All About Esophagectomy

The surgical procedure to remove the oesophagus (the tube between your stomach and mouth), partially or completely, and reconstruct it by using a section of the large intestine or stomach is called esophagectomy.

Esophagectomy is commonly used to treat oesophageal cancer.

Uses:

Early-stage oesophageal cancer is frequently treated with an esophagectomy. Other than that, esophagectomy is also performed to treat oesophageal dysplasia (a condition in which cells in the oesophageal lining are precancerous or likely to develop cancer if not promptly treated).

Esophagectomy is commonly performed when cancer has spread to the stomach, lymph nodes or associated organs.

Some other conditions which require esophagectomy include:

  1. Oesophageal trauma.

  2. Swallowing of cell-damaging, or caustic agents such as lye.

  3. Problematic stomach disorders that make the passage of food to the stomach difficult.

  4. A previously performed esophagectomy was unsuccessful.

Procedures:

There are three ways a surgeon may perform an esophagectomy, which include:

  • Transthoracic Esophagectomy (TTE)

In this type of procedure, the incision is made on the chest. A TTE is mostly used to treat the following conditions:

    • Cancer present only in two-thirds of the oesophagus

    • Barrett’s oesophagus (Abnormal cell transformation in the lower oesophagus)

    • Damage to the oesophagus by swallowing a caustic agent

    • Reflux esophagitis (stomach acids return to the oesophagus) complications

  • Transhiatal Esophagectomy (THE)

In THE, the incision is made from the end of the breastbone to the bellybutton. THE is performed for the following conditions:

    • To remove the cancerous oesophagus

    • To tighten or narrow the oesophagus in order to make swallowing easier

    • To fix issues in the nervous system

    • To repair frequent gastroesophageal reflux

    • Correct a hole in the oesophagus caused by any caustic agent

  • En Bloc Esophagectomy

In this procedure, the oesophagus, part of the stomach and all the lymph nodes in the abdomen and chest are removed. The incisions are made in the abdomen, chest and neck; the stomach will be reshaped and brought up to the chest to put back the oesophagus.

Potentially curable tumours are treated by en bloc esophagectomy.

Outlook:

Recovery time of esophagectomy is three weeks approximately. You are allowed to go back to your regular diet after a month. during D & C procedure there is hormonal imbalance which cause delayed menses.it take time around 6-7 weeks for appear menses again.you should wait for appear menses again,keep patience.

7th day of my period that is 20 april. We made out. Without protection, he didn't ejaculate inside me though. I don't basically have any symptoms now. But in case if I'm pregnant how many days after making will I know I'm pregnant? And till how many months is medical abortion applicable?

Hello, the earliest you can confirm pregnancy is after 7 days post last intercourse with a blood serum beta hcg test, which if positive shall confirm pregnancy, though on 7th day of menses you were in your safe period, and pregnancy seems unlikely. Medical abortion can be safely undertaken till 7 weeks/49 days.

A-around 15% of pregnancies gets miscarried, that is nature's rule. You should avoid next pregnancy for 6 months from the last abortion. Chances of repeat abortion or pretem delivery increases if one becomes pregnant within 6 months of miscarriage. Also if abortion/ miscarriage was caused by any viral infection, then it will get cured automatically within 3 months time. Use contraception for 6 months. Best one to use are oral contraceptive pills. No special treatment or tests are needed if you had only one miscarriage or spontaneous abortion.

Pain During Menstruation

Painful menstruation or menstrual cramp is termed as Dysmenorrhoea and refers to the discomfort or pain associated with menstruation. In 80% cases, cramps are an integral part of the preliminary dysmenorrhoea syndrome. Though this symptom is not deemed as a very serious clinical problem, the pain and its associated symptoms are acute enough to keep an affected woman from normal functioning for the first one or two days. These symptoms may take over one or two days prior to menses, reach their pinnacle on the first day and subside slowly with the passage of time. The pain can be typically depicted as being ached, cramping of the stomach and often reaching to the lower back portion.

There is probably no woman across the world, who has not undergone the painful menstrual cramps. But don't fret! Homeopathy treatment has a range of remedies to relieve you off this unease.

  1. Cimicifuga: Normally menstruation cramps indicate a pain that flows across the pelvic region from one side to the other. This remedy will particularly be helpful in neuralgic and rheumatic cases as well as congestive cases where it works effectively with Veratrum Viride and Belladonna. It is also very efficient in combating headaches preceding menses, acute pains across abdomen, labor like pains and scanty flow.
  2. Caulophyllum: This remedy is great for treating spasms and pain that flow downwards. The spasmodic periodic pains that need to be treated with Caulophyllum are mostly concentrated around the groin and radiate to other surrounding parts. Gelsemium can also be taken for treating neuralgic as well as congestive dysmenorrhoea when the pain bears down. The pains are often labor-like and spasmodic with flow of pale urine. In order to get faster relief, it can be taken along with hot bag press.
  3. Belladonna: If you are afflicted with the congestive forms of menstrual cramps, then you may take Belladonna. In most cases, a pain and discomfort is felt prior to the flow and a feeling of something heavy around the vulva can be relieved if you sit up straight. This pain occurs and subsides all of a sudden making the flow clotted and restricted. This kind of dysmenorrhoea is extremely painful and makes the genital dry. In case the pain gets severe and cuts through the pelvis, then Veratrum viride can also be administered.
  4. Zanthoxylum: This remedy is very effective in fighting dysmenorrhoea and uterine spasms. It is very effective when the pain is burning, agonising and extending down through the thighs making the limbs paralysed and numb. You can take this measure when the pain is affecting your ovaries as this medicine works closely for neuralgic form of menstrual cramps. Women with delicate nervous temperament can also take this remedy as it relieves from headache and eye pain caused from Menstruation.

Homeopathy has a huge range of medicines and it becomes difficult for a commoner to select an ideal one. Therefore, when you are deeply affected with menstrual cramps, you can always consult an expert before administering any medicine.

Hello doc, My clitoris is getting swell every time in periods. My doc says its cuz of fungal infection. This has happened two times that my clitoris is getting swelled. Is there something I need to worry about.

Over the years my period days have reduced from 5 days to 1.5 days. I got married 2 years back. Not wanting to get pregnant started taking pills in consultation with gynac. Hi You can take tablet's to control these symptoms after consulting gynaecologist and showing all your reports. You can also eat something before getting up from bed early morning to reduce these symptoms.

Could Your Pelvic Pain be Endometriosis?

Endometriosis is a painful condition where the endometrium tissue lining the walls of the uterus, grows as implants outside the womb of the patient. This can lead to a number of painful conditions and is often considered as a common cause behind severe pelvic pain, as far as women are concerned. Here is all you need to know about Endometriosis and pelvic pain.

  1. Painful Menstruation: Depending on the stage of the condition, the implants could remain on the surface or go deeper into the ovaries which may cause acute and severe problem during ovulation as well as the menstrual cycles. This is one of the symptoms that one must not ignore. If you are having painful cramps that affect the pelvic area and the abdomen, and if these cramps last throughout the duration of the cycle, then there are strong chances that you are experiencing the pain that comes with Endometriosis. 
  2. Pain During and After Intercourse: While there are many reasons why women may face pain during intercourse, it may be noted that vaginal and pelvic pain that come during and after intercourse could point towards the presence of Endometriosis, which is causing irritation and discomfort due to the implants. 
  3. Bowel Movements and Urination: Usually, pelvic pain may trickle down to and also get activated by the bowels when there is a motion as well as urination. This is a common problem that can cause pain, which spreads throughout the region if the patient is suffering from Endometriosis. 
  4. Bloating and Other Symptoms: When the abdomen and nearby regions face bloating due to the implants and their painful spread, the pelvic region as a whole bears the brunt in terms of severe pelvic pain. The bloating and other symptoms like loose motions and constipation can also create pain in the abdomen and pelvic region on a persistence basis. Sharp pains may also shoot up and down the lower back due to such symptoms which should not be ignored if they do not abate within a few days. 
  5. Misdiagnosis: Many times, the bloating and pelvic pain may be linked with pelvic inflammatory disease, which causes pain in the muscles and joints. But if you are having pain in the abdomen as well as problems during your menstrual cycle, then it may be helpful to have the tests for Endometriosis done as well. 

In order to diagnose the condition, the doctor must ensure that proper imaging tests like MRI and CT scans as well as an ultrasound with lab tests based on blood samples have been conducted.
