My mom, age 60 years have hypertensive crisis (180/120) a week ago and have been vomiting and experiencing severe headache for a week. She did take some antihypertensive medication and her bp falls to 150/100 but even so headache and vomiting still present. She also did lumber puncture and the result was: Amount: 1 ml Color: colourless appearance: clear coagulation: nill protein: 50mgs% sugar: 76mgs% total cell count: 04cells/cumm differential cell count: all lymphocytes malignant cells: not seen others: RBC ++ can you please tell me what the result is and what should she do next. Does the presence of Rbc indicate cerebral hemorrhage?

Hi, RBC in lumbar puncture mostly indicate traumatic CSF means blood from layers of skin mixes with csf. For Subarachnoid haemorrhage xanthochromia is diagnostic. Its very difficult you to understand. I just want to tell you that get a non contrast CT brain, it will exclude cerebral haemorrhage. In a hypertensive patient with vomiting & headache one must exclude cerebral haemorrhage.

Epilepsy, First seizure happens till now Actually I want to marry a girl who suffering from epilepsy & she faced only one seizure at the age of 12 years after starting the treatment she never face any seizure till this time i.e. 26 years of age but she needs to take a proper nap in night which was recommended by doctor. I need your recommendation over this issue.

Hi only single seizure episode at 12 years of age. She needs tapering off the medications. Before that go for MRI brain and EEG. If this two are normal then her medicine can be stopped. There will not be any problem in marrying her.

Hello sir my father age 63 feel continuous headache, giddiness,weakness and sway to right side fundus normal past 2 month. He has done mri brain and cervical on 27/10/2017 report are as follow. MRI HEAD-mri brain finding reveal a moderately enhancing t2 hyperintense intraventricular mass lesion seen in inferior aspect of 4th ventricle measuring approx 2.9*2.9*2.5 cms in size with swi hypointense calcific areas seen within lesion .the lesion causing mass effect with anterior displacement of medulla oblongata and obstruction of inferior aspect of 4th ventricle with moderate dilation of b/l lateral and 3th ventricle with csf ooze around both ventricle. There is edema in medulla oblongata. Differentials include subependymoma and ependymoma need clinical and histopathological correlation MRI cervical-Disc dessication involving cervical discs with osteophytes seen at the end of vertebra. There is posterior disc bulge involving C5-6 disc causing mild indentation of thecal sac. My father is diabetic and hypertension problem. Please help me and tell me surgery is safe in this age with diabetic and hypertension problem.

Hello, first of all I congratulate you for giving a detailed history. Regarding management of patient, he should undergo surgery to prevent further complications. Already there is obstructive hydrocephalus. Hypertension and diabetes are not a contraindication for surgery unless its uncontrolled. In any Brain surgery, there is always a risk, but I think you should go for that.

I was epileptic since 1988. I was diagnosed with eptoin to take and then after few years mazetol200. Since then I am taking this Medicine one in the night. But I don't have fits since 13 years. I am 47 year of age. Please advise.

Since last 13 years you are seizure free. So you do MRI brain and EEG. And If the reports are normal, then your anti seizure drugs should be tapered slowly and stopped.
