Hi lybrate-user. Lung cancer does not carry a good prognosis. Your father in lawhas stage 4 disease which has been treated with chemotherapy and targeted therapy. If your father in law has developed respiratory problems even after osimertinib, it means that the disease is in progression. He should undergo imaging with ct scan of chest to assess the disease extent.

Sorry to know about the condition of your father. Afatinib is better than erlotinib or gefitinib with respect to results but is costlier. I would suggest you discuss with your treating oncologist and plan ahead.

You have not mentioned about your previous pet ct report. If the current pet ct suggests progression of disease then we have to change medication and if its static or improved than last pet ct then we have to continue same treatment.
