Suitability of the treatment can be commented upon only after information about er, pr, her 2neu on tumor tissue is available. Principally treatment of early breast cancer is surgery, chemotherapy and radiotherapy. The chemotherapy regimen will depend upon the biomarkers on tumor as mentioned earlier and the chemotherapy doses will depend upon her body surface area.
Since you have mentioned 'n1' she will also requie radiotherapy.

Regimen given this way is known as r dose adjusted epoch and not r chop. I will not comment on dosages as it will be decided by treating oncologist based on patient tolerance. Typically after completing such 6 cycles. After a gap of at least 2 months. We do a repeat pet ct scan to assess response and rule of any additional role of any therapy. Thank you.

As it is a known auto immune disease which is affecting the kidney and lungs and we have already seen that once you stopped the treatment pulmonary symptoms had developed and if we discontinue the treatment than there can be death due to kidney and lung failure and with ongoing treatment we are clearing the auto antibodies and making her survive for more years to come

Is CEF is right chemo treatment nat ER +/PR + and Her 2- I am 41, unmarried, premenausal female. Diagnosed with Invasive breast cancer (lump on the right chest wall-upper inner quadrant high up) lymph node and axillary node positive. Type: ER +, PR+,her 2 -.Size of the tumor is 2.7 cm.and they say its a LABC (locally advanced breast cancer) .Started with NACT (CEF every 3 weekly). Done with the first chemo 15 days back with CEF (cyclophosphamide-750, epirubicin,-120 fluorouracil-750 mg. I am really confused as I don't see much data on hormone positive and Her 2 - patients given this CEF chemo (either adjuvant or neoadjuvant). My fellow patient (of similar types) from other hospitals are given AC/T. Also I met one of senior oncologist who suggested to shift from chemotherapy and instead start Neoadjuvant Endocrine Therapy (tamoxifen 20 mg for next 3 months). Would really appreciate if you share your expert opinion the right treatment path for my case.

Dear Sir My wife 42 years was suffering from Adrenal insufficiency from Feb 2009 along with Hypo Thyroidism .Her ANA was positive since 2009 however she was doing fine till 15th Dec 2016 and she was on Wysolone 5 and Thyrox 88 (Dosage tapered from 25-50-75--88 (recently)}in last 7 years. On.16. December 2016. Her Blood pressure got elevated to 234/ 134 and when we admitted her into Hospital .Her Creatinine was 3.4 which has gone UpTo 5.44 and with 3 shots of pulse steroids like Methyl prednisolone and one Immunosuppressant drug Cyclophosphamide it came down to 3.67 which is again increasing now and reach to 4.40 on 12th Jan 2017. Her Platelet were approx 46000 at the time of admission which dipped to 22000 and after that she was administered 3 SDP and 1 unit of blood and Platelet reached to 1.22 Lakhs. However again it came down to 19000 few days back and it was 42000 on 12th Jan 2017 Already 5 sessions of Plasmapheresis has happened Report of bone marrow biopsy is awaited. Doctors Panel (comprising of 2 nephrologist 1 Rheumatologist Hematologist) were discussing the idea of adding rituximab however she got some lung infection--they have differed the decision until infection is over. Blood Pressure is fluctuating since last one month--Systolic---(120--200) and dystolic (80--110) even after 4 drugs from last 30 days. Please advise Imp--sjogren syndrome is confirmed through APAP reports and ANA profile is strongly positive for SS-A.
