How does this medication work?
This medicine works via multiple mechanism as it is a multi-drug composition. Firstly, Telmisartan displaces angiotensin II with very high affinity from its binding site at the AT1 receptor subtype, which is responsible for the known actions of angiotensin II. Secondly, Hydrochlorothiazide is a diuretic which inhibits a sodium-chloride symporter SLC12A3 in the distal convoluted tubule to inhibit water reabsorption in the nephron. The symporter reabsorbs sodium and creates an osmotic gradient for water to be reabsorbed, inhibition of which prevents water reabsorption. Lastly, Amlodipine is an calcium channel blockers. It works by inhibiting the entry of calcium into the cardiac and vascular smooth muscles and prevents the contraction of the muscles and thereby reduces the blood pressure.
