How Laparoscopy Is Beneficial In Gallstone Surgery?

Gallstones are a medical condition in which the formation of stones takes place in the bile duct or the gallbladder. One of the major factors responsible for the formation of gallstones is an elevated excretion of cholesterol by the liver, most of which remains undissolved by the bile. The undissolved cholesterol may crystallize resulting in the formation of gallstones (yellow cholesterol stones, a condition termed as Cholesterol Gallstones). In some cases, increased level of bilirubin in the bile (triggered by a liver problem, liver damage or other medical conditions), which doesn't undergo a breakdown, may lead to stone formation (known as Pigment Gallstones). Here, the stones appear black or dark brown in color.

Gallstones are common among women, especially those who are 40 years and more. Obesity, diabetes, liver disorders, unhealthy diet (rich in fats and cholesterol), certain medications (those containing estrogen) can also trigger the formation of gallstones. Gallstones left untreated and unattended can give rise to serious complications such as blockage of the pancreatic duct (resulting in Pancreatitis) or the bile duct. There may also be inflammation of the gallbladder. In extreme cases, a person may even suffer from something as serious as Gallbladder Cancer.

Laparoscopy to remove the gallstones

  • While a lot of treatments and medications are available to deal with gallstones, most people opt for Laparoscopy. What makes laparoscopic surgery superior to the other open surgeries is that it is minimally invasive with a better and quick recovery. Also termed as Cholecystectomy, the laparoscopic surgery for the removal of gallstones as well as the gallbladder involves the following steps.
  • General anesthesia is given to the patients before the surgery.
  • The surgeon makes 3-4 small incisions in the abdomen.
  • The surgeon then inflates the stomach to get a better view of the internal organs (including the organs affected). Carbon dioxide gas is passed into the stomach to inflate it.
  • In the next step, the surgeon carefully inserts a laparoscope (a narrow and long tube that comes with a high-density light and a front camera with a high-resolution) through one of the incisions (usually the one close to the belly button) to aid in the surgery.
  • Nex,t the surgeon inserts the surgical instruments needed to get the gallstones and the gallbladder removed.
  • Before the surgical removal of the gallstones and the gallbladder, an important X-ray of the bile duct called the Intraoperative Cholangiography (shows the bile duct anatomy) is done.
  • With the gallbladder and the stones removed, the laparoscope and the surgical instruments are then removed and the incisions stitched carefully.
  • The gallbladder is known to store the bile pigment. With the removal of the gallbladder, there is a small rearrangement. The bile pigments now move from the liver into the small intestine via the bile duct.
  • The patients may require spending 1-2 days in the hospital. The patient is expected to be in a better shape (less discomfort) within 2-3 weeks.
  • In case complications arise during the surgery, the surgeon may have to switch onto an open surgery.

Gallstones - Know The Different Types!

Gallstones can be the cause behind quite some pain and discomfort if not treated by an Ayurvedic medical expert. This helps you get over the ailment before it can inflict any more pain. Before learning about the treatments for the condition, it is important to learn what exactly is a gallstone? A gallstone as the name suggests is a peculiar stone-like formation found in the gall bladder. The stones vary in size and number from person to person and are mainly constituted from bile components that settle inside gallbladder. Gallstones as mentioned above are only stone-like formations and not really stones. They are formed from the cholesterol, bilirubin and salt contained in the bile.

How are gallstones diagnosed?
In the majority of cases, it becomes impossible to detect whether a person has gallstones or not. It is only when the stones close or block a specific duct and become the cause for inflicting some pain, does the chance of the presence of a gallstone crosses the mind. Different types of Gallstones:

The main building component of these pigment stones is bilirubin. The bilirubin, in turn, comes from the bile, which is an enzyme secreted by the liver and carried into the gallbladder to be stored there.
  • Cholesterol Stones: The Cholesterol stones, on the other hand, are the most common kind of gallstone there is. 80% of the times, patients are detected to have cholesterol stones. These stones are characterised with a yellowish green colour.

Causes leading to Gallstone:
Only when gallstones reach a particular size or an overwhelming number that they start blocking ducts, which in turn cause the pain. But what results in the formation of these gallstones? There are a number of things that can lead to the formation of these stone like formations in your gallbladder. Some of these reasons include-

  • Increase in weight
  • A non-efficiently functioning gallbladder
  • Following a wrong diet
  • Over consumption of cholesterol rich food
  • Genes, and most commonly
  • Bile

While these are the common causes for bile stones, the most common causes of pigment stones are certain kinds of medical conditions, such as- sickle cell anaemia or other blood diseases and cirrhosis of the liver.

Ayurvedic cures for Gallstones:
According to Ayurveda, there are three doshas, namely the Kapha, pitta, and Vata, which lead to the formation of gallstones. Opting for Ayurvedic treatment helps you eliminate the requirement of harsh allopathic medication or for that matter surgery. The three major point covered by Ayurvedic treatment for gallstones include- modification in diet, herbal and all natural Ayurvedic medication and detoxification.

Ayurvedic gallstone treatment includes detoxification, herbal medications and diet modifications. A common therapy is a liver flush, which can include olive oil, lemon juice and spices. Sir. I have burning mouth and tongue recurring. Especially my tip of tongue feel soft and hypersalivation is present. I am on Tab. Metolar 50 mg. Trika. 5.mg. cholecystectomy done 2016. Food poisoning is very common and irregular bowels. Acid reflux is high. My teeth is misaligned. Please advice.

My mother was having bile duct stone 16 mm now that is removed before two days, doctor had advised her to take udiliv 300 for 1 month is this medicine safe and useful for her, this was her 3rd ercp. Please doctor give your advice.

You have to improve your food habits do 1. Take 2/ 3 glass of warm water in the morning before brush 2. Take more water in day 3. Take meals at fixed hrs, chew food properly/ completely, no eating quickly 4. Take small amount of food at a time, take more frequent meals - may be five times a day. 5. No spicy/ fried/ fast/ junk/processed food. 6. No smoking, chewing gum, and carbonated beverages. 6. No milk for few days, can take curd, no uncooked salad 7. Avoid constipation 8. Use nibu pani (lemon water) 2/ 3 times a day 9. Do not drink/ store water in plastic bottles. 10.relax and walk for 30 mts daily 11. Take enough rest- do not lie down immediately after eating. It is often a misconception that you can lose fat from a specific place. If you lose fat from the body, then the fat loss would be from the entire body and won’t be restricted to a specific area of your body. Exercise and diet combined together form an effective tool which you can use to lose fat from your entire body.

The fat in the body is stored in the form of triglycerides, which the body cannot break down for energy. The triglycerides are first broken down to glycerol and fatty acids, which are then released into the bloodstream, following which they can be used as energy. 

Initially the body uses the available carbohydrates present in the blood for energy; fat is only used as fuel after the carbohydrates supply is exhausted. Hence, prolonged steady state exercise, such as jogging for 50-60 minutes is an efficient method of burning fat. Another strategy you may employ to lose fat from your body is by a method called high intensity interval training. 

High intensity interval training (HIIT) is increasingly becoming the new fad in fat loss. This method utilizes a concept where you do an exercise at full thrust for a short time of 20-30 seconds followed by a short rest period of 10-15 seconds; this comprises of one set. Repeat for another 9 to 10 sets for a proper HIIT workout. The idea is to burn energy during the exercise and after the exercise is complete.

The exercises in HIIT are done at full throttle which means your heart rate centers around an anaerobic zone and this creates an oxygen shortage in the body. An effect called EPOC (excess post-exercise oxygen consumption) is induced in the body that results in an elevated metabolism for up to 24-48 hours after the workout. So you would be burning fat simply by doing nothing.

The fat loss process occurs when the fat cells around the body shrink. Doing body specific exercise such as crunches to lose fat from the stomach does not work and will only work the muscle underneath the fat. The liver is a vital organ in our body because it performs many important functions. This includes filtering harmful substances in our blood, producing bile to facilitate digestion of food, storing energy and nutrients as well as producing hormones and enzymes that the body uses for performing essential functions.However, too much consumption of alcohol can affect the liver, lead to serious consequences related to our health, and also result in an alcohol-related liver disease (ALD). An ALD may, in turn, cause disruption of the digestion process and weaker immune system. It may even increase the risk of developing cancer.

Causes:

As the name suggests, the prime cause behind the development of the alcohol-induced liver disease is the consumption of alcohol for a long duration. However, some of the other causes behind the development of alcohol-induced liver disease include the presence of an existing liver disease such as fatty liver or hepatitis, inherited liver disease, or a liver disease induced due to excessive or prolonged consumption of certain drugs.

Alcohol-induced liver diseases (ALD) can be of three types. 

  1. Alcoholic fatty liver disease: Alcoholic fatty liver disease or steatosis occurs due to the deposition of fats in the liver, which is an end result of excessive alcohol consumption.
  2. Alcoholic hepatitis: Too much of alcohol consumption can also lead to liver inflammation, which is called alcoholic hepatitis.
  3. Alcoholic cirrhosis: This alcoholic-liver disease occurs when the liver is scarred due to excessive consumption of alcohol. In this case, the healthy and soft liver tissues are replaced with dead and hard tissues.

Symptoms: Some of the signs and symptoms, mostly related to ALD, are not noticed in the initial stage. It is only after a few years that the symptoms may appear. Some of the common symptoms include abdominal pain, loss of appetite and weight loss, swelling in the abdomen as well as the legs, feet, and ankles, a yellowish appearance of the skin, itchy skin with hair loss, and bleeding and complications such as liver cancer or ascites.

Diagnosing alcohol-related liver disease is based on your history of alcohol consumption. But, there is no single test that can detect this type of disease, so a combination of tests will be suggested by your physician before a specific type of alcoholic liver disease is diagnosed.

Ayurvedic home remedies

Ayurveda is known to be greatly beneficial for the treatment of alcohol-induced liver diseases. The use of natural ingredients like herbs can not only treat the liver disease but also promote the overall well-being of the liver. The treatment will include abstinence from alcohol, as it is very important when it comes to undoing the damage that alcohol has caused to your liver. The natural ingredients that can treat alcohol-induced liver disease include:
