I'm 30 years old. I have vertigo. I feel very giddy tired and have a neck ache. What should I do?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned pain in neck region as the main problem. You are actually very young to have persistent pain. If there is only localized pain in neck region with no other associated symptoms like radiating pain, numbness, paresthesias etc, and no systemic symptoms like fever, loss of appetite, loss of weight etc, my advice to you for initial 2-3 weeks will be: - to maintain proper posture of your neck while working and sleeping and observing principles of neck ergonomics - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or even tab paracetamol which suits you - intensive physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied 3-4 times in a day - analgesic spray for local application like dynapar qps spray can be used and is easily available. If there is no improvement in 2-3 weeks with this conservative management, we will need to evaluate you further in form of dynamic x rays to look for any instability as a cause for pain and some blood investigations to rule out any deficiencies etc. You can also have vertigo due to neck issues and other causes as well, you can take tab vertin or stugeron on sos basis if you unbearable vertigo but in that case we ill have to get mri cervical spine done in your case. Avoid bending in front. Postural correction- sit tall, walk tall. Apply hot fomentation twice daily. Extension exercises x 15 times x twice daily. Bhujang asana. Core strengthening exercises. Back stretching. Do the cat/cow stretch.

My muscles are soft throughout the body. I can touch my bone in arms easily. I do exercises with intervals. My chest muscles also soft even after exercising. But when I tight my muscles they are hard. In other times they are so soft. How can I get rid of those soft muscles and gain hard muscles.

This is nothing but fat layer on muscles, which is soft. When you exercise muscles are activated and they get harder and bigger. On rest they go back to normal size. You need to reduce body fat % by avoiding all packed, processed & fried food to reduce fats.

If you have low back pain and sciatica you can undergo intermittent pelvic traction where in your half of your body weight will be added in the electronic traction so that that would help you to reduce the pain, along with Interferential therapy stimulations which would help you to get relieved from the radiating pain. Apply ice for the pain to get subsided immediately. If you cannot go immediately for the physiotherapy treatment. Ice can be kept in the low back where you have back pain. It is suggestible to wear Lumbo sacral belt which would help you to build the abdominal muscle tone which itself would make you feel better. You can also wear MCR chappals which would help you to have less weight falling in the painful back.

Backacke can be due to numerous reasons like back strain, big tummy, slip disc, lumber spondylosis. For best treatment please send an x ray image & consult privately. Till then you can do following home remedies: 1) mix camphor in coconut oil and boil it. Massage it followed by hot fomentation. 2) have turmeric & honey milk. 3) put a hot-water bag on back supported by a cushion. 4) add ginger while brewing your tea 5) if you are overweight, go for weight loss program 6) stay away from cold, go for hot water baths.

It may be due to a fracture or a ligament injury. First thing to do is to apply lots of ice. Get an x-ray done. Further treatment is decided based on the x-ray and your symptoms.
