Contraception - Myths and Misconceptions

MBBS (Gold Medalist, Hons), MS (Obst and Gynae- Gold Medalist), DNB (Obst and Gynae), Fellow- Reproductive Endocrinology and Infertility (ACOG, USA), FIAOG
Gynaecologist, Kolkata
Today 26th september is the world contraceptive day. Sex is a basic human right. But to have sex safely and without fear of unwanted pregnancy should be the priority. Many women across the world die everyday as a result of unwanted pregnancy, mostly due to unsafe abortion.

So, couples can avoid pregnancy and enjoy sex life fully, only if they can follow safe and effective contraceptive advice. Let me clarify some popular myths and misconceptions abut contraception.

1. one contraception is useful for all

While the ideal contraceptive is necessary but it does not exist. So, what may seem suitable for a couple may not be useful for another couple. We need to assess various points like number of children they have, wish to have further children and plan to have children, general health of wife and husband (and any diseases they suffer from), frequency of intercourse, chance of infection, age of the partners etc.

2. Regarding barrier methods
Most popular barrier method is male condom. But the myths are that" condoms reduce sexual pleasure" and" condoms get easily damaged leading to failure.

Condoms are specially useful for those who are at risk of infections, who do not have very frequent intercourse and who are not suitable for other methods. Condoms do not decrease sexual pleasure, some of the condoms have medicated products which can increase the duration of intercourse and also act as lubricant to facilitate intimacy.

But men should know how to use it properly. It should be worn in erect penis expelling all the air and projecting the tip beyond the tip of penis. While withdrawing penis after intercourse, the condom should be held at the base of the penis. Actually failure/ rupture of condoms occur because of mishandling of condom or not knowing proper use of it. So, before use, please make sure that you know its correct use.

Definitely some men can feel latex allergy and they have to use non-latex condom, which are widely available.

There are barrier methods for females also- like female condom, diaphragm and cervical cap etc. But they are not very ease to use and are not so popular.

3. Oral contraceptive pills (ocp)

Myths-" ocp causes increase in weight" ocp causes cancers.

Facts- ocp seldom causes gain in weight. The gain in weight is coincidental, that means you have some other reason of gaining weight- like not having exercise, ignoring diet control and some other diseases.

Ocp rather reduces risk of cancer- especially ovarian cancer (which is very difficult to diagnose and very lethal), cancer of colon, cancer of endometrium (inner lining of uterus). Of course, there is little risk of breast cancer, but the risk is not very high, it can be easily detected by examination by doctor. So, if you take ocp, get rid of all the fears. Just try to have self- breast examination every month.

4. Intrauterine contraceptive devices (iucd)-

Most popular is copper t. Medicated iucds are also available, that contains hormonal drugs.

Copper t does not increase risk of infection, a common misconception lies among woman.

5. Emergency contraceptive pills (i pill etc).

Often couple uses these pills regularly after intercourse. Remember these pills contain very high content of hormonal drugs, which can lead to serious side effects like hormonal imbalance, irregularities in periods (and thus can prevent you from getting pregnant in future, even when you want pregnancy), ectopic pregnancy (pregnancy outside tubes that are life-threatening) etc. So, use regular contraception if you want to prevent pregnancy. I pill is not meant to be taken for every time of sex.

6. Ligation-

Ligation of male (vasectomy) and female (tubectomy) is done as permanent contraception for couples having more than 2 children. Ligation in male does not disturb sexual activity. It is easier than female ligation. Again, female ligation is also a very safe technique.

So, please remove all misconceptions and myths about contraception. Have safe sex, enjoy life.

Benefits and Risks of Ashwagandha

Benefits and Risks of Ashwagandha

The amazingly complex herb, ashwagandha, happens to derive its name from the two sanskrit words, "ashwa" and "gandha".It is an exquisite herb, which can significantly cater to your body in more than one way due to its numerous health benefits.

Here are some of the most common advantages and risks of consuming this herb.

The Benefits

  1. Enhances your brain: Ashwagandha is an antioxidant and helps keeping free radicals at sea, and prevents them from damaging your brain cells.
  2. Fights emotional imbalances: Sudden emotional imbalances and even more sudden mood swings are probably the biggest and the most imminent problems of the youth today. Ashwagandha cuts out the stress stimuli and has a soothing effect on you, thereby making sure your emotional imbalances do not come in the way of your well being. Ashwagadandha can naturally lower cortisol levels.
  3. Cures wounds: Ashwagandha is also believed to cure your physical wounds, and a controlled amount of this herb, is believed to save you from one-sided paralysis of your body.
  4. Is anti- cancerous: Ashwagandha, according to a few studies, acts as an anticancer agent because it helps in slowing down the growth of cancer cells and cancerous tumors.
  5. Treats insomnia: Ashwagandha has its rejuvenating properties as well, which help in calming your nerves down, relieves you of your stress. It is considered to be an effective tool in treating patients with insomnia.
  6. Helps in weight loss: Cortisol not only acts as a stress hormone in the body, it can also make you fat. Ashwagandha is known to naturally lower cortisol - the stress hormone.
  7. Lowers Blood sugar levels: Ashwagandha also potentially decreases fasting blood sugar levels, which means that it may also help to improve glucose metabolism.

But, like every other herb, ashwagandha also has some risk factors associated with it.

The Risks

  1. Pregnancy issues: According to a few studies, this herb can have an adverse effect on you if you are a pregnant woman, putting you at the risks of stillbirth. Though there hasn't been much research to back up this fact, it still remains one of the largest concerns during pregnancy.
  2. Hyperthyroidism: Certain studies claim, that ashwagandha can majorly contribute to hyperthyroidism, although even this, hasn't been proven. So, if you happen to be patient of hyperthyroidism, you should consult a physician before consuming the herb.

Natural or Cesarean Delivery: Which One Should You Prefer?

Natural or Cesarean Delivery: Which One Should You Prefer?

Child deliveries during pregnancies are of two types. If you are pregnant or have someone in your family who is then you can either go for caesarean section (or C-section) delivery or normal delivery, which is also known as vaginal delivery. In some case C-section delivery is chosen by your doctor as it may be a safer option in case you have some health complications. Diabetes, high blood pressure or herpes can cause complication that may in advance let you know whether the delivery procedure would be C-section or the normal one. Here are the pro and cons of both the procedure.

Natural Delivery: Benefits and Disadvantages

  1. Shorter stay in the hospital and time for recovery can help you in case of normal delivery.
  2. It may be physically painful but at the end of the day as it is a natural delivery method and you will experience less postpartum pain.
  3. There would be lesser chances of respiratory problems in your baby.
  4. Normal delivery may cause fear and anxiety which may lead to huge complications. Even your baby may face the risk of being deprived of oxygen from cord compression.
  5. It is also more painful and unpredictable.
  6. There may be increased chances of pelvic organ prolapses in normal section deliveries.

C-section: Benefits and Disadvantages

  1. You will be well in advance told about the delivery date by your doctor and hence planning is made easy. 2. If you have any infection like herpes chances of your baby being is reduced by c section delivery.
  2. There will be lesser risk of pelvic floor injury and few cases of urinary continence.
  3. There will be no risk of birth related trauma like bruises and swelling.
  4. There are surgical risks of anesthesia which can cause severe adverse side effects like headache, nausea and vomiting.
  5. There are also chances of heart attacks and blood clotting in veins. You may also have chances of postpartum infection in c section delivery.
  6. Numbness around the area of operation and bleeding may require a hysterectomy which is a surgery to remove the uterus.

There are many other pro and cons of the two methods of delivery and you should always consult your doctor and follow proper medications for better delivery of the newborn.

Ayurvedic Pregnancy Strategies

There are 10 classic strategies to be used during pregnancy that can support the health and well-being of mom and baby, and keep the prana and apana vata in balance.

1. Favor Your Cravings. The most important advice is to eat what you naturally desire. Follow a well-balanced diet, with meals including adequate amounts of protein, healthy starch and veggies. Remember, you are eating for two body types, so you should not follow a strict diet for one body type. Honor your unique cravings in moderation, especially after the fourth month when the cravings you experience are more likely those of the baby.

2. Balance Vata. The major theme during pregnancy is to balance vata, the energy of the nervous system. This is best done with a diet of fresh, plant-based, whole or sprouted grains and non-processed foods. Warm, freshly cooked foods with healthy oils of olive, ghee and coconut are good. Avoid eating leftovers as much as possible.

3. Foods to Avoid:

Hot spicy foods
Uncooked leafy greens – as they can cause gas.
Undercooked beans and lentils – which can also cause gas.
Artificial flavors, preservatives and chemical additives
4. Enjoy These Three Tastes. The three tastes that balance vata are sweet, sour and salty. During pregnancy the sweet taste should be most emphasized, because it is the most sattvic or wholesome for the baby. This includes sprouted breads or those cooked without oils, whole grains, rice, fruits and healthy starches like sweet potatoes, quinoa, cooked beets and carrots. These foods are emphasized on the Winter Grocery List.

5. The Right Milk Does a Baby Good. Drink warm, vat-pasteurized (heated under 135 degrees F), non-homogenized milk with ghee made from grass-fed cows. One cup of warm milk with a teaspoon of ghee twice a day is said to boost immunity and the complexion, or “ojas,” of the baby.

6. Daily Massage with an Ayurvedic Herbalized Massage Oil. These oils feed the microbiology on the skin, as well as calm the skin-based sensory nervous system of the mom. The calmer the mom, the calmer the baby. This can be self-massage, or an opportunity to get massaged by a loving partner. Gently massage the abdomen and, in the 8th and 9th months, spend extra time on the nipples to prepare for nursing.

7. Five Tips to Conquer Morning Sickness:

1. Roast cardamom seeds, powder them and eat a small pinch through the day.

2. Snack on dry crackers or toast. It helps to always keep a little food in the stomach.

3. Temporarily eat from the Spring Grocery List or Kapha-Reducing Diet.

4. Sip tea made of 1/4 tsp. ginger powder and/or fennel seeds and hot water.

5. Try sleeping in a semi-reclined position by putting a pillow between the box spring and the mattress.

8. Get Gentle Daily Exercise. A great way to get this is to walk for 30 minutes each day.

9. Rest Up. In the 8th month, you should get as much rest as possible. This is a delicate time when the subtle nutrient fluid called “ojas” that supports vitality, complexion and immunity is passed between the mother and baby.

10. The MOST important pregnancy principle is to remember to be happy. The partner’s job during this time is to keep the mama-to-be happy. Of course, the mom must practice daily acts of happiness as well. The partner should avoid travel, be home in the evenings and fulfill every desire of the mother-to-be.

Sometimes, we don't find any obvious cause for infertility. Its called unexplained infertility. Have you undergone diagnostic laparoscopy and tubal patency test. If yes then you can go for ovulation induction / iui as your age is just 25 yrs, you have ample of time. You can also just relax and wait for spontaneous pregnancy. If you are unable to conceive by any of these methods for another 3-4 years then go for ivf icsi.

Hello, if you are not willing to take pills then just wait for your menses to arrive. If they arrive then you are not pregnant. Anyhow chances of pregnancy during each cycle is just 35-40 % so you may wait as you don't want to take any ecp as well.

How do you deal with a one year old child who doesn't want to eat anything other than the mothers milk?

BHMS
Homeopath,
How do you deal with a one year old child who doesn't want to eat anything other than the mothers milk?
With patience!! I can understand that it is stressful but one has to wean gradually.you have to start by gradually allowing the baby to taste and try all the different types of foods available.first try with bland potatoes sweet potatoes, boiled carrots and peas.then you can spice up bit gradually.if he is not eating try to blend everything and then offer in a liquid or pasty manner.and try to feed him after he has played a while or when he has just woke up from his sleep.the times when they are actually hungry. don't offer food after the baby has been fed with breast milk. It is a very gradual process. It will be alarming only if the child starts showing signs of malnutrition, until then you just keep trying.

How can we do safe sex as we don't know that our partner is affected from STD like HIV or many sexually transmitted diseases? Which things should have to follow ?Please give complete guidance ?And which things should have to use by both male and female partner?

CCEBDM, PG Diploma In Clinical cardiology, MBBS
Cardiologist, Ghaziabad
1. No sex with unknown person 2 .no sex with multiple partner , be faithful to one partner 3.Get tested and know your partner’s HIV status. 4. Have less risky sex. Oral sex is much less risky than anal or vaginal sex. Anal sex is the most risky type of sex for HIV transmission. 5.Use condoms correctly every time and lastly do not get in to all these sex stunts . Get married and lead a good quality life rather than finding ways to have satisfy your sex desire here and there. Good luck.
