Know More About Prostate Cancer!

MBBS, MS - General Surgery, M. Ch. (Urology), DNB (General Surgery), MBA

Know More About Prostate Cancer!

Know more about prostate cancer Prostate Cancer is a form of cancer that affects aging men commonly. The prostate gland is responsible for producing seminal fluid or semen, from its position under the bladder, surrounding the urethra. Its function is under control of testosterone hormone produced by testicles. Prostate cancer refers to the growth of a malignant mass of cells which can also be called a tumour.

More about this cancer:

Risk Factors: The various risk factors are studied for the development this cancer. Advancing age, a family history of the same disease, obesity, western diet and black race are the risk factor for the development of this cancer.

Types: There are basically two types of prostate cancer, including fast growing or aggressive, and slow growing or non-aggressive. Yet, it is also important to remember that not every abnormal growth in this gland can be termed as a tumour, or is a sign of prostate cancer. A malignant growth signifies prostate cancer. This tumour can grow at a fast pace for aggressive prostate cancer, while it can grow and spread slowly for the non-aggressive type.

Symptoms: It is possible not to have any kind of obvious symptoms when suffering from this kind of cancer at early stage. The symptoms usually begin to show up when cancer has reached a more advanced stage. These include urinary problems, like poor flow, frequency, frequent need to wake up for urination during the night time. The patient may go through pain and difficulty when it comes to urinating. This pain may also occur during ejaculation. The patient may also find blood discharge in the semen upon ejaculation. Pain in the back bone and pelvic are also common symptoms of prostate cancer when it spread into bones. Spine involvement may gradually turn into numbness in lower limbs with paralysis. Blockage of ureters may lead to kidney failure

Diagnosis: The diagnosis of prostate cancer is suspected by digital rectal examination and PSA blood test. If any nodule or hard area is suspected on digital rectal examination or PSA is high, then prostate biopsy is done to confirm the cancer. When in doubt, Multiparametric MRI of prostate may guide the necessity of biopsy.

Treatment: Early cancer limited to prostate can be cured by surgery (Radical Prostatectomy) or Radiotherapy. Advancement in surgical techniques due to robotics and laparoscopy, has made Robotic radical prostatectomy as the first choice of treatment for localized prostate cancer. The advance cancer is treated by hormonal manipulation of testosterone blockade. It can be achieved medically by various injections or surgically by removing both testes. If cancer still grows despite hormonal manipulations, chemotherapy helps in rescue.

Prostate cancer is a treatable disease. We should not fear with it rather fight with it. Ultimately, we can win over cancer.

Pityriasis Amiantacea - A Guide!

MBBS
Dermatologist, Pune
Pityriasis Amiantacea - A Guide!

Pityriasis amiantacea is a disorder of the scalp. Some of the visible signs of this condition include adherent, silver, thick scales. The scales resemble the color of the asbestos and often surrounds the tufts of hair. While the condition is manageable, care should be taken to manage this condition right from its inception. Here is a brief overview of this scalp condition.

Origin of Pityriasis Amiantacea:
Pityriasis Amiantacea is inflammatory in nature. It comes from the seborrheic and psoriasis dermatitis spectrum. Experts are of the view that this condition is a result of the exaggerated response of the body to a primary condition of the scalp. Earlier studies have suggested that there is a possibility that Pityriasis Amiantacea may have originated from lichen planus, pyogenic infection and superficial fungal. In most of the cases, it has been witnessed that fungus has a close relation to this condition.

Exposure of Pityriasis Amiantacea and its diagnosis:
This condition has low exposure among females. It is mostly observed among, young adults, children, and adolescents. The diagnosis is done clinically. A specialist might prescribe dermoscopy to rule out other conditions of the scalp. If the result is positive, the scaling will reveal a diffused yellowish or whitish pattern. While the condition is local, it has all the possibilities to spread to other hairy parts of the body. Some experts are of the view that Pityriasis Amiantacea has a close association with temporary alopecia. While histopathology is not prescribed by most doctors, however, biopsy of the scalp is suggested in many cases.

Does Pityriasis Amiantacea result in hair loss?
The answer is yes. As a result of the scaly crust, the encased hair tends to fall. This being said, it is entirely a reversible condition. The hair comes back in 9 out of 10 cases. Medical ointments and antibiotic shampoo can help to grow back the lost hair. Apart from this, home-made protein solutions can help in the regeneration of the hair and get back the original color of the hair.

Treatment options:
This is a treatable condition that might take some time for a complete cure. It involves the keratolytic and tropical anti-inflammatory treatment. This is accompanied with salicylic ointment and topical corticosteroids. A tropical mineral oil is often prescribed by doctors to ensure that the adherent scales are removed. Some doctors might also suggest a clobetasol propionate shampoo. Even ketoconazole and silver sulfadiazine is helpful to get rid of the scales. If in case the infection is accompanied with Staphylococcus aureus, antibiotics are prescribed to ensure that the condition does not turn from bad to worse. Systematic therapies are suggested if this condition is accompanied by an underlying condition such as psoriasis.

Alcohol & Gastric Bypass - Can They Be Mixed?

Fellowship in Minimal Access Surgery, Fellowship in Bariatric Surgery, M.S.
Bariatrician, Lucknow
Alcohol & Gastric Bypass - Can They Be Mixed?

Gastric bypass, one of the weight loss surgeries bring about many changes and alterations (physiological, metabolic, hormonal) in your body. The gastric bypass surgery rearranges the stomach into an upper pouch (the smaller part) and a relatively larger lower part. It is the small pouch that is connected to the small intestine, where the absorption of nutrients and minerals from the ingested food takes place. The surgery plays a significant role in limiting the intake of food. Thus, post-surgery, a person's body reacts differently than it used to before the surgery. Many, if not all, people experience difficulties in coping up with the changes.

Patients are often advised to refrain from alcohol consumption after the gastric bypass surgery. Surprisingly, and quite alarming, cases of alcohol abuse increases post the surgery. According to a survey by the American Society for Metabolic and Bariatric Surgery, few people, who had no prior history of alcohol abuse developed one after the weight loss surgery. What triggers the alcohol abuse in some patients is still a debatable issue.

Many patients experience an emotional breakdown, stress, anxiety, and depression after the surgery. People who needed no reason to binge on food are suddenly expected and made to consume a limited quantity of food. Unable to do what they loved doing the most, people look for an alternate solution. As expected, in most cases, alcohol becomes the much-needed solution. What started as a means of distraction gradually becomes an integral part of life. Hormonal changes associated with the surgery can also result in panic attacks, stress, and anxiety.

Incidences of alcohol abuse may also be high in patients who had a history of drinking (regular or occasional) and smoking before the gastric bypass. Under normal circumstances (before the weight loss surgery), there is a partial enzymatic breakdown of the consumed alcohol in the stomach before it passes into the small intestine. However, post-surgery, the small size of the stomach does not facilitate such enzymatic breakdowns. Thus, the alcohol passes into the intestine, where it gets rapidly absorbed. As a result, a person gets into an inebriated state at a faster rate.

It is because of this reason that doctors advise the patients to keep away from alcohol as much as possible after the surgery. To keep away from or reduce the instances of alcohol abuse after the gastric bypass surgery,

  • Look for healthier and low-calorie food options when you get an urge to eat.
  • Meditate, go for morning and evening walks. These physical activities act as great stress busters.
  • If you are feeling low or going through an emotionally low phase, share it with your loved ones. Do not suppress your emotions.
  • Consult your attending physician in case of any physical discomfort

anxiety and depression

How to Choose the Right Sunscreen for Your Skin Type?

MBBS, DVD
Dermatologist, Bangalore
How to Choose the Right Sunscreen for Your Skin Type?

Sunscreens help shield you from the sun's dangerous ultraviolet (UV) rays in two ways. Some work by scattering the light, reflecting it away from your body. Others absorb the UV rays before they reach your skin.

A few years ago, choosing a good sunscreen meant you just looked for a high sun protection factor (SPF) -- which rates how well the sunscreen protects against one type of cancer-causing UV ray, ultraviolet B (UVB). SPF refers to blockage of UVB rays only.

Research soon showed that ultraviolet A rays (UVA) also increase skin cancer risk. While UVA rays don't cause sunburn, they penetrate deeply into skin and cause wrinkles. The Environmental Protection Agency estimates that up to 90% of skin changes associated with aging are really caused by a lifetime's exposure to UVA rays.

Whether you are heading out to the beach for relaxing day of swimming, or traveling out on the nature trail for a fun day of hiking, sunscreen should be an important part of your day. Follow the instructions contained in this guide to learn how to choose a sunscreen that is right for you.

Clearly, you will want a sunscreen with broad-spectrum or multi-spectrum protection for both UVB and UVA.

Tips for choosing the right sunscreen:

  1. Broad Spectrum: It means the sunscreen protects against UVB and UVA rays and helps prevent skin cancer and sunburn.
  2. High UVA protection: Sun's UV rays can be easily prevented, and thus severe kinds of skin troubles can be eliminated. In this case, the product label needs to be checked so that you can get 100% assurance that your skin will be protected against UV rays for sure.
  3. Water resistance: Pick a sunscreen which is Water Resistant (effective for up to 40 minutes in water) or Very Water Resistant (effective for up to 80 minutes in water). This means the sunscreen provides protection while swimming or sweating up to the time listed on the label. Even when using a water-resistant sunscreen, you should reapply after getting out of the water or after sweating.
  4. Medicated Products: Do not go by the local brands rather you must choose only medicated products.
  5. Daily usage: Using sunscreen on a daily basis is a great necessity. In fact, this is one of the most important steps for getting most productive effects. You can apply the cream, especially before going under direct sun.
  6. Skin-friendliness: The sunscreen should not contain any toxic elements or chemicals, otherwise your skin will get adversely affected. Different kinds of skin damages can occur such as spots, patches skin allergies or infections if you do not choose the right sunscreen apt for your skin.
  7. Correct application of sunscreen: Apply on exposed areas: You should apply sunscreen, especially on those body parts that usually remain exposed towards sun all the time. If you frequently wear bikinis, then you should include legs, thighs, arms, back, face and other areas.
  8. Enriched with moisturizers: If the sunscreen is equipped with moisturizers, then your skin will become smoother and soothing effects will be found. Some of the moisturizers consist of organic ingredients that can improve skin quality and texture to a great extent. These moisturizers also provide sufficient nutrients to your skin.
  9. Frequent application: If you are willing to get best results and utmost protection from UV rays, then nothing can be the right way-out other the applying the sunscreen on a frequent basis. After every two-hours, you are advised to apply the sunscreen.
  10. Wearing sun-protection accessories: Apart from applying the sunscreen lotion, you are advised wearing different kinds of useful sun-protection accessories for getting greater protection. Some of the most prominent accessories are sunglasses, long sleeves, umbrella and others.

Razor Bumps - Why Do They Occur?

MBBS, DNB(Skin & VD)
Dermatologist, Delhi
Razor Bumps - Why Do They Occur?

It is an inflammatory reaction to the hair shaft entering the skin, most common in blacks who shave regularly.

Who gets razor bumps?

About 50% of black males are said to be affected. Other races can also be affected. Though the condition is most commonly reported in men, women who remove unwanted hair also suffer from this condition.

Causes of Razor Bumps-

The condition is caused by both genetic factors and various hair removal methods.

Genetic factors:

  1. Curved hair follicles and curly hair
  2. A defect in the gene for the hair protein keratin 6hf

Hair removal methods may cause razor bumps by either creating a sharp hair tip that re-enters the skin away from the hair pore (extrafollicular penetration) or by the hair shaft breaking off at different lengths and piercing the follicle from within the skin (transfollicular penetration). An inflammatory reaction is then set up in response to the hair fragments lying in the skin. The following methods may cause razor bumps:

  1. Shaving
  2. Tweezing
  3. Waxing
  4. Electrolysis

Symptoms-

  • Ingrown hairs
  • Red, skin coloured or dark bumps
  • Bumps containing pus

What are the complications of razor bumps?

  • Pigmentary problems (darkening of the area most common)
  • Scarring
  • Abscess formation
  • Psychological problems

Razor bumps are most common on the beard, however they have been reported to occur on the pubic area, legs and scalp.

The Treatment-

There are several treatment options to deal with razor bumps already present. They include:

  • Moisturizers
  • Retinoids
  • Steroids
  • Antibiotics
  • Alpha-hydroxy acids
  • Chemical peels
  • Microdermabrasion

Prevention of Razor Bumps:

It is true to say that 'prevention is better than cure'. Here are the preventions-

  • Laser hair removal is the best method of removing unwanted hair in individuals affected with this condition. The laser device is programmed to selectively target the pigment in the hair follicle, destroying it, while leaving the skin intact.
  • Electrical clippers are found by many to cause less bumps than razors. If one must shave it is important to avoid a close shave to help prevent razor bumps. The following is a guide to safer shaving methods:
    • Brush hairs with polyester sponge before shaving.
    • Wash beard, use pre-shave cream or gel.
    • Avoid double and triple blade razors.
    • Shave in the direction of hair growth.
    • Do not pull the skin taut.
    • Use a sharp blade, rinse after each stroke.
    • Apply a cold compress after shaving, then a soothing aftershave preparation.
  • Clipping hairs with a small pair of facial scissors is an option for women with few unwanted hairs.
  • Chemical depilatories (hair removal creams) are another option for hair removal.
  • Electrolysis is a permanent method of hair removal but requires several sessions. It is operator dependent and can cause bumps and scarring. Waxing and tweezing also have the potential to cause bumps.
  • Eflornithine hydrochloride is a cream that slows the growth of hair and therefore can be used in combination with a hair removal method. It is marketed for women with unwanted facial hair.
  • Anti-androgen drugs can be used in females who have hirsutism (male pattern hair growth e.g. beard hairs)

You may be suffering from androgenic alopecia which is also called as pattern baldness. The hairs start reducing in density at the crown (top) and also the hairline starts refeeding from the front. This is usually progressive throughout life unless treated. This is usually accompanied by severe dandruff which we call seborrhoea. The FDA approved treatment for this condition is a lotion and tablets. There is no other alternative. No oil or cream will magically work because the hairs are getting miniaturised and eventually disappearing due to the hormonal effects on the hair roots. Please call me or contact me with photos using online consult option for proper examination and treatment. Do not start treatment without consulting. I can give guidance and then you can continue treatment on your own.

Post Kidney Transplant - 6 Diet Tips

Post Kidney Transplant - 6 Diet Tips

A kidney transplant is a surgical procedure where your kidneys are cleansed and rebuild using the kidneys of a deceased person, in order to flush out the excessive waste fluids from the kidneys. It is of paramount importance and your dietary patterns and lifestyle should be altered after you go through a kidney transplant. Your dietician will help you out by preparing your diet chart to clearly list out the foods that you should consume and those which you should avoid after the surgery. The diet chart that would be prepared for you would be highly subjective and it would totally depend on your lifestyle, medical history, family health records, genetically transmitted conditions and such others. Apart from improving the condition of your kidney and assisting your transplant, the diet chart will also help you to lose weight, boost your metabolism and enhance your immune system. Some of the most common recommendations that your dietician may give you are as follows:

  • Eating at least 5 full servings of fresh fruits and vegetables each day as both are an important part of a healthy diet for a person who has just undergone a kidney transplant. The vegetables should mostly be eaten after cooking/boiling as per the suggestions of your dietician.
  • Having enough of fiber rich foods in your daily diet is essential as it speeds up the process of digestion and prevents constipation by promoting frequent and easy bowel movements.
  • If you are a non-vegetarian, then you should be going for lean meat alternatives which are low on fat and high on nutritional value. Examples of such meat are fishes and poultry. Additionally, you should also avoid all kinds of red meat as it is high on fat.
  • Consume low-fat milk and dairy products as they help to prevent the fat accumulation inside the body and also maintain an acceptable level of calcium and phosphorus inside your body.
  • Maintain a low level of iodized salt in your meals, as it helps to keep your blood pressure under control.
  • Over the counter health supplements like vitamins and proteins can also be taken, if recommended by your dietician.

Brow Lifts: Post-Surgical Care And Recovery!

MBBS, M.Ch - Plastic Surgery
Cosmetic/Plastic Surgeon, Ahmedabad
Brow Lifts: Post-Surgical Care And Recovery!

A brow lift is also known as a forehead lift, is a surgical cosmetic procedure that corrects flaws in the skin above the eyes. It flattens the lines and wrinkles on the skin, tightens the sagging skin that appears to advance in age and restores youthful skin. More and more people are undergoing the treatment every year with a high rate of success.

Features that can be corrected with brow lifts:

  1. Sagging and horizontal eyebrows that create an expression of exhaustion
  2. Deep horizontal folds on the forehead skin
  3. Excess fatty skin that droops over the eyelids
  4. Frown lines between the eyebrows

Types of brow lift and the respective procedures

  1. Traditional or coronal brow lift: The patient is put under general anesthesia and an incision is made on the forehead, just below the hairline to hide the scar. The excess skin is removed and the necessary skin tissues are lifted and tightened for a smooth appearance. Finally, the incision is stitched up.
  2. Endoscopic brow lift: This surgery is also done under general anesthesia, but a number of tiny incisions are made on the scalp. An endoscope is put in through the incisions so that the area of surgery is seen on a computer screen. Other surgical instruments are used to lift the skin tissue on the forehead, reposition the eyebrows and remove all the extra skin and muscle tissue. Then, the incisions are closed with stitches. Now-a-days endoscopic brow lift is undertaken requring specific instrument and training.
  3. Temporal incision brow lift: This procedure is done with eyelid surgery. It requires larger incisions than those of endoscopic brow lift and these cuts are made just above the temples. Then the tissues above the eyebrows are repositioned, extra tissues between the eyebrows are also removed to flatten the frown lines.

Post-surgical care and recovery

  1. You have to stop smoking and taking anti-inflammatory or blood thinning medication for a few weeks before and after the surgery.
  2. Using a cold compress is recommended in case of post-surgical pain. There may also be prescribed ointments that are applied at the cut.
  3. Your head needs to be kept elevated when lying down for a few days.
  4. You should have gauze and clean towels readily available in case of minor bleeding.
  5. You may also experience slight swelling and bruising on the non-operated areas of the face. The pain and itching sensations experienced during recovery are milder in case of traditional brow lift than in case of endoscopic brow lift.

Here's what you can do to maintain a healthy prostate:

1. Workout to stay fit - Exercising regularly and maintaining an optimum weight can help to keep your prostate gland at an optimal health. Several types of research have revealed the beneficial aspects of exercising. It's said that physically active men are less likely to suffer from enlarged prostate if they performed a physical activity of low to moderate intensity. Likewise, physically active men who ran for 90 to 108 minutes are 20% less likely to develop erectile dysfunction.

2. Eat fruits and vegetables - Including fruits and vegetables in your diet can help to maintain a healthy prostate. Guava, papaya, tomato and watermelon are some of the options that spell good for your prostate. Vegetables like cabbage, cauliflower, broccoli and Brussels sprouts should also be included to boost the health of the gland. Ideally, you should have about 5 servings of fruits and vegetables daily.

3. Have more soy products - Eating more soy products like tofu, soy flour, soy nuts can help to keep cancer of the prostate at bay. A study published in the British Journal of Urology International revealed that men who had soy products saw a 40% decrease in their risk of developing an enlarged prostate.

4. Have selenium-rich foods - Having foods rich in selenium (a powerful trace mineral) like tuna, eggs, cashews, onions, garlic, etc can reduce your risk of developing prostate cancer.

5. Say yes to healthy fats and no to trans and saturated fats - Choosing healthy fats like avocados, nuts, and olive oil can aid in boosting prostate health. At the same time, limiting saturated fats and trans fat intake are vital for keeping the prostate gland healthy.
