Good afternoon everybody!

I am Dr. Anjali Bapat. I am a Gynecologist and I practice at Mahim. Now today I am going to tell you something about PCOS or Polycystic Ovarian Syndrome. Why I'm telling you this is basically because in my practice I come across many young girl from the age group of 20 to 40, who complain of an irregular period. Because of the lot of information on Google as well as in some magazines, on sites, their friends, all of them they conclude that any kind of an irregular period that they are suffering from Polycystic Ovary. Some of them even get extremely scared that ‘’Oh! Mam, I'm I suffering from Polycystic ovary so is it something very very serious’’.

Well what I would put it like this is that Polycystic ovary is over diagnosed and undertreated condition. What I mean is that after doing Sonography many of the patients it is seen that in the ovaries there are multiple cystic polystyrene cysts are present and hence they are labelled as polycystic ovary. Mind you girl that polycystic ovary is not something very difficult to treat or it is not something that is going to cause danger to your life or it is going to cause a problem in the conception once you get married or if you are married then you are thinking of a child. It's a very easily treatable condition. Let me tell you what it is, first of all, some girls have genetic predisposition towards polycystic ovary.

Which I mean is that some of their genes carry certain traits like for example diabetes, diabetes is like a genetically origin. The same way these girls have their autosomes have certain gene which makes them have a polycystic ovary. This particular genetic trait gets exaggerated or get manifested due to various lifestyle issues that this generation is facing. Which are those issues? First and foremost no exercises. We all are very busy we get exemption which means a very tiring body, tiring mind but is not an exercise. Exercise is exactly the opposite of that. There is a physical activity but it releases a lot of hormones like a endorphins, serotonin etc, which in fact releases the stress. So often I see girls who do not either have time or inclination to exercise. Second most important thing is your dietary habits. A large amount of vegetarian people consume mainly carbohydrates, so a high carbohydrate load is another causative factor which causes this kind of a manifestation of polycystic ovary.

The third most important point is the sleep, thanks to Whatsapp, thanks to Facebook and many other internet and other media. Yes, we need to be active on it but to a certain limit. Often we see that at the end of the hectic schedule when goes off to the bed we switch off the light about 12 o'clock or 12:30 and after that on an average about an hour or so we are glued to our screen. This flickering illuminated screen just before going to sleep causes hormonal imbalance. This is the most detrimental thing. The fourth most important point is stress. Stress is basically any situation which we find a bit difficult to deal with. How are we going to eliminate this stress from the surrounding? Unfortunately no. The stresses are going to be plenty what one has to learn is the response to this stress. Hence once all these four factors if you can control naturally the possibility of polycystic ovaries goes down.

As far as the treatment part is concerned, I want to tell you that there is a tried, the first it's a triangle one apex of the triangle is a polycystic ovary, second apex is obesity and the third one is insulin resistance. You understand polycystic ovary, you understand obesity that is increasing the weight. The third one Insulin resistance which is very important. What it is? Insulin resistance is the condition where the cells of our body can not adequately use the carbohydrates that are present in the body. Hence this high load of carbohydrates gets converted into what is slowly gets converted into glycogen and later on into visceral fat. So we typically a lady who is not so fat, who is not so obese, but if we do her body composition her muscle mass is very poor and percentage of a visceral fat is very high. This visceral fat is a big culprit, it causes a lot of hindrance in the metabolic processes.

Hence a control of an obesity, a control of a insulin resistance is absolutely mandatory for the control of polycystic ovary it’s a tried. One is dependent on the other what I means that obesity causes insulin resistance, insulin resistance causes PCOS and vice-versa. So it’s a by double-ended arrows for the triangle. When polycystic ovaries are also having a science of what is called as a Hyperandrogenism, that means the receptors for the androgens on the hair, facial hair, on the scalp hair or on various skin, is very sensitive to the androgens that are present in the body and this causes excessive hair growth of the body parts for the thinning of the scalp hair which is a male pattern of thinning. When such kind of such things are seen symptoms are seen at that time certainly a medication is required from the Endocrinologist out from the Gynaecology. But mine you what I see the PCOS girls that I see a large percentage is the beginners of this PCOS problem.

So with a minimal amount of a medication and most important lifestyle changes one can keep it under control but if you are having a flared up polycystic ovaries don't worry it does not usually cause a problem in the conception and if the problem occurs various assisted reproductive technologies can take care of it. So to conclude, polycystic ovary is the problem of present generation. To control it there means there are methods and you have to use it properly and scientifically. How Does Age And Lifestyle Affect Fertility?

While you are too busy running a rat-race, chasing education, career, and everything extravagant and lavish, you might be slowly slipping away on your chances of parenting a baby. Find that strange and wayward? Well, your fertility might have already started taking a dip. The chances of conception drop down to less than 10% as a woman approaches the mid-thirties and a man, mid-forties. This is because increasing age takes a toll on the quality and the quantity of eggs produced.

Chromosomal problems accompanied by risk of miscarriages tend to bring down pregnancy rates. So much so, that a child, born to older parents, might have higher risks of suffering from learning difficulties, psychological disorders and even autism.

Not only age, but one’s innate lifestyle habits and practices can also be a few reasons behind infertility.

  1. Age and Fertility: Women’s fertility starts to decline when women are in their early 30s (and the decline speeds up post-35). Men’s fertility starts to decline at 45 years of age. As well as declining fertility, there is an increased risk of pregnancy complications for older mothers and and with older fathers, an increased risk of the child having autism spectrum disorders, mental health disorders and learning difficulties.

  2. Weight and Fertility: Obesity, owing to a sedentary lifestyle and an unhealthy diet, can cause and aggravate hormonal irregularities in both the genders. For men, these imbalances result in coital problems and dip in the sperm count, while for women, they pose threats such as miscarriages, risks of a still-born child and other pregnancy complications. Women who are obese and smoke are more than twice as likely as healthy weight women to have a baby with congenital heart disease.

  3. Substance Abuse: Smoking and excessive consumption of alcohol cause early menopause in women and reduce fertility rates in both the genders.

  4. The Appropriate Timing: If you fail to keep a track of the most fertile period of your menstrual cycle based on the days you ovulate, you might miss out on a good chance of getting pregnant. Pregnancy is technically possible during the five days before ovulation and the day of ovulation. The likelihood of actually becoming pregnant, though, is dramatically increased if you have intercourse in the three days leading up to and including ovulation.

  5. Stress: In today’s times, some amount of stress is unavoidable. However, being subject to constantly high levels of stress on a daily basis ups the secretion of the stress hormone, ‘cortisol’; that eventually takes a toll on one’s libido, thus wreaking havoc on one’s chances and aspirations of parenting a child. Also, what too much stress does is that it mars personal relationships, thus further increasing one’s woes.

  6. Pollutants: With rapid urbanization and unchecked industrialization, the world you live in was never more polluted than now. Such increased and consistent exposure to pollution makes one all the more vulnerable to infertility.

Can STDs Be The Reason Behind Pelvic Inflammatory Disease?

Pelvic Inflammatory Disease, also known as PID, is characterized by an infection that occurs in a lady's reproductive organs. Usually this occurs as a result of a complications arising from a STD. The disease causes severe damage to the ovaries, fallopian tubes and the uterus. It is one of the bigger causes of infertility in women

Causes-

The cervix which is the entry to the uterus has the function of stopping bacteria that are present in the vagina from spreading to the other organs. Once infected by a STD, the cervix can no longer perform its function of preventing the bacteria from spreading to other organs. PID develops when the infection spreads from the cervix to the upper areas of the genital tract. 

There are certain factors which increase the chances of being affected by PID: 

  1. Being sexually active 
  2. Already affected by STDs such as gonorrhea 
  3. Past history of PID 
  4. Having multiple sexual partners 

Symptoms-

Women may experience pain while urinating and engaging in sexual intercourse, coupled with sore feeling in the stomach along with nausea. Abnormal vaginal discharge and a rise in body temperature are common symptoms as well. 

Treatment-
Once the disease has been diagnosed by a doctor, a treatment plan is devised. The treatment options that you can avail of are: 

  1. Medications: If the symptoms are mild, then the doctor will prescribe antibiotic medications. Usually the antibiotics are administered by mouth unless the symptoms are severe, when the drugs are administered intravenously. Your partner would also require medications if you happen to be diagnosed with PID, so that he/she does not catch it. 
  2. Surgery: Surgery is recommended by the doctor in case of tissue inflammation and the presence of pus (abscess). The aim of surgery is to get rid of the abscess to prevent any further infection. 

In addition to the treatment methods, there are certain precautionary measures that you can adopt to be safe: 

  1. Have protected sex.
  2. Try and stick to one sexual partner.
  3. Visit your gynecologist for regular checkups.

Normal Delivery - Things To Remember!

Giving birth is a life-changing event for every mother and it leaves an everlasting impression in your life. Every delivery is unique and you can just gain from the experience of others, but don’t expect it to be the same. 
You will experience many changes in your body and will learn to take care of yourself and your growing baby as soon as you find out that you are pregnant. Pain is a part of labour that every woman has to undergo. It is the fear of labour pain that make many pregnant women opt for c-section. However there is no denying the fact that c-section is a major surgery and requires several days to recover.

Keep in mind the following tips to go in for a normal delivery:

  1. Try to get prenatal education, which guides you about labour, birth and natural labour pain management techniques, such as breathing, self-hypnosis, relaxation and other coping mechanisms. You should check with your hospital too as most of them offer an evening or weekend course.
  2. Pick a gynaecologist who has a history in delivering babies naturally.
  3. Keep yourself physically active by doing mild exercises throughout pregnancy.
  4. You should keep a calm environment in your labour room.
  5. Don’t gain too much weight as women who aren't overweight do not have much difficulty in delivering babies vaginally.
  6. Opt for fewer rather as more tests, treatments and interventions during your pregnancy tend to interfere with normal birth. Whenever tests or procedures are recommended, you should ask about the need of undergoing such tests.
  7. Continue with your walks as it helps in relieving the pain.
  8. Go in for a water therapy. A shower, bathtub, birthing pool and hot compresses are other tools for easing pain and helping you relax. Spend as much time as possible in water.
  9. Use various positions such as sitting on the toilet, kneeling, and squatting to ease vaginal delivery.
  10. Massage/back rubs equally help in easing the pain.
  11. You can also go in for Effleurage which is a light massage of the abdomen to ease your vaginal delivery.
  12. Apply warm or cold compresses.
  13. Avoid hearing negative stories about labour.
  14. Eat a healthy balanced diet as the health and development of your baby depends on your nutrition.
  15. Stay away from stress and anxiety. Read good books about parenting and be in the company of good lively people.
  16. Use relaxation/breathing techniques to keep yourself calm.
  17. Guided meditation and prayers undoubtedly will reinforce your faith in yourself to deliver a baby vaginally.
  18. Have adequate sleep. Avoid drinking tea or coffee two hours before going to bed in order to have uninterrupted sleep.

Following these tips religiously can definitely increase your chances of having a normal delivery.

What Is Emergency Contraception?

Contraception refers to something, which works to prevent a pregnancy from occurring. But what is emergency contraception? This sort of contraception is made use of in case other methods of birth control do not work or a woman has had unprotected sex.

This sort of contraception is also referred to as the morning after pill and postcoital contraception, which effectively means that it is taken after sex. Well, if while having sexual intercourse, there is a breakage of the condom or it slips off the penis, then it is a good idea to make use of emergency contraceptives, as should a woman if it so happens that she misses two or more birth control pills over the course of her monthly cycle.

While it is true that a large majority of emergency contraceptives, which are available in the market today are in the form of pills, note that this is not the only form of emergency contraception. As a matter of fact, copper containing intra uterine devices (IUDs) happen to be the most effective way to prevent a pregnancy from occurring, but this is only true if they are fitted within five days of unprotected sex. Again, even in this case, the sooner, the better!

Most people believe that an emergency contraceptive is almost always successful in ending a pregnancy. This is quite inaccurate as the main intention of an emergency contraceptive is not to end a pregnancy, but to prevent it from happening in the first place. This is exactly why it is so essential to have an emergency contraceptive pill after sex as soon as possible. The longer a woman delays, the higher the risk of fertilisation taking place and pregnancy then being left to be a foregone conclusion.

It is important to keep in mind that emergency contraception is not a substitute for taking precautions when it comes to sex. As a matter of fact, it can be said that there is no substitute for being responsible and making use of condoms or birth-control pills in a prudent way. After all, who wants to have the stress and the repercussions of a potential unwanted pregnancy? It makes quite a bit of sense to take some extra care so that there is no need for anxiety, as it isn’t really required and sex remains as much fun as intended!

Your PCOS Diet During Pregnancy!

Polycystic Ovarian Syndrome or Disease (PCOS) is a very common condition, wherein there are multiple cysts in the ovaries. As a result, there are many changes which the body undergoes and it is not limited to the gynecologic system. A woman with PCOS may also find it difficult to conceive, and so, once she is pregnant, precautions are essential to ensure there are no complications.

Pcos predisposes women to diabetes, hypertension, obesity, cardiovascular complications, lipid metabolism disorders, uterine cancer in long run. Some of them are listed below:

  1. Preeclampsia: When the blood pressure readings are high during pregnancy, it is known as preeclampsia. It brings with it a whole lot of complications including the need to cesarean section, premature birth, etc. So, it is best avoided, and diet can help to some extent.
  2. Diabetes: Gestational diabetes which manifests as higher sugar levels only during the pregnancy is very common in women with PCOS. The increased hormone levels in PCOS increase insulin resistance, thereby increasing sugar levels. This needs to be managed through a combination of diet, exercise, and lifestyle changes.
  3. Preterm labor: Women with PCOS are at a slightly higher risk of premature labor.
  4. Weight-related issues: PCOS leads to weight gain, and this could be a problem during pregnancy. It is essential to discuss with the doctor as to what would be a good weight range and stay within that range throughout pregnancy. Weight gain brings with it a host of complications and so best avoided.

Dietary changes:

With PCOS, during pregnancy, strict cautious diet planning can help in avoiding complications and allow for an easier pregnancy. Though they may not solve every problem associated with PCOS, dietary modifications can have a significant effect on the overall health and well-being. Listed below are some easy-to-make changes:

  1. Increase consumption of fibres like greens, nuts, pumpkin, berries, whole grains, almonds, etc. are included. This ensures that digestion is a prolonged and gradual spike in blood sugar levels.
  2. Increase protein-rich foods like soya, tofu, eggs, and chicken, which help in avoiding binging. They are light on the stomach and help in weight management.
  3. Foods which are generally anti-inflammatory including tomatoes, olive oil, spinach, fresh fruits, and omega-3 fatty acids help in controlling blood pressure and cholesterol levels.
  4. Supplements to include omega-3 fatty acids, prenatal vitamins, vitamin D, and calcium if required ensure that the baby gets the required nutrients for optimal growth.

What to avoid:

Anything that can spike up calories and is of low nutritional value should be avoided.

Good afternoon everybody!

I am Dr. Anjali Bapat. I am a Gynecologist and I practice at Mahim. Now today I am going to tell you something about PCOS or Polycystic Ovarian Syndrome. Why I'm telling you this is basically because in my practice I come across many young girl from the age group of 20 to 40, who complain of an irregular period. Because of the lot of information on Google as well as in some magazines, on sites, their friends, all of them they conclude that any kind of an irregular period that they are suffering from Polycystic Ovary. Some of them even get extremely scared that ‘’Oh! Mam, I'm I suffering from Polycystic ovary so is it something very very serious’’.

Well what I would put it like this is that Polycystic ovary is over diagnosed and undertreated condition. What I mean is that after doing Sonography many of the patients it is seen that in the ovaries there are multiple cystic polystyrene cysts are present and hence they are labelled as polycystic ovary. Mind you girl that polycystic ovary is not something very difficult to treat or it is not something that is going to cause danger to your life or it is going to cause a problem in the conception once you get married or if you are married then you are thinking of a child. It's a very easily treatable condition. Let me tell you what it is, first of all, some girls have genetic predisposition towards polycystic ovary.

Which I mean is that some of their genes carry certain traits like for example diabetes, diabetes is like a genetically origin. The same way these girls have their autosomes have certain gene which makes them have a polycystic ovary. This particular genetic trait gets exaggerated or get manifested due to various lifestyle issues that this generation is facing. Which are those issues? First and foremost no exercises. We all are very busy we get exemption which means a very tiring body, tiring mind but is not an exercise. Exercise is exactly the opposite of that. There is a physical activity but it releases a lot of hormones like a endorphins, serotonin etc, which in fact releases the stress. So often I see girls who do not either have time or inclination to exercise. Second most important thing is your dietary habits. A large amount of vegetarian people consume mainly carbohydrates, so a high carbohydrate load is another causative factor which causes this kind of a manifestation of polycystic ovary.

The third most important point is the sleep, thanks to Whatsapp, thanks to Facebook and many other internet and other media. Yes, we need to be active on it but to a certain limit. Often we see that at the end of the hectic schedule when goes off to the bed we switch off the light about 12 o'clock or 12:30 and after that on an average about an hour or so we are glued to our screen. This flickering illuminated screen just before going to sleep causes hormonal imbalance. This is the most detrimental thing. The fourth most important point is stress. Stress is basically any situation which we find a bit difficult to deal with. How are we going to eliminate this stress from the surrounding? Unfortunately no. The stresses are going to be plenty what one has to learn is the response to this stress. Hence once all these four factors if you can control naturally the possibility of polycystic ovaries goes down.

As far as the treatment part is concerned, I want to tell you that there is a tried, the first it's a triangle one apex of the triangle is a polycystic ovary, second apex is obesity and the third one is insulin resistance. You understand polycystic ovary, you understand obesity that is increasing the weight. The third one Insulin resistance which is very important. What it is? Insulin resistance is the condition where the cells of our body can not adequately use the carbohydrates that are present in the body. Hence this high load of carbohydrates gets converted into what is slowly gets converted into glycogen and later on into visceral fat. So we typically a lady who is not so fat, who is not so obese, but if we do her body composition her muscle mass is very poor and percentage of a visceral fat is very high. This visceral fat is a big culprit, it causes a lot of hindrance in the metabolic processes.

Hence a control of an obesity, a control of a insulin resistance is absolutely mandatory for the control of polycystic ovary it’s a tried. One is dependent on the other what I means that obesity causes insulin resistance, insulin resistance causes PCOS and vice-versa. So it’s a by double-ended arrows for the triangle. When polycystic ovaries are also having a science of what is called as a Hyperandrogenism, that means the receptors for the androgens on the hair, facial hair, on the scalp hair or on various skin, is very sensitive to the androgens that are present in the body and this causes excessive hair growth of the body parts for the thinning of the scalp hair which is a male pattern of thinning. When such kind of such things are seen symptoms are seen at that time certainly a medication is required from the Endocrinologist out from the Gynaecology. But mine you what I see the PCOS girls that I see a large percentage is the beginners of this PCOS problem.

So with a minimal amount of a medication and most important lifestyle changes one can keep it under control but if you are having a flared up polycystic ovaries don't worry it does not usually cause a problem in the conception and if the problem occurs various assisted reproductive technologies can take care of it. So to conclude, polycystic ovary is the problem of present generation. To control it there means there are methods and you have to use it properly and scientifically. Menstrual cramps are a common occurrence in women who are of childbearing age. The pain is mostly felt in the back or lower abdomen. Menstruation is unavoidable and natural, but the cramps associated with it can disrupt your normal lifestyle. You find it hard to focus and concentrate on your work, among other things. Fortunately, there are ways to counter your pain.

So, here are 10 ways to relieve your period cramps-

  1. Over-the-counter pain medications such as ibuprofen and naproxen can provide relief. Ibuprofen is effective in lowering prostaglandins (hormones responsible for menstrual cramps). But you need to take the medicine 3-4 days before your periods start.

  2. This may surprise you, but birth control pills can also reduce pain. Besides regulating your periods, they also maintain a steady hormonal level in your body. When hormones aren’t fluctuating, the severity of pain reduces.

  3. Maintaining a healthy diet can provide long-term relief from menstrual cramps. If you have a healthy weight, you would experience lesser pain. Oestrogen is produced by fat cells and abnormal amounts of fat cells can increase its production, and when you have a healthy weight, you can prevent hormonal imbalances.

  4. Drink plenty of water—it not only keeps you hydrated but also keeps your hormones in balance.

  5. Stay away from beverages that contain caffeine, such as coffees and sodas. Caffeine is inflammatory in nature, and it will only worsen your pain.

  6. Yes, moving too much during periods sounds crazy, but exercising can reduce your pain! If you exercise regularly, your stress levels reduce (too much stress worsens pain) and your body releases endorphins, which will lessen the pain considerably.

  7. Cramps are stubborn, and sometimes all the preventive measures fail. At times like these, heat is your best friend. Whether it is a heating bag on your abdomen or a nice, warm bath, heat can reduce your pain and make you feel comfortable.

  8. Sometimes, magnesium deficiency can trigger pain and make your life difficult. Do consume plenty of magnesium-rich foods such as dark leafy vegetables, bananas, yoghurt, nuts, fish, whole grains, etc.

  9. Sweet or salty cravings are common during periods. But you need to stay far away from all kinds of processed foods, because they will only cause more pain.

  10. Yoga is an excellent remedy for menstrual cramping.
