Mr. lybrate-user, Thanks for the query.
From the given information it is clear that she is already on very high dose of Glimepiride 4 X 2 tabs. Usually there is no benefit in giving such a high dose of glimepiride. Plus she is on Vildagliptin also. It is essential to bring blood glucose level to: FBG <100 mg & PP 150 to 170 mg & HbA1c%<6.8, in order to prevent long term complications of diabetes. According to the treatment you are getting, I can see the effort of your doctor. I think you should consider taking insulin atleast for one time in a day to control it along with your medication. As you have hba1c of 10.8, it's good for your health to start insulin atleast once daily. Pls consult to your doctor about it or you can consult me online m.

Hello, thanks for the query.
Both the products are same. The er refers to extended release, which means the drug is released slowly over 12 hours in the gut. That helps maintain a longer therapeutic level. It is beneficial more so if a person has any side effects like gastrointestinal disturbances if plain metformin is taken. Both are fine. Thanks.
