Dry Eyes - What Are The Symptoms?

 

Hello, 

I'm doctor Jatin Ashar practicing ophthalmologist. My areas of specialization include cataract, cornea transplant, Laser refractive surgery and dry eyes. Today I'll be speaking to you about dry eyes, we often see young people who come to us saying that their eyes are gritty, itching and they always feel tired at the end of the day after the long hours of work on the computer so what is it that leads to this problem or dryness in their eyes? quite often we have seen that patients especially young people who are sitting on long hours on the computer often complain of their eyes being tired after three to four hours of computer usage, this condition is called dry eyes. In case of dry eyes what we see is the patient would complain of irritation in their eyes, itchiness, dryness or grittiness, tired feeling and often even decrease in their vision.

These patients may often complain also of nighttime blurriness of vision also difficulty in reading, halos around light and a lot of times they may find it difficult to read or drive, especially at the end of the day. Dryness of the eyes is not just related to the computer use there are multiple other reasons of dryness of the eyes, there are certain glands that are there on our lid margin these are called meibomian glands, they have a secretion which is similar to a lipid or oily secretion in the eye, this leads to the protection of the layer which is your tear film and this prevents evaporation of the tear film, it is seen there are a lot of people that have got blockage of these glands called meibomian glands dysfunction. This blockage leads to evaporation of the tears from the eyes and leads to dryness of their eyes.

Other reasons of dry eyes include poor tear production this condition is called sjogren's syndrome and it is quite often associated with other eye diseases or systemic diseases such as rheumatoid arthritis or other collagen vascular disorders wherein people may experience even dryness in their eyes along with dry mouth and this leads to poor swallowing action in these patients and also poor clearing in their eyes. The treatment for dry eyes is in the form of drops and oral medications. For computer users a simple rule which is called 20/20 rule is that what is prescribed to them, that is after every 20 minutes of constant computer usage a 20 second break is what is warranted wherein people should not be using the computer or their mobiles or any near activity should not be done, in these 20 seconds they can gaze at something that is distant or close your eyes for any second, this would lead to the prevention of the dryness in their eyes, also frequent blinking of the eyes has to be voluntarily done this leads to spreading of the tear film along the ocular surface and prevents dryness of the eyes. When patients cannot have any relief using this it is often prescribed that they start using lubricating drops.

These drops will smoothen their ocular surface and their eyes and decrease the dryness. Often when the patient also have a decrease in their vision along with dryness and redness of the eye inflammatory markers are tested in the eyes using special tests. Once they are seen to be positive on these markers, anti-inflammatory drops such as topical steroids or oral anti-inflammatory agents are started for these patients, often these lead to relief of the dryness symptoms. It is important to note that the patient may be required to be on a long term follow-up and long term medications for dryness may be required for most of these cases. Apko aankh mein cataract hai, motiyabind hai? hai. So cataract aur motiyabind what people called in colloquial language is the most common cause of poor vision in elderly population so what exactly is cataract? like a camera human eye behaves in the same manner, in the camera there is a lens that focuses light onto film, same way in human eyes have a lens which is transparent when we are young and this is the lens that focuses light to reach the retina. As we age this lens starts to become solidified and this is what leads to the formation of cataract, so with cataract there is scattering of light and because of that early symptoms in the form of glare or in the form of halos starts to appear, this is what people start experiencing when they have an early cataract, slowly their vision starts to decrease and over a period of time they start seeing things dull and eventually they start losing their vision.

The treatment for cataract is quite simple, it is a simple surgery which is done using a special machine called phacoemulsification, it's no longer like what we used to have in the older days that the cataract surgery required, the cataract to become extremely mature, now even an early cataract can be operated with new technique called as microincision cataract surgery. In this procedure, what we do is make a small incision which is only 2 millimetres in size and using a special device or phacoemulsification machine we give longitudinal or transitional ultrasound energy that breaks down the hard cataract into small pieces which is then sucked into the machine. Using the same 2 millimetre incision an intraocular lens which is folded on itself is implanted in the eye, this lens takes the place of your original lens, so in short what is done during cataract surgery is a small nick is made in the side of the eye and using this nick a special machine enters the eye that removes the cataract and then we place a new lens inside the eye using the same incision, it is now it is a new lens makes you see everything clearly.

So in today’s era we have different types of lenses that are available to us in the cataract surgery so broadly what we use are two different types of lenses, one is called monofocal, other is called multifocal lens of focal lens. So what does a monofocal lens do? As the name suggests it has only one focus so patients would be independent of their glasses for distance, but for reading activity one would need to wear glasses; however, if one wants to opt to be independent of glasses for both distance and for reading, one may choose multifocal IOL. In today’s era cataract surgery is not just what is common to everyone, what is done is customized cataract surgery wherein we do a series of tests before the cataract surgery in order to understand one's requirement and then we can help someone to be independent of their glasses for all the range of activities such as reading, watching computer and also for the distant activities such as driving. My areas of speciality are cornea, cornea transplant, lasik and laser refractive surgeries and customise cataract surgeries. Today I am speaking to you about lasik surgery. So what is it that one needs to understand when one wants to opt for getting rid of their glasses, though glasses are something that are in fashion today, but not everybody wants to use glasses and contact lenses are often cumbersome for people to use, to maintain their hygiene everyday and the wear time is something that people don't want to get into, again people want to be independent of their glasses, to enjoy their life in terms of contact sports, going swimming or to enjoy all the activities that they cannot do because of the glasses, so laser refractive surgery or Lasik is the answer to their problems.

This surgery is basically a very simple procedure by which one can get rid of their glasses, so how does one undergo lasik surgery or how do you decide whether you are a candidate for laser refractive surgery, when one is about 18 years of age and has a stable refraction for at least six months without any change in their numbers and is not pregnant or lactating then one can think about undergoing a few evaluation tests, which are called pre lasik evaluation tests wherein we check their corneal power i.e. using the machine call topography, we check for their eye glass power using an automated refraction, look at the corneal thickness map using the pachymetry map and look at the aberrations in the eye using aberrometer.

Once we have done these investigations then we deem to understand whether the patient is suitable to undergo the laser refractive surgery or Lasik, having understood that what is suitable to undergo lasik procedure, this surgery is done in the following manner so let us understand the steps that are involved in doing the laser surgery, so what we basically do in this procedure is to reshape the cornea that is the front part of your eye, this is like a transparent glass, a thin flap of the cornea is made which is a circular flap when we raise the corneal flap using a special instrument called microkeratome and that is left attached to the rest of the cornea using a hinge, after doing this the laser energy is delivered to the cornea which reshape the cornea in proportion to the number that has to be corrected and the flap placed back. On external appearance your eye looks as it was otherwise, this procedure took a few seconds and is relatively painless and has a quick recovery.

Most of the people gain 85 to 90% of their vision within the first few hours after the procedure and rest of the vision recovery happens over the next couple of days. In terms of the routine activities they can resume their activities like watching television, reading a book or seeing their mobile phone right from the same day of the procedure, contact sports however are best avoided for a period of one week till the complete healing occurs. Postoperatively a few drops have to be instill in the eye for a period of one week to one month and patients can resume their normal activities like computer work in just a day or two after the surgery. Most of the patients are pain free immediately after the surgery; however, a few may experience a little discomfort in their eyes for the first few hours after the procedure which vanishes by the next day of the surgery. Postoperatively patients may experience a little dryness in the eyes, for which a few lubricating drops may be prescribed by us which have to be put in the patient’s eye and they are relieved of their symptoms. The advantage this procedure gives is that they are rid of their glasses permanently and they can enjoy a spectacle free life all throughout.
