COLONIC DIVERTICULAR DISEASE
Diverticular disease is a condition that is accompanied with 3 diverse conditions that contribute to the formation of small sacs or pockets that occur around the colon wall. These include diverticulosis, diverticular bleeding, and diverticulitis. The symptoms of Diverticular disease include acute constipation followed with very loose bowel movements. Cramps and stomach pain occurs along with fever and chills.
HOW IS COLONIC DIVERTICULAR DISEASE DIAGNOSED?
The initial assessment of patients with suspected acute diverticulitis is comprised of a thorough history and physical examination, including abdominal, rectal and pelvic examinations. Useful initial examinations may include a CBC, urinalysis, and flat and upright abdominal radiography. If the clinical picture is clear a gastroenterologist may not need any tests. When the diagnosis is in question, other tests such as CT, water soluble contrast enema, cystography, endoscopy and ultrasound may be performed
HOW IS COLONIC DIVERTICULAR DISEASE TREATED?
Medications such as antibiotics may be prescribed initially. Surgical treatment of the disease is evaluated based on the stage of the disease and symptoms. Emergent sigmoid colectomy is performed for patients with the following:
1. Diffuse peritonitis
2. Failure of conservative treatment
3. Persistent sepsis despite percutaneous drainage
4. Very low threshold, immunosuppressed and immunocompromised patients who are likely to fail medical treatment and present with perforation
