What is Stuttering( Stammering) and How Speech therapy can help?

BSC - Hearing and Speech
Speech Therapist, Salem

What is stuttering?
Stuttering affects the fluency of speech. It begins during childhood and, in some cases, lasts throughout life. The disorder is characterized by disruptions in the production of speech sounds, also called" disfluencies" most people produce brief disfluencies from time to time. For instance, some words are repeated and others are preceded by" um" or" uh" disfluencies are not necessarily a problem; however, they can impede communication when a person produces too many of them.

Some examples of stuttering include:
*" w- w- w- where are you going" (part-word repetition: the person is having difficulty moving from the" w" in" where" to the remaining sounds in the word. On the fourth attempt, he successfully completes the word.)
*" ssss ave me a seat" (sound prolongation: the person is having difficulty moving from the" s" in" save" to the remaining sounds in the word. He continues to say the" s" sound until he is able to complete the word.)
*" i'll meet you - um um you know like - around six o'clock" (a series of interjections: the person expects to have difficulty smoothly joining the word" you" with the word" around" in response to the anticipated difficulty, he produces several interjections until he is able to say the word" around" smoothly.)

What causes stuttering?
The exact cause of stuttering is unknown. Recent studies suggest that genetics plays a role in the disorder. It is thought that many, if not most, individuals who stutter inherit traits that put them at risk to develop stuttering. The exact nature of these traits is presently unclear.

What treatments are available for stuttering?
Most treatment programs for people who stutter are" behavioral" they are designed to teach the person specific skills or behaviors that lead to improved oral communication. For instance, many slps teach people who stutter to control and/or monitor the rate at which they speak. In addition, people may learn to start saying words in a slightly slower and less physically tense manner. They may also learn to control or monitor their breathing. When learning to control speech rate, people often begin by practicing smooth, fluent speech at rates that are much slower than typical speech, using short phrases and sentences. Over time, people learn to produce smooth speech at faster rates, in longer sentences, and in more challenging situations until speech sounds both fluent and natural" follow-up" or" maintenance" sessions are often necessary after completion of formal intervention to prevent relapse.

Is anxit .5 mg tab contains steroid. For how long this tab can be used. What type of side effects does it cause.
Anxiety is not a steroid, but still a dangerous sleeping pill. On a longer run they will shrink your brain, resulting in early onset dementia, personality changes, memory changes, even withdrawal seizures. Kindly consult a psychiatrist, get your sleep evaluated, and he will gradually wane you off the sleeping pills and give pills which promote sleep by acting on the cause of sleep disturbance, he may also educate you on sleep hyigiene.

I always have fear of ejaculation when I get erections. I get very anxious when I have sexual thought or when I watch anything intimate. I become fearful anxious nervous. Heart beat increases very fast. Why is it. How to get rid of it and be normal. M 24 unmarried male. Is it Anxiety? Is it treatable?

I always have fear of ejaculation when I get erections. I get very anxious when I have sexual thought or when I watch...
There is certainly anxiety and I believe some level of hysteria is also seen. You need to meet with a counsellor or a sexologist and seek some guidance through some personal time. You will not get ejaculations as soon as you get erections. So just dismiss that idea. You are working yourself up probably due to some misconceptions about sex or you have developed a tendency of getting over-excited.

BSC - Hearing and Speech

Hi lybrate-user. For Stammering, only Speech therapy is helpful. I’m not sure whether Speech Therapists work at Govt hospital at your place. Check for Speech therapy department within the Govt hospital. Very few Govt hospitals have this dept at your place. My advise is go for quality based speech therapy whether given at Govt based or private. For more info, contact pls. Thanks.

I am suffering from a smoking problem. I just can't quit it. If I do not smoke I feel really uptight like depressed. Please suggest me something for quitting smoking.

PG Diploma In Counselling
Psychologist, Hyderabad
Dear Lybrate User, I can imagine how difficult it can be for you to deal with Smoking compulsion which has lead to depression. There is a huge amount of energy behind these acts like smoking habit, there is a powerful drive in addiction. We need to know what that drive is? Lets call your smoking Problem as an addiction for an instance. Addictions are all substitutes actions They take the place of a more direct response to feelings of helplessness in a particular situation. Like you are more controlled by your addiction than the real you. You can work with this issue by seeing a counselor or a clinical psychologist. Hope you feel better soon sending you power to deal with this. Take care Tip: Let that child mature (your sub personality who likes to smoke) and give a nick name and talk to the sub personality is to who you are? Example: "Relax" ,In control" could be the nick name of that sub personality in you. With this 10 to 15 minutes conversation you will understand the drive and slowly you can take in charge of this compulsion.

Sir one of my relative facing schizophrenia he was afraid not existing thing and fear from people how it cure immediately. Can I know it?

MD - Psychiatry
Psychiatrist, Vijayawada
Your relative has to get treated by a psychiatrist. The medicines that she should use are called antipsychotic drugs and they are prescribed by psychiatrist after detailed assessment. Along with treatment your relative should also get counselling sessions/therapy. Called insight oriented psychotherapy. Initially pts have to use medication for 2 years at least after symptoms have subsided. In general there is no cure for schizophrenia but around 15% of schizophrenia pts recover and return to previous level of functioning. And rest are symptomatic in some way or another. Most importantly medication has to be used everyday without skipping doses and follow up with psychiatrist regularly as pts need dose titration if required. The patient should have good support from family n friends and should be treated with respect.

Hello sir 1 month back my uncle expired due to sudden bike accident in native place, after 4 days in my mind repeated...
If you are not satisfied with your current doctor, you can consult another. But at no time should you decide to medicate as you please without consulting your doctor. Apart from medication, you need to reach out to a counsellor to develop some skills and techniques that you can never get through medication alone. You need to learn to detach from your uncle's death. This only means that you are too suggestible and this can hamper your development all through life.

Dear lybrate-user, OCD is a treatable disorder. You need not have to suffer for 4 years spoiling your studies. Please consult a psychiatrist. She/he will diagnose whether you are suffering from OCD or with other illness having OCD symptoms. She/he will assess you medically also. Later few tablets of SSRI or Clomipramine will be prescribed to you at tolerable level of dose. It may be continued for many months even for 2 years or so. If necessary, ERP (Exposure and response prevention) therapy will be added. ERP also needs repeated sessions. Please consult a psychiatrist and take treatment.

Homeopathic Remedies For Autism!

Homeopathic Remedies For Autism!

A mental condition that takes shape during early childhood, Autism is identified by characteristics like difficulty in communication, the use of language, understanding abstract concepts and building bonds with other people. This brain development disorder is also characterized by the performance of repetitive behaviors in the affected individual.

Homeopathy approach in treating Autism

Diagnosis of the condition by a homeopath requires the adoption of an extensive and intensive approach. From collecting detailed information about symptoms like a reaction to vaccines, thermal sensitivities and medical history of the family to understanding sleep postures; treatment of this condition requires an understanding of every vital factor associated with the condition. The hardcore approach is vital for understanding the stimulus/response connection of symptoms. This is because whenever an autistic child undergoes stressful situations, the reaction is a sign that a homeopath cannot ignore. By completely understanding the cycle of stress and reaction, a homeopath can only be able to suggest the right treatment program.

What about the remedies prescribed for treating the condition?

Being a holistic branch of medicine, homeopathy takes into account the complete physical as well as the mental makeup of the individual. Additionally, in this alternative medicine, the homeopath also takes into consideration the details of the disease while recommending remedies. Remedies like Carcinocin, Secretin and Thuja are favored the most for treating the condition. At the same time, the appropriate remedies are recommended after having a thorough understanding as to whether the individual falls within the constitutional makeup of the remedies considered.

Another essential aspect of this branch of medicine in the treatment of this brain development disorder is the practice of using LM potencies (homeopathic medicines on the last scale of potencies). These remedies are not only acted faster but are also gentler compared to others. They have been found to be very effective in preventing the individual from falling prey to the old symptoms and can be frequently repeated as well as used for a long time.

Role Of Inflammation In Suicide - From Mechanism To Treatment!

Role Of Inflammation In Suicide - From Mechanism To Treatment!

Ending one’s life is terrible. But often, instead of analysing the patient’s mental condition at that time, we tend to judge them. The most common cause for suicides is undoubtedly depression. Adverse social or financial conditions often contribute to the mental condition of patients. However, a recent paper published in the journal Neuropsychopharmacology might change the way we think about suicides. Lena Brundin and her co-workers have come to the conclusion in the paper titled “Role of inflammation in suicide: from mechanism to treatment,” that our immune system can trigger off suicidal thoughts and tendencies. Let us find out how:

  1. There is proof that certain medicines that are used to treat cancer patients can affect the immune responses. This in turn can trigger off suicidal tendencies.
  2. Certain injuries can lead to brain disorders which are related to inflammatory responses that might be responsible for some changes in behaviour often leading to suicidal thoughts.
  3. Just like certain medications trigger of suicidal thoughts, there are some that might diminish them like Lithium. It is used to treat bi-polar disorder. What it does is decrease immune related chemicals. Another drug has proved to be very effective in reducing suicidal thoughts and tendencies – Ketamine. Ketamine, like Lithium, has been found to block certain chemicals that are related to immune response.
  4. Though the whole topic is still under extensive research, it can perhaps be concluded that certain drugs have the ability to lessen suicidal thoughts and depression because they block some chemicals that are related to immunity.

The suicidal mentality is very complex. It requires not just medicines, but counselling, empathy, love and understanding. Often, many patients cannot come out of their depression, resulting in therapy sessions that are fruitless. In those cases, medicines that might reduce the negative thoughts and moods will come handy. It will help the patient initially to deal with the situation and start with a lot of positivity. Every year, almost one million deaths occur due to suicide. Not only does it leave behind survivors who attempt to kill themselves but also broken families that struggle to get over the death. If this research is successful, several people will benefit from it, all over the world.

You have some understanding of bipolar it seems and I hope you are accurate in your assessment. Anyway, it will help to get a professional to give you a more accurate diagnosis. You need the help of a counselor to understand and deal with this two-year old development. By yourself it is very difficult to deal with this problem. If it is unipolar depression, then there may be some mitigating circumstances in your earlier life that needs to be explored. If it is anger related, then it is likely that fatal thoughts may be troubling you too. You may follow these instructions and see how well it works for you: Have a good night’s sleep, have a good breakfast of more proteins, meditate often, remain free of stress, eat a lot of fiber, nuts, avocado, exercise regularly, eat dark chocolate, do Yoga meditation exercises, etc. I suggest you do the opposite of what this depression makes you feel like doing (actually, not doing): you will need to fight this condition. You must become active; stay upright during the daylight time; meet people; never sleep during the day, wake up by 6 am every day, play some active games, especially contact games, do physical exercises, talk to people and join some social clubs, attend Yoga classes etc. Watch sitcoms on TV or comedies and cheer yourself up. Go for excursions in groups, for outings, camps, conferences, and religious conventions. Get a pet dog and spend time training it, exercising it and relating to it. Expose yourself to some sunlight every day, at least, 30 minutes but not in the scorching heat. Whatever happens, please incorporate these three important adaptations in your life: always be responsible, be respectful, and be functional. If you did these three, lots of things will go well in life. Please pray and have faith in God to alleviate your sufferings. Don’t wait for others to help. Use your own motivation, which might be at its lowest, but persevere and win this battle. Above all to be really happy, you need to live in love and for love. Learn all about emotions and how to handle them and that will get you out of the depression rather easily and quickly. A counselor is there only to facilitate you, all the hard word must come from you, and your cooperation with that person is very critical for your success. Be positive every day and learn to be contented with what you have. Do some left brain exercises: it is the happy brain. Here are a few suggestions: shut your left nostril and breathe, move your eyes from right to left and vice versa for at least half a minute at a time, and do calisthenic exercises with some form of counting, regularly. Whatever happens please cooperate with the therapy and do not discontinue until the condition is completely resolved. If you explored the anger angle, you may discover that it has its roots in childhood perhaps.

It is difficult to make a diagnosis about bipolar as a lay person. You need the help of a professional for accuracy. Consider the following information: If you do not trust the doctor’s diagnosis, you should seek a second opinion. It can be a crippling disease if not treated and if treated early the prognosis is good. So quickly visit a psychiatrist and seek his/her opinion and you must then follow the medication prescribed. Comply with all the medical prescriptions and work in tandem with a counselor for a long time and learn coping skills and techniques to handle the daily challenges you will face. There are certain adaptations that will suit you during the elation phase and there are certain behaviors that are required during the depressed phases. In the meantime you can do certain things that will give you benefits anyway. Emotional therapy will alleviate a lot of the problems faced in this condition because it is a mood disorder. You need to particularly look at your levels of anger both for its reasonableness and to express it in appropriate manner. I suspect that much of this anger may have origins in early childhood that you may never be aware of now. Talk to your parents and explore that angle. This information will help the counselor too. Also you will need to check whether there is any genetic factor involved. You must exercise regularly, eat healthily and sleep normally too. If your motivation and cooperation is good much can be achieved. You must have a time-structured schedule filled with interesting and reasonable activities and it must be monitored for compliance and delivery. The reason I say this is that many times you will be inclined to have grandiose notions and confidently believe that you can do without the medication. This will set you back many months in the treatment and delay and sabotage the recovery: forewarned you are forearmed. If this combination of medication with therapy is maintained for at least three years without any episodes or breakdowns, it is possible to taper the medication in consonance with the doctor. Bipolar is characterized by two extreme poles i.e. Feeling very high (mania) and suddenly feeling very low (depression). You can ask the doctor for the symptoms he noticed in you for a personal understanding. Regarding some of the symptoms here goes: Mania is a term that describes the emotional highs of bipolar disorder. In general, symptoms of a manic episode may include: •High energy, excessive activity, and/or restlessness •Overly good mood •Irritability •Fast, erratic talking •Racing thoughts •Inability to concentrate •Little need for sleep •Feelings of power •Poor judgment •Reckless spending •High sex drive •Alcohol or drug abuse •Aggression •Refusal to admit that there is a problem Bipolar Symptoms: Depression In bipolar disorder, manic episodes alternate with periods of emotional lows, which are known as depressive episodes. Signs of a depressive episode may include: •Sadness, anxiety, or a feeling of emptiness •Hopelessness •Feelings of guilt, feeling worthless, or feeling helpless •Lack of interest in activities that were once enjoyable, such as sex •Low energy •Trouble with concentration or memory •Restlessness or irritability •Excessive sleeping or insomnia •Changes in appetite or weight •Pain or other physical symptoms not explained by an illness or injury Thoughts of death, or suicidal thoughts or attempts to kill the self.

I want to know what are the activities should be followed to cure depression or a daily schedule.
How you got diagnosed as depression because depression is diagnose if it covers five criteria like low feeling, negative thoughts, aversion to socialization, weeping tendency, sleeplessness or excessive sleep loss or excessive appetite and this has mostly two reasons, one is due to circumstantial and other assumed stress. For treating it you need both counseling and medication. Do meditation daily at morning after wake up with more positive things and at night sleep think about positive thing avoid to use mobile.
