In fistula in ano; a nodular swelling appears in area surrounding anal opening. Off and on pus discharge is seen from this swelling. This problem can not be cure by oral medicines or some ointments. Though she is having satisfactory blood sugars, her blood pressure is high and she is having cardiac symptoms.
Breathlessness and chest pain which responded to nitroglycerin also suggest cardiac symptoms. Continue nitroglycerin till you consult cardiologist for complete cardiac evaluation such as ecg, echo and angiography (if suggested by your cardiologist)

Pin pain after stunting is usually not due to heart. It is usually muscular in origin. Still, I would advise you to get an ecg done, and show it to cardiologist.
