treatment. The goal of any treatment is to reduce pain and make your shoulder joint work properly again.

Nonsurgical Treatments: 

Nonsurgical treatments include rest, using of a sling to protect the shoulder and modifying your daily activities. This means you may not be able to do activities that cause shoulder pain. Your doctor can also prescribe pain and anti-inflammatory medicines. Strengthening and physical therapy follow after pain relief of this type of treatment.

What you must remember is that nonsurgical treatment relieves pain and improves function in the shoulder, but does not improve shoulder strength. The size of your tear may also increase over time.

Surgery

Surgery, which attaches the rotator cuff to your bone, is done if you suffer from continuous pain and symptoms. If you have a large tear, conservative treatments will not work and surgery will have to be resorted to. If neglected, over a period of time the tear may become larger leading to further pain and difficulty in shoulder movements.

If you have been suffering from chronic shoulder or arm pain, waiting and watching is definitely not an option. You must consult an orthopedic surgeon preferably a shoulder specialist at the earliest.

Shoulder replacement - Know all about it!!!!

A shoulder replacement surgery is performed when your shoulder is so seriously injured that it has to be changed for normal functioning to continue. It may also happen when the bones of the joints are so badly broken that they cannot be fixed in a way that your shoulder can function properly. Apart from bone fractures or sudden injuries, some diseases like crippling arthritis can require you to undergo this surgery. It should only be done if the condition cannot be fixed with the help of physiotherapy, exercise or medication first.

It is advised that patients undergo this surgery with shoulder surgeons trained for this surgery.

Types of shoulder replacement surgery

There are several types of shoulder replacement surgery. They are-

• Total replacement surgery - This surgery is more commonly done than the other types. The joint at the shoulder is a ball and socket joint, meaning a ball-like bone fits into a socket like joint. In this surgery the ball is replaced with a metal ball and the bony socket with metal socket with a plastic component in between.

• Partial replacement surgery - In this type of surgery, only the bony ball is operated upon and replaced with a metal ball.

• Reverse replacement surgery - In this procedure, a bony ball is replaced with metal cup and the bony cup is replaced with metal ball i.e the alignment of the joint is reversed. This surgery is done in elderly patients with arthritis due to neglected rotator cuff tears or in shoulder fractures which badly broken bones. These patients usually have very little movement in the shoulder

Things you can do before surgery to facilitate the process

• You should get diagnostic tests done well in advance, like X-rays and related imaging.
• You should cut down on the level of your addictions, like smoking and drinking.
• The doctor will ask you to reduce intake of certain medicines like painkillers.

Recovery after shoulder replacement

  1. The stay in hospital is usually 2-3 days. Advanced pain management techniques are used nowadays for virtually painless experience after surgery.
  2. Usually shoulder movements are started after pain relief.
  3. Patients are able to do daily activities in 3 weeks but lifting hand overhead may take some weeks thereafter.
  4. You should ask a friend or relative to help you with daily tasks around the house, and for transport for the first few weeks.
  5. Supervised rehabilitation under an experienced physiotherapist is a must.
  6. Many patients gain near normal function and are able to do daily activities of living.
  7. You will need to change your daily routine a bit, like avoid putting heavy strain on the shoulders or lifting weights.

Get an x-ray, blood investigation and if required MRI and consult spine surgeon. Night pains are a symptom of bone tuberculosis also. When ruled out then you can treat dietary deficiency.

As you are breast feeding you should not be taking pain killer medicines. Try to take back support while sitting, learn to breast feed the child while lying down on the side, apply ice packs to the back and ask someone to gently massage the back and relieve the spasm. This will help in about a month's time. Especially best feeding in lying down position lying on one side.

I am having back ache in lower part since last 3-4 months. There is no issue found in the X ray. But the pain is unbearable at times and I am not able to stand up or take a turn. I get up from bed in a tilt position because I am not able to stand straight in the morning. The pain is persistent and has cycles of high and moderate. I took physiotherapy sessions 2 months back for sometime and it appeared to be fine during that time but recurred when I left those sessions. Please advice.

Shoulder Pain - 5 Common Causes Behind It!

The shoulder is one of the most important joints in the body that uses a ball and socket to join the arm to the rest of the body. Any pain can make it difficult to carry out motion in a comfortable manner. The shoulder consists of the long arm bone called the humerus, the collarbone or the clavicle, as well as the shoulder blade called the scapula. A layer of cartilage provides essential padding to the bones in this area and its two main joints. There are a number of issues that can cause shoulder pain.

Let us get to know the six most common ones:

  1. Rotator Cuff Injury: This is the most common cause of shoulder pain and it occurs when the four muscles or the tendons are injured. These muscles and tendons usually control the movement of the arm. This injury can involve strains and tears which may also be caused by constant lifting of heavy objects.
  2. Rotator Cuff Tendinitis: This condition affects the tendons and muscles that help in moving the arm freely. This usually happens due to inflammation in the tendons. It is commonly experienced by patients who are actively involved in sports and other heavy physical pursuits in their line of work.
  3. Frozen Shoulder: This is a debilitating condition that restricts the free movement of the arm and is also known as adhesive capsulitis. When the shoulder tissues become too thick, it leads to the growth of scar tissue that hinders proper movement and causes pain.
  4. Osteoarthritis: Osteoarthritis is a condition that comes as part of aging bones that also become hollow and brittle over the passage of time, which leads to joint and bone pains in various parts of the body. It is a degenerative disease that is caused due to prolonged wear and tear as well as other factors including family history and sports injury.
  5. Tendon Sheath Inflammation: The fibrous tissue that connects the bones and the muscles is called a tendon. The presence of the tendon makes it possible to pursue activities like running, jumping, lifting, gripping and more. It is protected by a sheath known as synovium, which also lubricates the same. Any injury or tear to this sheath can cause shoulder pain.

Acute and persistent pain that does not abate after 2 weeks or gets even worse should be treated with immediate medical intervention. Kindly consult a shoulder surgeon for further evaluation.
