Here are some tips for how to handle your back pain: -stop normal physical activity for the first few days. This will help relieve your symptoms and reduce any swelling in the area of the pain. -Apply heat or ice to the painful area. One good method is to use ice for the first 48 to 72 hours, and then use heat. Take pain relievers medicine. --While sleeping, try lying in a curled-up, fetal position with a pillow between your legs. If you usually sleep on your back, place a pillow or rolled towel under your knees to relieve pressure. -Do not perform activities that involve heavy lifting or twisting of your back for the first 6 weeks after the pain begins. -Avoid bending forward and sitting in a place for long time -Stretching and strengthening exercises are important. But, starting these exercises too soon after an injury can make your pain worse. -Some Physiotherapy treatment like IFT can give you some relief . -A physical therapist can tell you when to begin stretching and strengthening exercises and how to do them. There are many conditions that can cause knee pain. - Ice is useful for the control of pain and swelling. It can be applied to the knee for 15 to 20 minutes as often as every two to four hours. The swollen knee should be elevated above the level of the heart while icing. -activities that cause pain should be avoided temporarily. -If the knee is swollen or sore, the following positions and activities should be avoided until knee pain and swelling resolve: Squatting Kneeling Twisting and pivoting Jogging Aerobics, dancing Playing sport You need detail assessment with a Physiotherapist to find out what causing you this pain and pain management for your back and knee pain. Best wishes

I am having pain in my knee and back. Last year I had stone in my kidney. Is it has something to do with that? what should I do?

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist, Delhi
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Well renal stone can lead to a severe pain from your lower flank region on one side going towards your groin region. So if you are having that kind of pain, please consult your urologist. But you should not be having any knee pain because the pain will not go till there. Anyways get your urologist opinion done and let me know, I can advise you from an orthopaedic point of view for your back pain and associated knee pain. Do not hesitate to contact me if you need any further assistance.

Advice. Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold).

I am 82 years old male. I am suffering from left knee arthritic pain. I have been managing with homoeopathy and pain killers. Now, the pain is becoming worse, making it difficult for me to walk. I want to avoid surgery. Have you any advice?

I am 82 years old male. I am suffering from left knee arthritic pain. I have been managing with homoeopathy and pain ...
Hello 82 is a ave where your knee joint gets eroded a lot which leads to tremendous pain when you walk or climbs stairs. Hence you need to take care of your knee in order to decrease the joint loading by unloading it, the way to unload it, is to keep it hydrated by sitting at the edge of the couch/bed and tie a 2 kg weight cuff at the ankle and move/swing your knee in easy gentle low amplitude oscillations for ten minutes twice daily. Wear a knee brace with hinge daily when you are off the bed. Have lots of fresh fruits specially citrus fruits. Have anjeer four piece twice daily. Avoid lady finger, spicy foods, avoid peas, brinjal and tomatoes. Avoid mustard oil. Avoid tobacco in any form (if at all you consume) do knee strengthening (quadriceps, vmo, gluteus, tfl, hamsrings) exercises and hamstring and quadriceps stretching. Quadriceps drill. Partial squatting (30-40 degree bend) if your knee permits. If possible visit a skilled orthopedic physiotherapist. Hope it helps. Take care.

Intra-articular Injection in the Knee For Osteoarthritis

MBBS, MS - Orthopaedics, Fellowship in Joint Replacement , M.Ch - Orthopaedics

Intra-articular Injection in the Knee For Osteoarthritis

What are they?

Intra-articular injections or intra-articular corticosteroid injections are steroid medicines that are injected into the joint space directly, for curing and treating painful and inflammatory arthritic joint syndromes. In general, this procedure of treatment is adopted by orthopedic practitioners since steroidal drugs, when administered orally, are hardly of any use for osteoarthritis patients.

Steroids are natural substances which are akin to the hormones in your body. Steroids are mostly taken for reducing inflammation. Apart from inflammation in the bones, intra-articular corticosteroid injections are also administered for alleviating inflammation in the ligaments and tendons in patients suffering from osteoarthritis.

Note that intra-articular corticosteroid injections are prescribed to osteoarthritic patients only if the pain and inflammation do not subside with the use of NSAIDs and analgesics. Corticosteroid injection often provides temporary relief, when other oral medicines have failed to provide comfort to the patient.

Hyaluronic Acid Injections

Another form of intra-articular injection is the one containing intra-articular hyaluronic acid. Hyaluronic acid is a natural fluid that aids in lubricating the joints and allows them to function smoothly. It acts as a shock absorber in your body. However, with osteoarthritis, the hyaluronic acid in the affected joints wears out. Depending on the type of injection, you may get one shot of hyaluronic acid intra-articular injection, or three to five injections spaced a week apart.

The Process

The process of administering such injections needs the doctor to first clean the area. In case of swelling, local painkillers can be injected. Next, the doctor inserts a needle to withdraw the excess fluid that’s within the knee joint and then with the same needle in place, the doctor finally injects the medication into the knee joint.

The basic side-effects of intra-articular injections include mild pain at the site of the injection along with a small buildup of joint fluid. However, these side-effects are usually never a matter of concern and they get resolved on their own within a span of a few days. If you wish to discuss about any specific problem, you can consult an orthopedist.

U might be having an acute attack of rheumatism. I won't advice pregnancy in a acute attack. Again the chance of transmission to the baby should be ruled out. Have your disease controlled first n then plan pregnancy.

Go for fasting once a week. You can go for absolute fasting or rely on a liquid diet to digest ama (toxin). Fasting should be followed by hot fomentation or by taking a bath with lukewarm water or steam bath. After fasting and fomentation, mix powdered long pepper, black pepper and dried ginger in equal quantities and take 1 teaspoon of this powdered mix with lukewarm water twice a day. This will help you restore your digestive fire. Take one tablespoon of castor oil before going to bed, once or twice a week. It helps in cleansing ama (toxin) build up and balancing vata. Mix equal quantities of powdered fenugreek seeds, turmeric and ginger powder. Take one teaspoon daily in the morning and evening. For dry fomentation of the affected joints, take a cloth and fold it into four, fill it with hot and sand make a potli (bag) out of it. Use this bundle for the fomentation of different joints. This is very helpful in reducing pain and swelling.

Patient needs to be examined in detail & be investigated. Anyway you may try- sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck back knee & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take. For emergency treatment contact your nearest hospital or family doctor. If it does not give relief in 4-5 days then inform me.

My wife, aged 45 years, is suffering from osteoarthritis and is having acute pain on the right leg now. She had this problem initially on her left leg since the delivery of her second child and has been diagnosed as the above condition and was under medication (both ayurvedic and allopathic) for a few months till the pain got subsided. As this problem persists, we have already consulted with an orthopedic about the nagging pain and he has suggested a knee replacement surgery based on the x-ray images taken recently. We request an opinion from you on the alternative ways available (for instance, ayurvedic).

DNB Rheumatology, Grad. Cert. Paed. Rheumat, MD - Paediatrics, MBBS
Rheumatologist, Bangalore
At 45 years it is unlikely that the cause of your wife problem is osteoarthritis. Her diagnosis needs to be confirmed by a proper evaluation by a rheumatologist. Please meet a good rheumatologist before going in for ant surgery. This is because in case her problem is due to some other disease other than osteoarthritis then she is likely to have problems in the knee again EVEN after surgery.
