I am 18 years old boy. My one tooth is half broken and got black. What should I do.
If the tooth breaks, have the tooth contoured. Fill the crack in. Place a crown on your tooth. If the tooth has been badly damaged and the nerve or pulp is exposed, the dentist may have to perform a root canal to save the tooth. If the tooth has been severely damaged, it may have to be extracted.

I am losing hair in my forehead. I tried different kinds of shampoo. What should I do to reduce hairfall.
Dirty hair causes more hair fall, and shampooing hair daily reduces falling of hair. Fixed amounts of hair fall every day. If hair are washed after a long gap, then also those hair will fall. It is possible that most of the hair fall happens during the massage before shampooing and while shampooing you find less hair fall.

Madam, eventhough you may be better as of now, relapse of depression can occur. The medicines help maintain the neurotransmitter balance in the brain. If your history of depression is long, it is advisable to keep meeting your psychiatrist regularly and continue medicine as per instructions. Feeling right now and having a worse episode later may be catastrophic. Its just like immunisation. You can choose not to get vaccine of a certain illness by avoiding the injection but later the consequences are severe.

My sex time is less and my wife did get satisfied of me. Please help me. Dr. help me.

BHMS
Homeopath,
My sex time is less and my wife did get satisfied of me. Please help me. Dr. help me.
Dear lybrate user, if you indulge in sexual relation after an interval of minimum 7 days then you are bound to discharge quickly during intercourse but that does not imply that you have any disease. It occurs due to loss of control of the muscles of penis. Normally it is seen that when a person does sexual intercourse daily he gains control over the muscles of penis & can stay longer during sex. You need to take homoeopathic mother tincture salix nigra q, 30 drops, thrice daily, after meals, in a cup of water to get rid of premature ejaculation. But before taking any medicine kindly notice whether you are having a second erection within 15 minutes of ejaculation for the first time. If you are not experiencing any erection within 15 minutes of ejaculation then you are surely suffering from erectile dysfunction. There will be a change of medication in this case. To get rid of erectile dysfunction you need to take homoeopathic mother tincture yohimbinum q, 30 drops, thrice daily, after meals, in a cup of water. Along with medicines you can also try a home therapy to boost your sexual performance. Soak 4-5 dry dates (khurma khajur) in a cup of lukewarm milk before going to bed daily. On waking up next morning eat those soaked dry dates & discard the milk. Repeat the process daily. This therapy often gives effective result within 15 days there are many conventional allopathic sexual pills in market such as adegra, viagra, penegra etc. Refrain yourself from using such pills as those pills contain a steroid called sildenafil. This steroid has a unique temporary effect on erectile problems. Patients using these pills for first time often report that their penis remain strong even after ejaculation. Such effects leads the patients to use sildenafil for prolonged course of time as they often feel that a full erection is possible only after taking sildenafil. Moreover it is very low cost & easy availability of this steroid has made it quite common as sexual medicine. But the bitter truth is prolonged use of sildenafil will permanently disable the normal physiological process behind erection & will render your body to become solely dependent upon sildenafil for erection. As a result you won't feel any slightest normal erection without taking sildenafil. This is an irreversible condition & it cannot be treated by any means. Once you attain this condition you become a life-time customer of that steroid sildenafil because you have to take sildenafil in order to satisfy your partner. As prevention is always better than cure I would advise you to check the composition of any sexual pills before buying. Discard any sexual pills which contains sildenafil in its composition. If you are suffering from premature ejaculation then along with medicine & home therapy you can try the kegel's exercise. You have to do this exercise once daily during the first time of urination after waking up from sleep at morning. Kindly note the following steps to perform the kegel's exercise:- 1) start urinating until you feel that you have voided almost half amount of urine from bladder. 2) when you feel you have voided almost half of the urine try to stop the flow of urine by squeezing the internal muscles of penis. 3) you should not use your hands to stop the flow. Your hands should be resting on your waist. You have to stop the urine only by your self control over the internal muscles of penis. 4) try to stop the urine flow by the control of internal muscles up to 20 seconds. On the very first day you won't be able to hold the urine for more than 3 seconds. However, practicing the exercise daily will improve your control over the muscles of penis & after 15-20 days you will be able to stop the urine for 20 seconds by the control of your pelvic muscles. This control of pelvic muscles will help you to get rid of premature ejaculation while you are performing sexual intercourse 5) after 20 seconds is over, void the rest of the urine. Do not stop the flow of urine for more than 30 seconds. N. P.- kegel's exercise is effective only in the case of premature ejaculation & it is not helpful for patients suffering from erectile dysfunction.

How Diabetes Can Affect Vision

MBBS, Diploma in Diabetology, DDM, CCACCD

How Diabetes Can Affect Vision

Diabetes is the condition where the blood glucose levels in your body tend to be quite high and can have an adverse effect on many of the organs in your body. The eyes are no exception and can be quite adversely affected by diabetes as well.

Let's look at the various problems you could face if you have diabetes:

1. Blurry vision: Diabetes can cause the lens in the eye to swell and this will affect the way you see. Because of the increased lens size, the eyes have difficulty in focusing on objects resulting in blurry vision. You will have to get your blood sugar levels back to normal and only then the vision will begin to correct itself. This will however take time to happen.

2. Cataract: Blurry vision for an extended period of time which progressively gets worse can be a symptom of cataract. Although cataracts can develop even with normal patients, they tend to accelerate and happen earlier in adults who have diabetes. Cataracts are usually fixed with surgery where the natural lens is removed and replaced with an artificial lens.

3. Diabetic retinopathy: Diabetic Retinopathy and Retinal Detachment are one of the leading causes of blindness in adults who suffer from diabetes. The retina is a very important part of the eye which allows us to see the images by capturing light and then sending them to the brain via the optical nerve. With diabetic retinopathy the smaller blood vessels in your retina may get damaged and thus end up causing damage to your vision. This can be of three types:

  1. Proliferative Retinopathy: In this condition very small blood vessels grow from the surface of the retina. The retina is the film at the back of your eye , and the tiny blood vessels are capillaries. These growing blood vessels are very delicate and bleed easily. If you have had diabetes for years your retinae may develop this condition. As the retina is damaged by diabetes, the diseased retina releases special growth chemicals. These chemicals make tiny blood vessels grow: these are called 'new blood vessels.
  2. Background Retinopathy: Background or nonproliferative diabetic retinopathy (NPDR) is the earliest stage of diabetic retinopathy. With this condition, damaged blood vessels in the retina begin to leak extra fluid and small amounts of blood into the eye.
  3. Maculopathy: The macula is the central area of your retina. It is responsible for all your sharp vision, such as used for watching TV or reading. It can become damaged in diabetes, with leaks developing (oedema).

4. Glaucoma: This is a condition where fluids build up inside the eye and it results in the pressure within it building up. This may damage the blood vessels within the eye and cause vision changes. Problems within the eye may not be detected till you experience vision loss. Some of the symptoms of glaucoma may include:

  1. Blurry vision
  2. Watering from Eyes
  3. Difficulty in vision
  4. Pain in the eyes
  5. Lights appear to have halos
  6. Headaches

Treatments: Blurry vision also tends to go away slowly once the level of blood glucose is controlled either via medicines or by diet changes. However, glaucoma or diabetic retinopathy may require a range of medicines to ease the pressure on the eye or to discharge fluid buildup. If none of these works, then relevant surgery may be required to resolve the problems.

Variations in ECG can b due to various reasons. Many carfiovascular diaeases as well as noncardiac abnormality give rise to these variations. Heart attack has some specific changes in ECG in 60 to 70 percent of patients. Normal cholesterol level is just crude marker for lipids in body but it does not give always idea abt the visceral or vascular cholesterol deposits called as plaque burden. Pt having heart attack can have normal cholesterol levels. If chest pain with ECG variation occurs you hv to see cardiologist to get confirmation. There are noninvasive test like cardiac enzymea ECHO and stress test to give confirmation for a heart attack. Dnt worry. Just get evaluated and rule out cardiac issues.

Hypertension and Diabetes - How They are Related?

MBBS, DNB (General Medicine)
General Physician, Delhi
Hypertension and Diabetes - How They are Related?

Hypertension or High Blood Pressure can trigger as well as worsen complications related to diabetes. Most people suffering from diabetes tend to develop hypertension later in their lives. Diabetes tends to increase one’s blood pressure, making him/her even more vulnerable to other heart diseases and cardio-vascular disorders. Diabetes often leads to a condition known as atherosclerosis, which is the hardening of the arteries. A combination of diabetes and hypertension drastically increases the risk of suffering from a heart disease. The risks of kidney diseases and retinopathy (damage to the tiny blood vessels present in the retina) also go up, resulting in blindness. With age, chronic hypertension can also result in numerous cognitive disorders such as dementia or Alzheimer’s disease (degenerative mental disorder).

How to prevent Diabetes and Hypertension?
1. Walking for about 40 minutes each day or engaging in any type of aerobic exercise can better one’s heart health. Chalk out an exercise plan upon consulting the doctor
2. Limit drinking and abstain from smoking
3. Go for regular kidney check-ups
4. Eat food rich in fiber such as vegetables, fruits, breads or whole grain cereals
5. Examine blood pressure as well as blood sugar levels regularly at home
6. Practice relaxation techniques such as meditation or yoga to cut down on stress level; as stress is one of the major contributors to diabetes and the subsequent hypertension
7. Shed off your extra pounds as blood pressure escalates with weight

To gain weight you should eat more calories than you burn. Plus not only you have to eat calorie dense food but you have to keep in mind that what you are eating is healthy too. In the process of weight gain you do not want to put weight in an uneven manner and look unhealthy rather than looking bright and healthy. 1. Start with changing your lifestly. Start working out if you are not active. The biggest myth that prevails now a days is that thin people do not need to work out. This is wrong. When you challenge your body, it is forced to grow. But this growth can only be possible if you are eating enough food to let the body absorb and grow in size and muscles. 2. Eat Healthy – Stop drinking and smoking too much if you are indulged in it. Limit your intake of junk food, pepsi, sugary stuff etc. Do not at all starve yourself. Make it a habit to eat something healthy every 3 hours. This means you have to eat 6 small meals in a day. Your Daily food intake should ideally consist of – a. Have plenty of nuts, dry fuits and seeds. Almonds, walnuts, sunflower seeds, pumpkin seeds, figs, dates and raisins. They are full of healthy fats needed by a human body. b. Good amount of fruits and vegetables – Bananas, apples, pomegranate, green leafy vegetables. Fruits and vegetables helps in absorption of food in the body and together, completes your daily nutrition need. c. If you are a non- vegetarian - Eat eggs, chicken etc. d. Eat whole grains – Try to have multi grains spread throughout the day. Wheat, brown rice, oats, bajra, daliya etc should be taken in multiple meals in a day. e. Eat Sprouts – Chana and moong which are full of protein and nutrients. f. You can supplement your diet with good quality multi-vitamin capsules. If you are lacking in protein, you can buy a high quality whey protein and have it with banana shake twice a day. g. You can take natural food items like wheat grass which is full of essential nutrients which helps in weight gain. Try to be patient while you are on the process of weight gain. Results take time to show. Just follow the regimen of weight gain religiously and you will soon start seeing the results.
