Hello, thanx for using Lybrate first do further blood tests for your husband like Liver function test HBV DNA level quantitative HbeAg those tests will tell about the infectivity of your husband that will also help in advising contact precautions. For you and your daughter do HbsAg testing first. As your daughter is born after 2000 she might be already vaccinated for HbsAg so do Anti HBS titer that will tell us about protectivity. Then if it is negative then you need to take four course of vaccine. Kindly review with reports.

Sir I am suffering frm jaundice since last 5 days. Bilburin is 3.sgpt around 1000. Is it a bad sign and I resumed job today but whn I went there I feel weakness and like I will fall down.

Hello, thanx for using Lybrate. SGPT of 1000, you need to be very careful. Take complete rest. It is better if you get admitted in a hospital. We need to monitor SGPT, shot levels every two days and also evaluate the cause for elevation of liver enzymes.

Hello, my friend was positive to HBsAg (quantitative) test 26920.11 IU/mL last september. And the LFT were also out of the normal range. Today he did a test HSbAg which came as NonReactive. As well as the LFT tests came normal. So my query is what are the tests he needs to do to find out the current status of the Hep B infection. And how to be sure that is he cured or the virus is still there? Thanks. Any suggesting will be highly appreciated.

Hello, There is chance of total cure in hepatitis B infection. To be sure whether he got cured or not kindly do HBVDNA level quantitative Anti Hbs antibody liver function test IgG anti Hbc antibody. Liver function test. These will help us to know more details.

Hello, My uncle suffered a ischemic stroke one month ago. Now, he is unable to walk by himself without support. He also has severe pain and swelling due to hemorrhoids and the general surgeon asked him to do an operation urgently. The issue here is that he he is taking Aspirin since the stroke. So, is it advisable to go for the surgery keeping in mind that the surgery requires stopping aspirin two days before the surgery and three days after, hence a total of 6 days without aspirin. Hello, thanks for using Lybrate. as patient is on aspirin and suffered ischemic stroke it is risky to interrupt aspirin therapy, but when there is another problem which is unbearable we need to do operation. Try to control it medically but if it is sever then we need to take risk and do surgery.

Hello, thanks for using lybrate. Kindly note that diabetes has become a common problem now a days. It disent affect the quality of your life if sugars are controlled strictly. Main problem with diabetes is if the sugars are not controlled correctly patients develop nephropathy, neuropathy, retinopathy, cardiac problems and erectile dysfunction. Kindly consult a general physician and take proper medicine and keep sugars under control and lead happy life.

Migraine: a common cause of Headache.

Migraine: a common cause of Headache.

Migraine is an episodic disorder consisting of severe headache with or without light and sound sensitivity. Migraine is second most common cause of headache.

Pathophysiology: Commonly occurs in 4 phases

  1. Premonitory symptoms
  2. Aura
  3. Headache
  4. Postdrome.

A phenomenon known as cortical spreading depression of leao is the most accepted theory for origin of migraine.

Common triggers for migraine headache are:

Emotional stress, hormones in women, not eating, weather, sleep disturbances, odors, neck pain, lights, alcohol, smoke, sleeping late, heat, food, exercise, sexual activity and obesity. Tyramine, odors and chocolate are unproven triggers of migraine.

Migraine headache are often worsened by rapid head motion, sneezing, straining, constant movement or physical exertion.

Migraine headache is characterized by Following features:

  • Unilateral pain nausea/vomiting
  • Throbbing pain photophobia and phonophobia
  • Aggravation by movement
  • Moderate or severe intensity

Migraine therapy is of two types.

  • Management of acute episode
  • Preventive therapy

Management of acute episode: 

Nsaid s like (naprosyn, ibuprofen or aspirin) to be taken the starting of headache.

If available triptans (sumatriptan, rizatriptan, almotriptan) are to be taken at starting of headache.

Other drugs less commonly used these days are ergot derivatives like dihydroergotamine, metoclopramide, prochlorperazine, chlorpromazine etc.

Combination therapy like triptan+ nasid commonly used treximet (sumatriptan+naprosyn) can be used.

Preventive therapy:

If number of episodes are more frequent and causing disturbance to daily living we will start on preventive therapy like

Betablockers, tricyclic anti-depressants and some anti epileptic drugs.

We need to maintain a headache dairy noting down number of episodes of severe headache and need to take naprosyn or almotan for improvement.

Inestigational agents: 

Lasmiditan and cgrp antagonists are being evaluated for treatment of migraine.

Hello, you may be suffering from piles or anal fissure which can be determined by examination and proctoscopy that is done by a general surgeon. Kindly consult him once. Then second is both the problems increase with hard stools and constipation. So keep your stools loose by taking more dietary fiber. Dietary fiber is found in fruits like mango, orange and vegetables.

Hello, not entirely but partly true. But bone pain due to deficiency of calcium and vit d occurs after 45 to 50 years of age, not at your age.

My stool color appears black since last 1 week. I read an article on web which says this can be a symptom of bleeding in intestine. Am really afraid what exactly is causing my stool color to be black please help!

Hello, I can understand your concern. Black color stools are seen in upper gastrointestinal bleeding but not only in that condition. If you are on iron supplementation your stool may be black color, if you have taken some fruits or vegetables also black color stools may be seen. Ugi bleeding is commonly seen in chronic liver disease patients. Kindly observe your stool for another two days, avoid spicy foods, eat vegetables if not improving kindly consult a physician. If you are taking iron supplement kindly ignore black stools. Hope this helps.
