Hello, Every antipsychotic medication is equally efficacious. If there is partial response then medication dose need to be increased and if there is no response than drug is supposed to be changed. Important part being dose given at a range which is effective. Do not switch to other drug until given at required effective dosage and for required period of time. At 57 years usually the dosage is kept low and gradually increased to monitor side effects. It is advisable to have healthy meals and regular exercise as these drugs increases chances of weight gain and its further consequences. You can contact for any further help.

Dear sir, If you are getting responses in BERA test, then it's unlikely that you have auditory neuropathy, there may be some other central auditory pathway disorders. Please consult some qualified and experienced audiologist for proper BERA test and CAPD testing.

Dear Mr. Dhawan, Any particular molecule cant be claimed as superior as response varies from person to person. One agent that is good for a particular person may not work that well for other. However, clozapine is one molecule that is considered superior to all others in resistant cases and is generally given when other medicines fail. Partial agonists such as aripiprazole have a less side effects but sometimes efficacy is found wanting. Your patient will need evaluation and appropriate suggestion. Take care!

Mr. lybrate-user, I would like to tell one thing, schizophrenia is chronic illness. Basically it requires anti psychotic drugs for treatment. There are numbers of anti psychotic drugs availabale in market for treatment of Schizophrenia. Some drugs are FDA approved, some are superior in one trial but not in other trial. Some are popular in few countries but not FDA approved. Actually selection of proper antipsychotic depends on number of conditions viz. Age, sex, duration of illness, associated co morbid condition, any medical condition, substance history, organicity, prior drug response, socio-economical status etc. As in your case female patient having auditory hallucinations, probably case of schizophrenia needs proper medication consedering above mentioned condition. Now come to your basic question, she is taking clozapine 200 mg. It is also very efficacious drug. You talked about clozaprexine. It is considered as a prodrug of clozapine. It is investigational new drug, originally created at Harward University. Atype of fatty acid (DHA) was added to clozapine in order to increase penetration of blood brain barrier. This drug was initially tested in rats, but no obvous findings have been found in subsequent studies. It is not currently available in INDIA. Ihope it might help you.

Schizophrenia is a chronic disorder and needs a longterm treatment. Auditory hallucinations can be treated, changing her antipsychotic medication can be tried if not, there are other psychological methods of treatment for auditory hallucinations. Cognitive behavior therapy for auditory hallucinations has shown good results.
