Shoulder Pain: Causes, Diagnoses & Treatment

Ask Free Question
Shoulder pain can be a persistent dull and nagging sensation in the upper arm or shoulder area. It can be easily differentiated from neck pain as it is related to the movement of shoulders i.e. it will increase and decrease with shoulder movements, especially overhead activities. The other signs to identify shoulder pain is if your shoulder movements are reduced.

If you find it difficult to scratch / wash your back, comb your hair or reach your back pocket, it clearly shows you are suffering from shoulder pain. Shoulder pain can happen due to various reasons, it varies from person to person depending on the activities he/she is involved in, for e.g. sports, manual job etc. It can be a single major injury or multiple small injuries due to repetitive usage patterns. Also, degenerative changes in various parts of the shoulder can lead to shoulder pain.

In case of middle to old age, injuries that can cause shoulder pain are:

Biceps Tendonitis
Acromio-clavicular Joint arthritis
Scapular Dyskinesia
Trapezitis
Suprascapular nerve entrapment In young age, injury due to sports or gym activity can cause shoulder pain
Superior labral anterior/posterior (SLAP tear)lesion

Also, traumatic or repeated dislocations of shoulder can cause shoulder pain due to:

Bankart lesion

Diagnose Shoulder Pain

Shoulder pain can be diagnosed only after thorough history taking and clinical examination. However, some imaging studies can be very helpful like:

X-rays: Through an x-ray one can view the shoulder in many different views.
Ultrasound: With an ultrasound, you can also check the damaged caused to the tendons and muscles of the shoulder but the quality of reporting depends upon the expertise of the doctor.
MRI: MRI gives a clear picture of the shoulder as it shows everything related to a shoulder joint that is joints, vessels, tendons and muscles that too from different angles.
Diagnostic Arthroscopy: A surgical procedure often used by orthopaedic surgeons to diagnose and treat issues inside a joint. Arthroscopy has the big advantage that one can use it to diagnose as well as treat at the same time.

Treatment for Various Types of Shoulder Pain

For Shoulder Impingement or Rotator Cuff tear: Give rest to the shoulder for a few days only Dedicated Physiotherapy Protocol Bursal injections If these fail then, (Surgery)Arthroscopic sub-acromial decompression If Cuff tear is found, then one can go for Arthroscopic/Mini-open repair
Labral Lesions: For anterior Labral lesions (Bankart's) following anterior shoulder dislocation then surgery in the form of Arthroscopic Bankart's repair is the only option to reduce risk of recurrence and degeneration.

SLAP Lesions

Pain relief and physiotherapy as first step If this fails then following surgical options are available
Arthroscopic debridement or repair of labrum
Biceps tenotomy or tenodesis

Arthritis

In this case, the joint is already destroyed and has to be resurfaced for pain relief and to maintain ROM. If Rotator cuff muscles are working, then Shoulder resurfacing hemiarthroplasty (preferred in young people), that has a life of 10 to 15 years, Total shoulder replacement (preferred in middle aged to old people) has a life of 10 to 15 years. But, if rotator cuff muscle are not working then Reverse Geometry Shoulder replacement is recommended, that has a life of 10 years.

It is always advisable to get yourself diagnosed properly in case of shoulder pain as management changes depending on clinical situations. Neglecting shoulder conditions can lead to bigger problems later on.

Orthopedic disorders include all the health issues and ailments that are related to the muscles, joints and ligaments. Orthopedic disorders are so widespread and so large in number that it is very difficult to list all of them down. However, among them the five most common orthopedic disorders are:

1. Arthritis
It is a chronic and a long lasting orthopedic disorder in which you witness the swelling or inflammation of one or more joints in the body. This condition is very painful in nature and it tends to deteriorate gradually. This condition usually affects elderly people and it also worsens with age which ultimately causes stiffness of the joints and limits your routine body movements.

2. Osteoarthritis
This is a degenerative joint disorder which leads to immense pain and discomfort in the joints of the body. This is one of the most serious forms of orthopedic disease that has been noticed. This disease involves degradation and decline in the overall quality of the bones and joints in the affected areas.

3. Lower back pain
This condition refers to periods of pain and discomfort in the lower part of your spinal area. This condition is very common nowadays and as a result of this pain, your back usually becomes tender, swollen and stiff. This pain also stops you from doing your overall routine activities.

4. Rheumatoid arthritis
It is an orthopedic disorder in which there is severe swelling in a particular joint, accompanied by small lumps on the affected area that further degrade the external appearance of the skin. In case of adults, this disorder lasts for a lifetime and worsens with time, if left untreated.

Common Treatment Options For Back Pain

Lower back pain is a very common discomfort causing condition that occurs anywhere on the back below the ribs and above the legs. The pain is very common and it is faced by most individuals at least once during their lifetime.

The most basic measures to cure your lower back pain are exercising well, eating healthy and maintaining a good body posture at all times. Here are some of the common treatment options for reducing back pain.

Spinal Manipulation: It refers to an ancient form of therapy in which your back pain is treated externally. This form of treatment is quite similar to a massage treatment and helps to relax the muscles in the lower back area, thereby reducing the pain. It is usually done by osteopathic physicians, chiropractors, occupational therapists and physical therapists.

Neurostructural integration technique (NST): The main role of this type of treatment is to naturally reset the affected area of the body in order to allow it to regain its normal form. The results usually begin to appear after at least 2 sessions of the therapy and the effects are long lasting in nature.

Acupuncture: This is an old technique which uses small needles at strategic locations on your skin. The main aim of the acupuncturists is to re-balance the flow of energy in your body and to ensure that you have an optimal level of energy in all parts of your body. This in turn helps to regain the normal state of your lower back if the right points on the body are targeted.

Other measures: Some preventive measures which ensure that the lower back pain does not recur are as follows:

  1. Keep your spine straight and posture correct
  2. Avoid excessive bending, stretching or weight lifting
  3.  Avoid heavy physical activities like sprinting
  4. Regular and light exercising

I went through the tibia surgery in which they have inserted rod inside tibia to integrate the fracture. It's been 4 years for the operation. Should I go for reverse operation to extract that rod. I mean is it necessary to eliminate or I can continue with the same.

There is no need to remove the implant unless there are some symptoms related to it. Implant removals are tricky and unpredictable operations which take almost as much time as the original surgery.

My mother is 50 years old and suffering from knee pain. How can I treat my mother?
Id she has not had any injury to the knee then at this age it is most likely to be degenerative pathology. This needs to be confirmed with xrays. Blood investigation to rule out inflammatory arthritis may be required if there is family history of ra. The first line of management in degenerative knee pain is conservative with physiotherapy and pain control with over the counter simple analgesic.

My mother having a knee problem since last 8 years and I concerned most of the doctors and also concerned in medical but doctors said that knee pain is caused by gas. Is their any treatment so my mother knee problem is cured.

Knee pain cannot be explained by gas. Your mom is 50 so she must have developed early osteoarthritis which is the most common cause of knee pain if there is no history of trauma. Try pain killers only when necessary and start physiotherapy to strengthen the quadriceps muscle. Give it 3 weeks and the re-evaluate.

Sir I have a problem of slipped disc in cervical spine. I am a dance choreographer so I should do my profession or I should stop it.

If the problem is recent and correctly diagnosed you should take rest for a couple of weeks and then start gradually with neck physio. Once your neck muscles are strong then you can gradually restart your dance activity.

Try rest x 2 weeks and then physiotherapy x 4weeks. Grade 2 signal does not mean a complete tear. Partial/low grade injuries can heal.

I think you are talking about osteoarthritis. The first step in tackling oa knees is to strengthen quadriceps with physiotherapy. Please try this for a few months to notice a difference.

It depends on your activity level and symptoms. If you have instability symptoms then you should be treated for it. If you are into professional sports then you should be treated aggressively. In any case it is advisable to try focussed physiotherapy for cal insufficiency. Knee brace is temporary.

You need to get good quality x-rays of shoulder with special views. Most likely the fracture would have united but the position may not be optimum leading to these symptoms. Also it depends on which part of your shoulder got fractured. If x-rays are ok then a course of physiotherapy with posture training may help. Also if you sit for long hours hunched in front of computer then that may be a cause for your current symptoms.

Is it slip of one vertebra over other or disc slip. If it is prolapsed disc conservative management with rest can take care of 90% of patients. If it is spondylolisthesis then physio is the first line of management.

You could have a ligament injury. Ligaments have poor blood supply and hence take time to heal. You should get an xray done and visit an orthopaedic surgeon because if it were a minor sprain it should have healed by now.

Explain to her that bones are weak at her age and if she were to fall she might develop a fracture. Also there are proven studies that walking aids reduce risk of fall significantly.

Based on your symptom set, I think you have shin splints. It requires rest so I would advise you to give running a break for a couple of weeks and then gradually build up again. However you should get an x-ray done to check if there is any stress fracture and get yourself checked by an orthopaedist.

Back pain due to disc problems?
Nerve blocks help in radiculopathy and not in low back pain, so if your pain is associated with leg pain which is shooting and electric in nature then it will work. Facet joint injections can work for back pain.
