Mr Inamullah, Your blood sugar level is very high, this means your diabetes is totally uncontrolled. What is your HbA1c%, it should be <7%. Diabetes is a disease that affects microvasculature (small vessels) and causes kidney problems and also erectile dysfunction. If you do not control diabetes effectively, by maintaining fasting sugar level closer tp 100mg and PP between 150 to 160 mg, HbA1c% <7, then ED is unlikely to improve. So please make effort to control blood sugar levels effectively.

Hello doctor. I am 43 years male. Weight 98 kg. Having diabetic family history. And tea totaler last week I take health package and found HbA1C 8.5 and AVG 196. SGOT 65 SGPT 76. My doctor prescribe me Metformin 500sr twice daily and Vitamin D3 weekly. Now my FBS is 126 and RBS 115. Can there any alternative for Metformin or I continue the same.

Mr. lybrate-user, Thanks for the query. With a HbA1c% value of 8.5, it is a definite indication of having diabetes. Since you have confirmed diabetes, it has to be treated. I am not very sure whether only metformin is enough to control the blood glucose levels. Discontinuing is out of question. Your weight of 98 kgs (if height is 5 ft 1 in) then the BMI is 40.83 kgs/M2 is the main reason for precipitating diabetes. You need to reduce the weight for better control of blood glucose. Thanks.

My father has got more uric acid. So he use to get severe joint pain often. He is taking tab. Zurig 80 since one month.And doctor asked to continue this tab. For lifetime.Kindly assist whether that is fine.

A high uric acid level can be caused when your body either produces too much uric acid or your kidneys don't eliminate uric acid rapidly enough. A high uric acid level may cause increasingly frequent attacks of gout, or it may never cause problems. A high uric acid level may also cause some people to develop kidney stones or kidney failure. And some people with a high uric acid level also develop high blood pressure, heart disease or chronic kidney disease, but it's often unclear whether this is a direct cause or merely an early warning sign of these conditions. Factors that may cause a high uric acid level in your blood include: Diuretic medications (water pills),Drinking too much alcohol,Genetics (inherited tendencies),hypothyroidism (underactive thyroid),Immune-suppressing drugs,Niacin, or vitamin B-3, obesity,psoriasis, Purine-rich diet — organ meat, game meat, anchovies, herring, gravy, dried beans, driedpeas, mushrooms and other foods Renal insufficiency — inability of the kidneys to filter waste Tumor lysis syndrome — a rapid release of cells into the blood caused by certain cancers or by chemotherapy for those cancers,Also, you may be monitored for high uric acid levels when undergoing chemotherapy or radiation treatment for cancer. Non-veg., take whole grain,oats,green veg.,plenty of water in day time, 1 glass of water at night before bed. Take tab. Zyloric 100 mg once a day. Tab. Ulracet sos for pain in joints.

What's the treatment for diabetic neuropathy in the lower limbs. Is there a way to reverse diabetic neuropathy.
Mr Amod, Diabetic neuropathy occurring in lower limbs can not be reversed, as damage to the nerve endings due hyperglycaemia is permanent. But further deterioration can be prevented by strict blood sugar control, and symptoms of neuropathy can be relieved by using certain drugs. It will be better if you give details like current fasting and PP sugar levels, HbA1c% and drugs being taken with dosage and come in a private conversation then i can give specific suggestions.

Diabetes can be controlled with medications and lifestyle management but cannot be cured. However recently developed bariatric surgery can help it. If he is fit for operation, do it. See a surgeon gastro-enterologist.

Hi lybrate-user delayed period could be because of hormonal imbalance that is due to deranged thyroid levels. Please get a urine pregnancy test to rule out pregnancy. Mostly it would be negative only as you used double protection.

I am diabetic type 2 male person aged 73 years. I am on oral medication. My everyday dose is as follows. Morning empty stomach 1 tablet Glyclazide 80mg plus i tablet 500 mg merformine. Evening before dinner 2 tablet 80 mg Glyclazide plus 1 tablet 500 mg Metformin. With this my sugar remains under control. Last testing done 2 months back indicate HbA1c as 7.1 and fasting sugar level 112 and post meals as 143. I have one more problem the waistline is constantly going up. It measures 40 inches on date. My height is 5feet 5.5 inches and weight 77 Kgs. I have no extra flesh except at belly. My BP level is also well under control. Earlier my HbA1c was 6.9. I donot know why the belly fat is going up with my best care. Some people tell me it is because of Metformin which I am taking. Please advise what care i should take to reduce the belly fat. Further what care I can take to reduce my HbA1c reading. My creatine level of kidney is also at 1.3 the last limit.

I am pretty sure the people who tell you that your belly fat is because of metformin are either not doctors or self declared degree less doctors. Metformin is the basic medicine which helps to decrease the belly fat and BMI of a diabetic its a wonderful drug and you are taking minimal amount only which is moderate dose. Gliclazide may increase your belly fat as it is insulin secreting drugs and insulin is a anabolic hormone and puts weight on you. i suggest change your medication. You can easily increase metformin to a maximum of 2 grams in divided dose. For further consultation kindly come in private question section.

Her thyroid level should be under control to feel well being and active. She might be taking medicine for thyroid, so get check her tsh level in every 6 month. She should take nutritious diet and a glass of milk daily. Give her homeopathic medicine to cure her trouble permanently. Yes. This could be prediabetic levels. Please try to control or contain it. Important points to control diabetes: a)daily 30 min walk twice in a day b)eat right, no fast food, no oily food, no spicy food c)no rice, no potatoes, no butter, no khee d)add more vegetables and salad e)no alcohol, no smoking f)weekly monitoring of weight and bp g)blood investigations like lipid profile h) almonds, soyabean i.) jamun powder j. Glucometer monitoring k. Small frequent meals.

Hello, Thanks for the query. Levocetirazine is fairly safe, however with a GFR of 45 ml/min, 10 mg dose is not recommended. Dose recommended for 30 to 50 ml GFR is 2.5 mg every alternated day. Thanks.

3 Causes Of Infertility In Women!

The inability to conceive after indulging in unprotected sex is known as infertility. It can also be referred to the biological incapability of a male to cause the conception or a woman being unable to carry the pregnancy for full-term. Research has shown that female problems contribute to over half of all the infertility cases while a majority of the other causes include sperm disorders.

Treatment of infertility depends on:

  1. Cause of Infertility
  2. Duration of Infertility
  3. Age of both partners
  4. Personal preferences

Causes:
Causes of infertility in women include:
1. Ovulation disorders: This is regarded as the most common cause of infertility in women. The disorders can be caused due to the following

2. Problems in fallopian tubes or uterus: Abnormalities in the uterus or fallopian tubes render the woman incapable of conceiving naturally. This might be due to:

3. Medications: There is a possibility that treatment will cure infertility. Examples include NSAIDs (non-steroidal anti-inflammatory drugs), Chemotherapy and Radiotherapy. Treatment of infertility might involve a significant amount of psychological, physical, temporal and financial commitments. In men, treatment is done to treat lack of healthy sperm or general sexual problems. The treatments include:

  1. Medications
  2. Surgery
  3. Change in lifestyle
  4. Sperm retrieval

Even though it is possible to restore fertility in women using only one or two therapies, a number of treatments might be required before conception is possible. Some of these treatments include:

  1. Intrauterine insemination (IUI)
  2. Stimulation of ovulation with fertility drugs
  3. Surgery to restore fertility

In situations where pregnancy does not happen spontaneously, Assisted Reproductive Technology (ART) can be used by couples to achieve pregnancy. It is a form of fertility treatment which involves the handling of sperm and egg. The entire ART team consists of psychologists, physicians, embryologists, nurses and lab technicians.

One common ART technique is In vitro fertilization (IVF). It is a process where an egg and sperms are manually combined in a laboratory dish, followed by transfer of embryo to the uterus. Some aspects involved in an IVF cycle are:

  1. Intracytoplasmic sperm injection (ICSI)
  2. Assisted hatching
  3. Donor eggs or sperm
  4. Gestational carrier

Some complications that may occur during the treatment of infertility are:

  1. Multiple pregnancy
  2. Ovarian hyperstimulation syndrome (OHSS)
