Mr. David duncan 65 years and a retired mason from dawenya reported to your health centre with complaints of palpitation and cough. Being the most senior nurse at the facility you are required to take his health history and contact the physical examination on him. Requirement 1: outline what you would want to find out about his symptoms in the history of presenting complains Mr. David duncan 65 years and a retired mason from dawenya Mr. Duncan indicated in the history of presenting complaints that the illness started about 3 months ago. In last 2 weeks both symptoms became worse. To him, palpitation means he feels his heart is beating faster. It comes on with physical activity and has worsened over time. He feels tired easily when he’s having the palpitating and has to stop all physical activities for relief. The cough is productive of whitish frothy sputum, not bloody not offensive and with no other associated symptoms. It is also worsening with time. Requirement 2: you decided to conduct a systemic inquiry (direct questioning) on Mr. Duncan. List the relative organ system in this component of the health history. You have found out in the systematic inquiry that Mr. Duncan experiences difficulty in breathing. He is breathless when lying flat but comfortable lying in the inclined position or sitting up. He has no wheezing or stridor in breathing. He noticed swollen feet that are painless. He has no chest pains but the sputum is very copious and frothy. Micturition is without pain but urine volume is scanty. He has lost appetite but no nausea or vomiting. He feels some heaviness on the right side of the abdomen though not with abdominal pain. He never had fever at any time during the illness. Requirement 3: at this stage of the history, you considered Mr. Duncan may have a cardiovascular problem of slow onset and that his past medical history might be relevant to the present illness. What information in his past medical history will you want to know of. You have summed up your history as a middle-aged man presenting with palpitation and cough that is productive of whitish frothy sputum and bilateral swollen feet. He is fatigued, lacks energy and lost appetite for food. He suffers shortness of breath and a thopnea without dizziness or fainting. He has a past medical history of hypertension but is poorly complaint and defaulted in treatment for months. From the history, you suspect Mr. Duncan is suffering congestive cardiac failure. Requirement 4: enumerate the physical examinations you will carry out on Mr. Duncan’s cardiovascular system. Instruction: list at least three findings – negative or positive under each component of the physical examination.
