I can not ask anything better than him. He is a very positive and soft spoken doctor. Always tries to encourage patient. My wife is a case of Anky loosing Spondylitis from last 8 years and unable to live normal life. May to December 2015 was worst phase of her since she had almost lost mobility and infact was unable to sleep entire night because of pain and stiffness in entire body. We consulted Dr. Nilesh in December 2015 and then everything started turning positive in our favour. The oral medicine SAAZ 500 which was never suited her body due to Gastric issue, started giving positive results without side effect. She was given Anti TNF very safely and that changed her life. She is living normal life now and presently undergoing taper course of Anti TNF. Oral medicine like Saaz and Nucoxia is not giving any side effects now. Thank you for giving her life back .

I was treated for Ankylosing Spondylitis under Dr Neelesh, while he was undergoing DM at KGMC Lucknow. My problem is under control. His knowledge in Auto immune diseases and Rheumotology is immense. His communication with patients is worth emulating by other Doctor. Lots of patients miss him at KGMC. With his help lots of patients are living pain free life. Apart from good Doctor he is a great human being.

As told earlier many a times it could be due to vitamin d deficiency in growing children. You can get her tested or just ask your doctor to give her empirical vitamins d supplements, which are pretty safe. Hope that helps. Regards.

Hi gout is a very severe disease, it affects joints, heart and kidneys. Most people forget that uric acid which deposits in joints can deposit in heart and kidneys too. So basically besides most painful joints it can kill also. Also as disease progresses the arthritis spreads and can have a devastating effect (google gouty arthritis, gouty top, gouty joint, gouty erosions images). The treatment is very simple, but to be taken continuously. Also it is a widespread misconception that you should stop dals. Dals are very healthy source of protein, and they need not be stopped in gout. Gout is a very severe form of arthritis. It has acute and chronic phase. In acute phase requires immediate management in the form of certain drugs. Chronic phase requires treatment to prevent attacks and later lower uric acid. Most doctors treat intermittently, which is why it spreads, you should treat it continuously like diabetes or hypertension. The concept is simple keep uric acid below 6 or in severe cases 5. It can be achieved in most patients by a single tablet. Bringing uric acid from 10 to 7 is of no use because uric acid will keep on depositing as long it is above 6 Attacks go gout can be treated with any form of therapy. But chronic gout requires a simple tablet in allopathy (like a single tablet in patients having high blood pressure), to prevent further attacks and prevent heart and kidney disease. No drug in ayurveda can do that.

Its always good to have milk for bones. But if knee is poorly affected get it replaced, that can give you a much better quality of life. The more you exercise and have dairy products the healthier your bones will be. For adequate exercise it might be good to consider getting your knee replaced.

It all depends on how bad is your hip. If your hip is irreparably damages, infliximab will not help. Infliximab will only help to reduce the swelling and associated pain, damage already done cannot be reversed, if pain is persistent then operation is the only option. Consult your doctor.

I am suffering from rheumatoid arthritis (32.1) and have anti CCP value 51 since last 4 months. Hi lybrate-user you are on good treatment, though google might show you side effects of these medicines, they are extremely rare and you will be regularly monitored for same, if there is any indication medication will be changed by your rheumatologists accordingly. Unfortunately ra most times is a life long disease in more than 95% of cases, so treatment is usually continuous like diabetes and hypertension. The side effects and damage caused by not taking medicines is much more than taking medicines. If you don't keep ra well controlled by taking medicines regularly then it can be life threatening also. Don't think that you are having side effects by reading google, consult and talk to your rheumatologist. As I said the side effects are very rare. Usually ra medicines are safe with minor issues for prolonged periods even 20 years safety of continuously taking these drugs has been documented.

Yes it is safe. Its non veg sources which increases uric acid due to high purine content, high purine is to be avoided in gout, not high protein which is a misconception widespread even among doctors. But gout remains silent for many years before coming again. Specially if your uric acid remains above 6. Was the first episode really gout? increased uric acid doesn't mean its gout.

But still suffering from disease And now my ankle hips and knees fusion. From last two month I am taking these tablets Sazo 1g twice a day Calten d 500 twice a day Retoz 90. Once a day Folitrax 7.5. Twice a week Folvite 5 twice a week Uprise - d3 60 k once a week Depo -medrol once a week And now my condition is very very chronic . Very high pain in right hip And high pain in ankle, knee, wrist, shoulder and neck. Now I am unable to stand so laying on bed and crying all the day and night. Do note one thing when I take these medicine I feel better for 1 month but after one month pain increased very high level . Before 2 years I took these medicines and then I felt better for one month after that pain increased. So I stopped these medicine and got ayurveda treatment for 2 years but didn't get relief. So I again consulted to rheumatologist and he prescribed me again to take these medicine and now again 1 month relief and after that pain is very very high. Sir why these tablets give me relief only for one month. Is any medicine not suits me. Or I should reduce dose of tablets. Or tablets are not right choice for me. Because pain is so so high.

Hi lybrate-user sorry to hear about you. But if your joint are fused then replacement of hip or knee might be a better option. The medicatios seem to be good but if they are not working ask your rheumatologist to try anti tnf injections, might be a bit expensive but worthwhile.

What is arthritis ? What is Rheumatology? Who are Rheumatologists ?

DM - Rheumatology, MD - General Medicine
Rheumatologist, Mumbai

Rheumatology is a field of medicine which deals with 'Auto-immune' diseases.

'Auto-immune' means diseases in which a part of your immune system, which usually protects you, gets automatically hyper activated and starts damaging your own organs.

All organs can be affected with rheumatic diseases - Joints (most common), spine, skin, muscle, eye, lungs, kidney, blood etc.

Because joints are most commonly involved, Rheumatologists are also called Joint / Arthritis.Specialists/ Immunologists. They are detectives who can detect more than 50 types of arthritis.

Rheumatic diseases can be very serious. They can affect children and young people also.

Timely treatment is necessary to prevent joint damage and even at times save lives.

(Rheumatologists are not orthosurgeons, they are physicians)

Hello Dr. My mother (age - 56 ) is suffering from joints pain and muscles pain since last 2-3 months. The pain is unbearable. She can't walk without holding any support and even can't stand for sometime. Some times she becomes aggressive n speaks very harsh words bluntly. She often forgets many things she has done or spoken. She has thyroid disorder since last 15 years n she was taking a tablet (Thyroxine Sodium Tablets IP, THYROX - 25 ) regularly in the morning empty stomach and she was doing very well with this medication. Now, 3 months before when she had these types of pains we consulted our family physician where her routine checkup had being done on regular basis as directed by the physician. We got some blood tests like Haemogram report, Renal Function, Arthritis Profile, Thyroid Function Test done for it. And the results says : Hemoglobin : 11.3 gm% R.B.C. : 4.18 mill./cu. Mm. W.B.C. : 7820/cu. Mm Platelet. : 3,06,000/ cu. Mm. polymorph. : 70% Lymphocytes : 25% eosinophil. : 02% monocyte. : 03% basophil. : 00% P.C.V. : 35.1% M.C.V. : 83. 97fl M.C.H. : 27.03 pg M.C.H.C. : 32.19% R.D.W. : 14.9% E.S.R. (Automated ) After one hour : 90 mm Serum Creatinine. : 0.99 mg/dl Uric acid : 6.41 mg% RA Test (Quantitative ) : 109.9 IU/ml Serum FT4 Estimation : 14.12 pmol/L Serum U/S TSH Estimation : 12.30 ulU/ml Our family physician proscribed the below medicines for it - Tab Thyrox 50 mcg 1-0-0 empty stomach Tab HCQS 200 (10) 0-1-0 Tab Etova-P 400 (30) 1-0-1 Tab Imutrex 7.5 (4) 0-0-1 every Friday Tab liquid-D3 (4) 0-0-1 every Friday She had completed the course of the above medicines. But there is no result. She has continue her Thyroid Tab as the physician said. We had also consulted other physicians. Some of them said the arthritis problem is because of Thyroid disorder, if she takes Thyroid Tab regularly n do routine checkup regularly then everything will be fine after few months. Some said she needed to take the Thyroid Tab of 50 mcg when she was taking it of 25 mcg. But our family physician never proscribed the 50 mcg Tab before this. Now here is my question Dr. Is/was my mother going under correct/right medication. Is her Thyroid Tab of 50 mcg is okk according to her report I've mentioned above. Shall we go for Ayurveda or naturopathic treatment for her arthritis problem. Give some suggestions for the problem. I will be thankful to you if you can proscribe some medication or remedies or any other treatments for her. Thanks a lot. Regards, Hiral Patel.

Hi hiral first of all you hv given a really comprehensive history, complements ob that. Your mother is suffering most likely from rheumatoid arthritus seeing her high titres ib ra report and history you gv. Immutrex and hcqs are goid tablets but they start working a bit late and immitrex need to be increased gradually. Initially you need to give some dose of low dose steroids to make the former medicines work more effectively. Normal physicians might not be well trained to treat ra, cobsult a rheumatologist of possible, remember natiroapthy and ayurveda does not lead to complete relied unlike allopathy and if you are late joints might undergo destruction nothing will work after that. Many a times naturopathy and ayurveda gives drugs containing gold and lead, steroid powder you can never check because most of time they are self made powders, we do use steroids for ra in allopathy but most of time its low dose, pt knows abt it and we try to stop it as soon as pt gets totally controlled. We monitor regularly also. Also ra is usually a chronoc diseaae, faster you act lesser the meds required in future.

Steroids in low doses are good to control arthritis, later they are tapered and slowly stopped. Meanwhile other drugs start taking effect. This adjustment can only be done by trained doctors, that is rheumatologists. Kindly consult one. Also will depend on type of arthritis. There are more than 20 common types.

Arthritis related enthisitis is curable? What are the methods to diagnosis the disease?
It's enthesitis related arthritis, era, there is no definite test, but a rheumatologist makes diagnosis clinically. He uses a combination of clinical findings, mri findings and hlab27 tests, none of tests is confirmatory in isolation. Era has variable course, it may persist, go into spontaneous improvement or patient may have recurrent attacks during his life time. It's difficult to predict course, but it's controllable with meds. Consult rheumatologist.
