Dear, what you are suffering from is a complication of diabetes called diabetic neuropathy. Your lethargy is also probably due to uncontrolled diabetes. So the first step for both the conditions is to treat your diabetes first .

My wife aged 62, diabetic-150-160, for 15 + years, taking glucobay 25 mg x 3, taking vit d+ iron+calcium. Has slight protein release in urine. So taking pressue tab 5 mg and cholesterol as well present problems - just after going to bed, after 3 or 4 hrs, every night without a miss, she experiences pain in the left side leg muscles. So loosing sleep for 3-4 hrs. So to compensate sleeps until 9-10 am. Result the whole house is awake. It was diagonised as" pvd" a scanning was done and it was concluded that the problem was due to poor blood circulation. Guide me to the extent which specialist or treatment to be followed. Patient is suffering for more than 5 years.

You have provided some details about your wife's condition but certain other details are also needed, which I am mentioning. Please let me know her hba1c level and is she taking any thing else for diabetes other than glucobay? because only glucobay does not control sugar level. Furthe what is her lipid profile? and which drug is she taking for that? then which 5 mg tablet she is taking for her high blood pressure? if she has been diagnosed to have pvd which means blood supply to that part of the lower limb is affected. This can be corrected by vascular surgeon and proper blood supply can be restored. That is the best option for her pain relief and getting undisturbed sleep. To get better control of diabetes she may have to take additional treatment.

Sir/mam, sometimes I feel a pain on my neck. Kindly tell me the possible reasons. Thank you.

Physio and Yoga are mechanical approach to strengthen muscles and relieve the compression on nerves. Ayurveda gives strength to nerves, muscles and are 'Vaatnashak' i.e. corrects vitiation of Vata. Quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day.

Hot fomentation x twice daily. Neck exercises. Neck stretching. Postural correction. Shoulder shrugs. Core strengthening exercises. Take frequent breaks at work use cervical pillow. Use back support. Self massage the back of neck.

Dear Lybrate user use soft inner sole sandle avoid bare foot walking, do stretching exercise of calf muscles and hot water and cold water application 30 minutes alternately repeat 10 days.

I am 24 yr old male and have pain just to the left of spine. Consulted Ms. Ortho 4 times and he said it was because of spinal tb, but quanteferon-gold tube test came negative. Is it belongs to bone disease or any thing else?

Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again. Do not ignore. It could be beginning of a serious problem.

Case 1 : A 65yr old women develops severe pain in the lower back while trying to lift her grandson from the floor. She was then rushed to the hospital and on investigating was found to have osteoporotic collapse of lumbar vetrebrae.

Case 2 : A 72 yr old female sustained a small twisting jerk while walking over the uneven surface and developed sharp pain in the hip region followed by difficulty in walking and when examined was found to have fracture in the hip.

These cases are just to exemplify that Osteoporosis may not cause any apparent symptoms. Patients may not know they have osteoporosis until they break (fracture) a bone.

Osteoporosis is defined as a systemic skeletal disease characterised by low bone mass and microarchitectural deterioration of bone tissue, with a consequent increase in bone fragility and susceptibility to fracture. Although the diagnosis of the disease relies on the quantitative assessment of bone mineral density, which is a major determinant of bone strength, the clinical significance of osteoporosis lies in the fractures that arise.

Common sites for osteoporotic fracture are the spine, hip, distal forearm and proximal humerus. The remaining lifetime probability in women, at menopause, of a fracture at any one of these sites exceeds that of breast cancer.

Osteoporotic fractures are a major cause of morbidity in the population. Hip fractures cause acute pain and loss of function, and nearly always lead to hospitalisation. Recovery is slow, and rehabilitation is often incomplete, with many patients permanently institutionalised in nursing homes. Vertebral fractures may cause acute pain and loss of function but may also occur without serious symptoms. Vertebral fractures often recur, however, and the consequent disability increases with the number of fractures. Distal radial fractures also lead to acute pain and loss of function, but functional recovery is usually good or excellent.

A dual energy X-ray absorptiometry (DEXA or DXA) scan can be used as a screening test for osteopenia (bone loss that precedes osteoporosis). This test measures bone density in the hip, wrist and spine and is more precise than an X-ray.

The National Osteoporosis Foundation recommends the following groups of people should have Dual energy X-ray absorptiometry (DEXA or DXA) scans to screen for osteoporosis:

All women age 65 and older

All postmenopausal women under age 65 who have risk factors for osteoporosis

Postmenopausal women with fractures

Women with a medical condition associated with osteoporosis

How Is Osteoporosis Treated and Prevented?

There is no current cure for osteoporosis. Osteoporosis treatment involves stopping further bone loss, and strengthening bones that show signs of weakness. Prevention of osteoporosis is key.

Prevention and Treatment:

1. Exercise : Exercise is important in helping improve muscle strength and balance. Consult your doctor for the type and duration of exercise that is right for you

It is also important to consider other medical problems that may also be present (heart disease, diabetes, high blood pressure) before starting any exercise program.

2. Quit Smoking and Curtail Alcohol

3. Calcium Supplements

4. Calcium-Fortified Foods

5. Vitamin D


Treatment: Medications

There are several types of medications used to treat osteoporosis.

Antiresorptive drugs

Menopausal estrogen hormone therapy.

Selective estrogen receptor modulators (SERMs).

Anabolic drugs: these are the only drugs that actually build bone mass. Teriparatide, a form of parathyroid hormone, is one example of this type of drug

You can help your back get back to normal by being extra careful about how you use it and doing a few easy, sensible exercises. -stay conscious of how you bend and lift: start the movement from your knees, not your back, when you lift baby, laundry, stroller, etc. -don't hunch over when you're feeding or cooing to your baby. A straight spine is a happier spine. -use a footrest to elevate your feet slightly when sitting. You deserve to put your feet up anyway! -try not to stand for long periods of time. Whenever possible, place one foot on a low stool when standing in order to take some pressure off your lower back. -do hot water fomentation for back pain. -weight management is another important thing for low back and knee pain. Yes we do have procedures to regenerate cartilage, but I have to assess clinically and imaging may b required for verifying before contemplating any procedure.

Actually pain at calf/leg region can be due to many reasons, commonly it is muscular in nature but it can also be due to tendons, nerves, blood vessels etc. Also pain can be associated with off and on stiffness. My advise to you for initial 2-3 weeks will be: - ice packs application intensively with elevation of your limbs at night - take a short course of anti-inflammatory medication as per your body's suitability - stop your physical activity till pain completely subsides - intensive physical therapy, training and proper muscle conditioning once pain has subsided - slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - we will get some blood tests for you to be done preferable empty stomach - cbc, esr, crp, serum vitamin d and serum potassium levels, thyroid profile once you have all these reports then you can contact me as I can advise you for proper supplementation do not hesitate to contact me if you need any further assistance.
