Hello, Your BMI- 21.48 Diabetes and Hypothyroidism diet plan suggest Do regular aerobic exercise for 1 hour daily (brisk walking, jogging, running, swimming, cycling etc) 2. Reduce weight if overweight 3control diet a) avoid sweets, sugar/honey, milk, milk product excess calorie and carbohydrate rich diet like white bread, cereal, rice, pasta b) avoid all refined food like maida, pasta, starchy food like rice, potato, bakery item, processed food, c) include more fiber like pulse, green leafy vegetable, cucumber, carrot, tomato, j legume. D) avoid sweetened fruit like banana, mango, litchi, chickoo, sapota, seethaphal jack fruit, grape etc. Can have fruits like guava, pineapple, papaya, orange, mousambi, strawberry, watermelon, pomegranate, jack fruit, sweet potato, a handful of walnut daily (akhrot) e) avoid white/polished rice, take unpolished rice like brown rice/wheat/jowar/ragi, moong and chana dal. I cannot comment upon ayurvedic medicines. Generally diabetes is not curable. It can be kept under control with treatment. If it remains uncontrolled it can lead to complications.

Treatments of Infertility

Treatments of Infertility

Female infertility is considered as the inability of conceiving. As per WHO it is described as the inability to be pregnant, maintain and carry forward a pregnancy to its full term. It is often of two types:

  1. Primary Infertility: This is the inability to give birth to a baby either due to the inability to become pregnant or maintain a pregnancy, which includes stillborn and miscarriage.
  2. Secondary Infertility: This refers to the inability to give birth when there is already a live birth or the woman was pregnant.

The causes of infertility can vary from being acquired or genetic. However, following factors are largely responsible for infertility:

  1. Genetic: Some genes can lead to mutation leading to infertility like, in case of BMP15 gene there is a hypergonadotrophic ovarian failure which causes infertility.
  2. Acquired: This is caused by factors like age, smoking, sexually transmitted diseases, obesity, body weight, eating disorders, diseases like diabetes, thrombophilia and also radiation and chemotherapy.

The treatment for infertility usually depends on the cause, age, duration of time for which a person has been infertile and preference of the patient. Infertility is believed to be a complex disorder and the treatment involves a huge amount of physical, financial and psychological commitments. All this also requires a lot of time. Some women might need just a minimum of two therapies to attain fertility while others might require different treatment before they can conceive.

Treatment is largely aimed to restore the fertility either by means of medication and surgery or help in reproduction with the help of some procedures.

Fertility Restoration through medication

This is the main treatment for women who are infertile because of ovulation disorders. These drugs include:

  1. Gonadotropins: These are injected to stimulate the ovaries. Drugs like Menopur, Repronex, Follistim AQ, Gonal-F, Bravelle restore the production of eggs, while Ovidrel and Pregnyl are used for maturing the eggs and for their release.
  2. Clomiphene citrate: This is taken orally and stimulates ovulation by releasing more Follicle Stimulating Hormone and Luteinizing Hormone from the pituitary gland.
  3. Metformin: This is used in females who have PCOS and also when insulin resistance plays a major role in infertility.
  4. Bromocriptine: Used when the infertility is caused due to excess prolactin production.
  5. Letrozole: This also induces ovulation

Fertility Restoration through surgery

Surgical procedures can amend the problem of infertility. However, these treatments are very rare these days owing to the success of other fertility treatments. These include:

  1. Tubal ligation reversal surgery: In case a tubal ligation has been performed it can be reversed by reconnecting the tubes again through surgery.
  2. Hysteroscopic surgery: This corrects or removes the abnormalities that cause infertility. This surgery can correct the shape of the uterus, remove endometrial polyps and improve chance of getting pregnant.
  3. Tubal surgeries: In case of blocked fallopian tubes laproscopic surgery can be performed to dilate the tubes, removed blockages or create a new opening.

Assistance in Reproduction through techniques

The most common used techniques for reproduction assistance are:

  1. Intrauterine insemination or IUI: In this about million healthy sperms are put in the uterus at the time of ovulation.
  2. Assisted reproductive technology: In this mature eggs are retrieved from a woman and this is then fertilized with sperms in a dish in the lab. Once the fertilization is done the embryo is placed in the uterus for further development. IVF is the most common and effective assisted reproductive technology. This procedure takes place over weeks with several blood tests and hormone injections.

Dealing with infertility is not easy and can be emotionally and physically draining. To cope with the various tests and treatments of infertility one should be fully prepared, eat a healthy diet, exercise reach out for help and support of family and friends and look for other options like a donor egg, adoption or no children.

Dear lybrate-user, if your blood sugar level is under controlled with your current lifestyle, there is no need to take any medicines. Diabetes is a lifestyle disease. So the best treatment is to maintain good diet and lifestyle accordingly. Just keep one thing in mind that your should be regularly monitoring your blood sugar. The most reliable test for monitoring is hba1c (shows the average bs for last three months) and if constantly it is maintained at around/ below 6.5, no need to get worried. Also at least once a year get your diabetic profile done from any reliable path lab so that any complication could be checked before time. Along with exercise focus on diet too. Frequent meals (every two hours) in small quantities is basic rule for diabetics. Keep your mind relaxed and cool. Do meditation, pranayam. Hope this would help you out. Thanks.

Mr Manoj, Hypertension and diabetes usually are seen together in many persons. Apart from taking appropriate medication, it essential to adapt a modified lifestyle, which means having a balanced and restricted diet, exercising regularly for more than 1 hour a day, avoiding sweets, deep fried foods, junk foods, alcohol and smoking. In case of hypertension salt intake also has to be low(2.3 g/day). From the data it is not clear what is level of blood pressure, which drugs are being taken for treating diabetes and current fasting and PP sugar leves and HbA1c%. Plus also inform you are exercising regularly, as your BMI is 26.19 kgs, which is high. Plus inform about dietary pattern. It will be good if you come in a private conversation with these details, where I will be able to give specific suggestions.

Exact diet for a person depends on weight, level of physical activity, sugar levels and present dietary habits. But a few things to be followed are avoid deep fried foods, sweets, direct sugar, fruits like mango, chikkoos, grapes and bananas. Also smoking and alcohol to be avoided. Non vegetarians should avoid red meat, fish and chicken are preferred.

if a person has developed hypothyroidism (lack of thyroxine hormone) then only remedy is to take the hormone under the guidance of a competent doctor. Dosage has to be decided based on tsh level and patient's weight. Please consult your doctor and start the treatment.

You should better consult any dietitian in your area for diet planning, according to his age, body structure and diseases. For diabetics, small and frequent meals are essential.

HI Sir, I am 35 years male. I have feeling sickness from last 2 month. Which includes body pain. Stomach upset and lower leg pain. Blood test shows TSH 7.44 ml of reference rang. 270 to 4.20. T3 and t4 is normal, I got some wait loss around 2 kg from last 2 month. I took thyronorm 25mg for one month and stop. Should I test TSH again? Now I am facing lots of stomach cramp and pain. Nov 2015 I did upper GI endoscopy report it says patulous GE JUNCTION GR'A' OSEOPHAGITS. Dr. advice me to take PANTOCID-l and MEVA-C caps. I took for one month and stop. Should I continue this? I am taking razo 20 every day morning but not getting relief from gas and stomach pain I bit worried about my health.

Hi lybrate-user I am surprised you have discontinued your medicines. Thyroid medications are generally needed lifelong. Gastritus and bloating problems need a lifestyle and medicine together to get better.. Accordingly Thyroid hormone replacement is given according to the most recent report snd your Body weight. You need to repeat FT3/FT4/TSH: empty stomach blood to be given. Share your current reports I can help you better. It's important  you maintain  your body weight exercise regularly. Weight gain increases thyroid hormone requirements. Avoid cruciferous   group of veggies  like cabbage and broccoli. Avoid soya bean in all forms. No soya, nugget, chunk, tofu or milk. Limit their intake: Brussels sprouts, cabbage, cauliflower, kale, turnips, and bok choy. Cooking the vegetables can reduce the effect that cruciferous vegetables have on the thyroid gland. Limiting your intake to 5 ounces a day appears to have no adverse effect on thyroid function. Decrease gluten  intake from processed  food. Always try whole grain varieties of cereals which is rich in fibres and help improve  bowels (Take your thyroid replacement  medicine several hours before or several hours after food rich in fibres. Otherwise problem  with absorption  of hormone. 20 to 35 gram of fibres daily from all sources. Avoid excessive fatty (deep fried) and sugary food as hypothyroidism  causes your metabolism  to slow down &weight gain. Avoid highly salted food, chips, pickles, sauces as they Contain  large amount of sodium. Hypothyroidism  puts you at risk of high BP and excess sodium in salt aggravates Coffee  in excess. And coffee with thyroid hormone  replacement  can prevent absorption. Alcohol. It is toxic to thyroid gland and prevents  hormone production and absorption. The problems of hypothyroidism  gets better with medicines, diet and exercise. You will start better as the levels of your hormone normalises. Initially  you need to do the TSH test every 6 weeks till normal TSH then every 3 months For bloating and acidity Diet, exercise, medicines. If control is poor with once a day dose medicines may have to be changed. Avoid spicy, fried food Have plenty of water and green veggies, fruits, curd. Exercise for 40 minutes every day. Go to sleep atleast 2 hours after a meal. Hope this helps

Check your blood sugar levels. If it is uncontrolled than also you can have bodypain. There are other reason as well. You can consult with me regarding further management.

I am 32 years old. I have thyroid problem. Initially it was hyper. And after taking radiio iodine, it became hypo. As of now I am taking thyronorm 100. I am gaining weight day by day. I have checked the tsh values last month. It was high. So now I am taking 150 thyronorm. How can I keep my thyroid in control?

Madam, with random blood sugar level of 250 mg, your present treatment is definitely not adequate, you need to look at increasing the dosage or adding a newer drug. Plus consider restricted diet and regular exercise. As regards other branches of medical practices for treating a severe diabetes, there is not going to be any benefit. Those drugs can be used only once a good sugar control is achieved, justas additional supportive therapy. In the world today other than oral antidiabetic drugs and insulin there is virtually no treatment available. So please look at these facts. Further choice is yours. With regards.

My wife is suffering for Diabetes Last 5 years. Her PP is 155 (after medicine taken), Fasting is 140 and HBA1c is 8.9. Now she taken the medicine as Trajenta 5 (1 in a day), Glizid Mr. 60 (1 in a day), Cetapin XR 1000 (2 in a day) as doctor advice. So please advice what kind of precaution taken with out medicine?

As hba1c is on higher side, it's important to be particular about food choices & timings. Properly follow the do's and don't of a diabetic diet. Avoid sweets & sweetened beverages from diet. Avoid high starchy foods and sweetened fruits & vegetables from diet such as rice, bananas, chickoo, grapes, mango, litchi, potatoes. Include more fiber such as dalia, & whole pulses in diet as they have low glycemic index and helps to control blood glucose levels.
