Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Acute middle back pain with sudden onset could be due to small disc protrusion at the back but it can also be due to other reasons as well. If you have only a localized kind of pain with no other associated symptoms like radiating leg pain, numbness, paresthesias etc, and no systemic symptoms like fever, loss of appetite, loss of weight etc, my advice to you for initial 2-3 weeks will be: - to maintain proper posture while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or even tab paracetamol which suits you - intensive physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied 3-4 times in a day - analgesic spray for local application like dynapar qps spray can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc let us observe you for a period of 2-3 weeks by which a lot of patients actually improve, if your pain still persists, then we will get some investigations done. Postural correction- sit tall, walk tall. Extension exercises x 15 times x twice daily. Apply hot fomentation twice daily. Avoid bending in front.

Hi Thanks for your query and Welcome to Lybrate. I am Dr Akshay from Fortis Hospital, New Delhi. Actually pain at calf/Leg region can be due to many reasons , commonly it is muscular in nature but it can also be due to Tendons, Nerves, Blood vessels etc. My advise to you for initial 2-3 weeks will be : - Ice packs application intensively with elevation of your limbs at night - Take a short course of anti-inflammatory medication as per your body's suitability - Stop your physical activity till pain completely subsides - Intensive physical therapy, training and proper muscle conditioning once pain has subsided - Slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - We will get some blood tests for you - Serum Vitamin D and serum Potassium levels, ESR and CRP Once you have all these reports then you can contact me as i can advise you for proper supplementation Do not hesitate to contact me if you need any further assistance. It requires detailed investigation and examination. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that patient is not allergic to any of the medicines that he/she is going to take. If it does not give relief in 4-5days, inform.

. Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.
