K Glim 2 MG Tablet SR Health Feed

This is 57 years female from Kolkata. I'm a diabetic (take (GLIMEPIRIDE 2 mg + METFORMIN 500 mg) after breakfast & METFORMIN 500 mg after dinner) & 10 iu insulin overnight ,fasting 80, PP 287 measured today. I want to know can I take glimepiride 2 mg+ metformin 1000 me twice a day to lower the glucose level. For the last 1 year, I'm seeing that my fasting glucose level is under 100, but PP is above 200. More over what is the difference will be in result if a patient take GLIMEPIRIDE 2 mg + METFORMIN 1000 mg instead of GLIMEPIRIDE 1 mg + METFORMIN 1000 mg?

lybrate-user, Thanks for the query. As mentioned your FBG is fine (93 mg), but PP is definitely high. The reason for fasting being low is the Insulin dosage being taken at night. You need to reduce the post lunch glucose level. So you may look at increasing only Glimepiride dosage rather than taking two tablets of double dose of both (glimiperide + Metformin). That may become excess. If you have any specific questions please do come back. You should target PP <100 mg (as is now), PP 150 to 170 mg HbA1c%<6.8.8. Apart from tablets diet restriction and exercise also have to be looked at. Thanks.

1.Do regular aerobic exercise for 1 hour daily (brisk walking, jogging,running, swimming,cycling etc) 2.Reduce weight if overweight 3Control diet a) Avoid sweets, sugar/honey, milk, milk product excess calorie and carbohydrate rich diet like white bread, cereal, rice, pasta b) avoid all refined food like maida, pasta, starchy food like rice, potato, bakery item, processed food, c) include more fiber like pulse, green leafy vegetable, cucumber, carrot, tomato, j legume. D) Avoid sweetened fruit like banana, mango,litchi, chickoo, sapota, seethaphal jack fruit, grape etc. Can have fruits like guava, pineapple, papaya, orange, mousambi, strawberry, watermelon, pomegranate, jack fruit, sweet potato, a handful of walnut daily (Akhrot) e) Avoid white/polished rice, take unpolished rice like brown rice/wheat/Jowar/Ragi, moong and chana Dal. F) Avoid ghee/ butter./coconut oil/excess salt intake like papad/pickle. G) Avoid red meat. Take fish/chicken. H) Avoid alcohol and carbonated drinks like soft drink 4.Adhere to a strict diabetic diet by consulting Dietitian 5.Monitor your fasting, PP blood sugar 3 monthly and HbA1c six monthly 6.Consult diabetologist/Endocrinologist for advice and treatment.

Mr. lybrate-user, Thanks for the query. With appropriate treatment blood glucose levels should be well controlled, like fasting < 100 mg, PP 150 to 160 mg & HbA1c% <6.8. Since your glucose levels are not in control, means present treatment is not enough. One is drugs being taken should be improved and second is look at your diet & regular exercise. Hello, Thanks for the query. To achieve better blood glucose control he may have to use Insulin Injections along with the oral drugs. It is also necessary to know his kidney functions, like serum creatinine, blood urea, microalbumin levels, and eGFR. The information given is too little. In a known case of diabetic we need to control sugar levels with medication , aerobic exercise and following a diabetic diet and few tips on diet say no to maida items like toast , khaari ,naan ,white bread . can drink tea but without sugar say no to Buffalo milk and can drink cows milk can eat brown bread can eat sprout’s can eat green vegetables, can eat chappati ,bajra ka rotla can eat one katori of rice twice a day with dal to avoid farsan food items can eat fruits say no to coconut water

It may happen sometimes, you have to go for regular follow-up with your doctor and mention about it ( you can check it at home and plot a chart ). Let the doctor review your case afresh taking into consideration all the facts and check your general health. Discuss about your concerns and what precautions to be taken. He doctor may reduce or change the medicine. At your end maintain a regular routine ( eat healthy food with calorie count ) and exercise as usual. Please revert after that for any more clarification.

lybrate-user. Upadhyay, Thanks for the query. Ideally glucose levels should be maintained as follows: FBG < 100 mg, PP 150 to 160 mg & HbA1c% <6.8. So the levels mentioned are high. One has to look at diet, exercise and then also towards medicine, whether to upgrade the treatment. Thanks.

Hi, Both my parents have diabetes. Mom takes Gluconorm G2 (Metformin+ Glimepiride) Dad takes Volibo M0. 2 (Metformin+ Voglibose) Both have a 500 mg+0.2 mg combination. What's the difference between them? Are they interchangeable?

Mr. lybrate-user, Thanks for your query. The drugs mentioned Glimepiride and Voglibose are different in their action. Metformin is supposed to reduce or delay glucose absorption and hence slows down blood glucose rise after taking food. Plus it also reduces insulin resistance hence helps blood glucose control. There are other beneficial effects like helps slight weight reduction. Glimepiride is a sulphonylurea compound, it stimulates insulin secretion by the beta cells of pancreas, thus increases insulin availability and controls blood glucose level. Its action is very powerful. At times it can cause low blood glucose level also. Voglibose is called a alpha glucosidase inhibitor, reducing conversion of disaccharrides (sucrose) to glucose. It is a milder drug. Therefore, these are interchangeable drugs. However, what is most important is that blood glucose should be well controlled with fasting glucose closer to 100 mg, PP 160 to 170 mg & HbA1c%<7. Then only all the long term complications of diabetes can be either delayed or prevented. Thanks,

Mr. lybrate-user, Thanks for the query. HbA1c% 10.2 is suggestive of totally uncontrolled diabetes. This is dangerous, can lead to early development of long term complications associated with diabetes, affecting eyes (retina), kidneys, peripheral blood vessels, heart and lower limbs. It is clear that present treatment is not helping in control. So please consult your treating physician and upgrade the treatment or if you seek online help, please come back with following details { Fasting, PP glucose levels, diet pattern, extent of daily exercise because with a BMI of 31 plus you are already in grade 1 obesity. That needs to be looked at. Your ideal body weight should be about 56 kgs, as against 82. I await your response. Thanks.
