He completed his schooling from Central School, N.Delhi (C.B.S.C), in 1971. He passed his M.B.B.S from the University of New Delhi, in December 1980. He then joined the Indian Army Medical Corps, in 1981. While serving in the Indian Army, he completed his Masters in General Surgery from ‘Armed Forces Medical College’, Pune in May 1990. Here, he had immense experience in trauma surgery and various emergency surgeries. In August 1997, he resigned from the Armed Forces as a Lieutenant colonel. He then worked for a few years in a couple of major hospitals in Kerala, in the departments of general surgery and gasterosurgery, carrying out all types of elective and emergency surgeries, including surgeries for gastro-intestinal malignancies and laparoscopic gastro-intestinal surgeries. In May 2004 he joined the ‘Amrita Institute of Medical Sciences and research centre’, Kochi, Kerala, a 1400 bed, tertiary referral hospital, as the chief of the Unit of diabetic lower-limb and Podiatric Surgery. He has been the organizing secretary of the International ‘Amrita Endocrinology and Diabetic Foot Conference’, held once in two years, in the institute campus. On 1st June 2015 he joined the Aster Medcity Hospital, a quartinary care institute, Kochi, Kerala, as Senior Consultant of the Department of Podiatric Surgery. This is the largest of its kind in India, and concentrates mainly on limb salvage. Cases managed include extensive necrotizing fasciitis, plantar-space infections, diabetic gangrene of the lower limb and osteomylites, and severely destroyed foot and ankle Charcot. This is the only center in India where foot and ankle reconstructive surgeries are being performed routinely in large numbers, in high-risk Diabetic patients. He has devised a novel technique of foot and ankle stabilization, ‘The Amrita Sling Technique.’ He has also devised techniques of using Polymethyl methacrylate, to replace severely destroyed Charcot foot bones. These manuscripts have been published in international medical journals, and the surgical videos have been uploaded in the You Tube. He has also developed a new polymer based footwear for diabetic patients, which has been ‘patented’. In January 2010, he was conferred with the prestigious Dr.S.C.Misra medical oration award. He is the principal investigator in a number of major research programmes. Most notable among these is the new invention of an entire foot bones replacement prosthesis. This again has been provisionally patented. By comprehensive management, the center has been able to maintain a limb salvage rate of 91.5%, in diabetic foot and lower-limb problems, comparable to the best centers in the world. He has over 200 research papers published in national and international journals. He has hands on experience of working in eminent diabetic foot centers in the United Kingdom and the United States of America. He has been invited as a guest faculty for many national and international Diabetic Foot conferences, the most notable being to the premier ‘World Diabetic Foot Conference (DFCon—2014)’, held at Los Angeles from 19th March ’14 to in 22nd March 2014. He was invited to speak on two topics, one on ‘Salvage procedures in Diabetic Foot infections’, and a ‘Video presentation of the ‘novel foot and ankle reconstruction techniques’, which he has devised. I have devised, For the First time in the world, a novel technique of foot and ankle stabilization, ‘The Amrita Sling Technique.’ This is lieu of an external fixator, after internal fixation. I have also devised techniques of using Polymethyl methacrylate, to replace severely destroyed Charcot foot bones. These surgeries have been uploaded in the You-Tube: The links are: 1. In all probability you have bacterial infection of the 1st toe of your Rt foot. Please limit your ambulation to the minimum. Please keep the right foot elevated on a chair while being seated to reduce swelling. You may need to take antibiotics, and hence please consult a general surgeon, who can examine you and prescribe the appropriate antibiotic if required.

It depends on the cause of Elephantiasis. If it is due to Filariasis, then she can have recurrent lymphangitis and associated fever and leg pain. This can be prevented by taking a medicine called Banocide, once every 6 months. Please consult a general physician who can assess the condition and prescribe the correct dosage and duration of the medication.

If it is confirmed that the abdominal pain you are having is due to Gall Stones, and you really are very symptomatic, it is best to get the GB removed by minimal access surgery.

As per your history, it seems to be an allergic reaction at the neck area, possibly due to an insect bite. The best way to manage it would be to apply Calamine lotion on the area three times a day. With this, in a day or two the allergy should resolve.

Take the positive aspect you have first. You do have a good appetite. Constipation and loose motions can be overcome when you have the right type of food. High fiber diet, fruits, vegetables, drink about 2 liters of water a day at least. This diet would even reduce the bloating feeling of the abdomen. In your case, taking smaller amount of food may be five times a day is better than consuming larger amount of food three times a day. Regarding gall stones and blocked hepato-splenic veins, please do consult a gastroenterologist regarding management.

The wound on the back can be dressed daily, first by cleaning with 5% Betadine solution. Then after applying Metrogyl gel, sterile gauze pad can be used for dressing. Once the wound has healed, to prevent further infection or abscess, personal hygiene is very important. You may take daily bath and wash the area with soap and water. This would help prevent such problems in future.

I having suffering l4 l5 disc bulge issue. I am planing to go to jogging everyday dose it effect on my disc pain.
If the L4/5 disc bulge is causing you no symptoms, then it is very fine for you to go jogging. However, you must do good warm up exercises before jogging, especially for spine flexibility, so that the disc prolapse will not aggravate and cause you symptoms. Yoga is the best form of warm up you can do.

Yes, it is very important for you to remove the existing corns and prevent further corns or callus formation of your feet. Since you are a Diabetic, corns and callus can become ulcerated, infected and cause sometimes serious foot problems. The best way to remove the corns and callus is to surgically scrape them and to be done by an expert. Please do not do it yourself or let any untrained person do it. Once these are removed, you have to wear foot wear made of soft, pressure distributing material like E.V.A. Elastoplast, Poron. Please keep the skin of your feet soft by applying Vaseline on the plantar, dorsal surface of your feet and the legs up till the knees twice a day. Please wear a cotton socks while wearing the diabetic footwear.

Left side abdominal pain in your case is due to the small left renal stones. Since these are small, presently you may try taking at least 2 to 3 liters or more of fluids daily. This can wash out the stones in time. You may get an abdominal USG done once in 3 to 4 months to see for the status of the stones and consult an Urologist if they are getting bigger.

AtL5-S1 a focal posterior central disc bulge with tear, mild to moderate canal stenosis, compression over the alloy sac, left traversing S1 nerve root in lateral recess. Neural foramina on either side however exiting nerve root look free in neural foramen. Hip n screening within normal limits. This is the report of MRI. please suggest.

MBBS, M.S. General Surgery
Podiatrist, Kochi
The MRI report informs that there is intervertibral disc prolapse at L4/5 area with cord compression. There is also disc degeneration of L5 SI. You probably would be having low back pain and pain and discomfort of the lower limbs. If you are markedly symptomatic, then minimal access surgery is indicated to remove the involved disc and relieve cord compression and symptoms. You can consult a Neurosurgeon or Orthopedic surgeon for the same. If you do not have any marked symptoms, doctor would probably advise physiotherapy and wait to see if it relieves your discomfort and then advise accordingly.
