DCT Test

DCT lab test or Direct Coombs Test is performed to detect the cause of anaemia due to autoimmune disorder. Normally, pale skin, cold hands and feet, shortness of breath and fatigue are common symptoms of anaemia. Doctors often advise DCT test to patients with persisting anaemia. The DCT test unravels the cause of such persistent anaemia. Many diseases and prolonged use of strong drugs produce excessive antibodies in the body. The antibodies stay bound to the surface of the red blood cells (RBCs) and in course of time, these antibodies destroy the RBCs causing severe anaemia. The DCT test helps to determine whether these antibodies are destroying the RBCs.

DCT test usually requires no special preparation. Ideally, this test is carried out on the advice of a doctor when the anaemia is unexplained. Before the DCT test, it’s correct to discuss about your current medications with the doctor. In some cases, your doctor may advise you to discontinue medication so that test results are accurate. Whatever the doctor’s advice, follow it unconditionally. Remember to wear sleeveless or short sleeve dress or shirt, so that blood sample collection is easily done.

Normal results mean that there is no agglutination or clumping of red blood cells. Abnormal results mean that the red blood cells are being destroyed due to agglutination or clumping of RBCs. A condition called hemolysis that causes the destruction of red blood cells by the immune system leading to autoimmune hemolytic anemia. Drug toxicity can also be a ruling factor for antibodies to develop and behave abnormally. Observations reveal that aged people have abnormal DCT test result without any risk factors or disease.

Just like other blood tests, you sit comfortably with your arm resting on a supportive prop. Before the venipuncture, a tourniquet is tied to your upper arm so as to locate the vein easily. The tourniquet is released as the sterilized needle is gently pushed into your vein. EDTA vial of 4 ml blood is the quantity of blood that is drawn as sample. The blood sample is stored in a test tube.

In case the DCT test is being carried out on an infant, venipuncture is avoided. Either the heel of a foot or the finger of a hand is pricked with a sterilized, use and throw needle. The blood sample may be collected into a small glass tube, a glass slide, or on a test strip.

Maintaining good hygiene is essential. Lukewarm water can be used. Do use cold water later. Do not frequently touch the pimples as that can lead to infection. Mr. lybrate-user, Thanks for the query. First thing to be done is to do a Thyroid Function test, check the hormone levels and come back with the results. The swelling could be due to hypothyroidism or due to hyper activity. Plus there is a need to get ultra sonography of the gland should be done. Please do not rub any juice on the gland. It may do more harm than any good. The thyroid swellings occur because of some internal functional problems. Which can be detected only by proper testing. Hence you may have to go to an endocrinologist near by for getting these things done. Thanks.

In hypothyroidism if TSH is not well controlled weight increase, dry skin, dry hair and hair fall, irregular menstruation, muscle and joint pains, swelling of face and legs and anxiety etc symptoms are quite common. First thing is to bring down TSH level <3 mU/L. Without that there is no improvement. Plus efforts to reduce weight will not succeed. Same is the case with dry skin. So please check your TSH and come back. There is no special diet required, what is needed is to reduce the food intake so that caloric intake matches your ideal body weight (62 kgs). Plus regular exercise. Madam if you can not give time to maintain your own health then there is no way I can be of help. Thanks.

For epidural analgesia, a catheter (small tube) is inserted into the epidural space in the spinal cord. It is through this tube that the drugs (painkillers) reach the desired nerves. 

Types of Epidurals: 

Epidurals can be 

  1. Injection with top-ups: Once given, epidurals help to relieve the pain that arises due to contractions. When the epidural starts to wear off, top-ups or refills are used. The refills can last for 1-2 hours. 
  2. Combined Spinal Epidural: Compared to the regular epidurals, the combined spinal epidurals are powerful, quick, and more effective. Here too, epidural painkillers are made to pass through the catheter, once the first injection wears off. It is also known as Walking Epidural. 
  3. Continuous infusion (patient-controlled epidural analgesia): Here, the other end of the epidural catheter is attached to a pump. This pump continuously infuses the painkillers into your back. For an effective response, top-ups may be used. Many hospitals allow patients to control the pump. 

Merits and demerits of having an epidural--

Merits: 

  1. Epidurals make the delivery smooth and hassle free. 
  2. Epidurals are relatively safe as only a small fraction of the medicine reaches the baby. 
  3. Women stay fully awake and experience no pain during the cervix dilation. This enables them to push the baby out with more energy.
  4. Depending on the requirement, the dosage of epidurals can be adjusted. 
  5. Epidurals help to lower the cases of C-section delivery. 

Demerits: 
Though epidurals make childbirth easy, one cannot overlook its demerits. 

  1. Infections can sometimes occur. 
  2. Sometimes epidural analgesia can have a patchy effect, causing some pain to persist. 
  3. Though rare, epidural can result in nerve damage. 
  4. Epidurals bring about a fall in blood pressure. At times, this can be risky for the baby. 
  5. Nausea, headache, itchiness, backache, soreness are the common side effects of epidurals. 
  6. Epidurals relax a woman, thereby reducing their urge to push the baby out.
