Contact Dermatitis - 3 Common Types You Must Know!

The condition which occurs on exposure to foreign substances and causes the skin to become red, itchy and inflamed is known as contact dermatitis. It is commonly known as a rash. Although most of the reactions to contact dermatitis are not serious, they can be very irritating till the itching subsides. The causes of contact dermatitis are usually case specific; they vary according to the different types of this disease.

The types of contact dermatitis are:

  1. Allergic contact dermatitis: This condition occurs when an allergic reaction is developed after exposure to foreign substances. The reaction occurs with a release of inflammatory chemicals that causes the skin to become irritated and itchy. If a person has eczema, he or she can suffer from this kind of rash. Causes of allergic contact dermatitis include:
    • Latex gloves
    • Poison Oak
    • Jewellery (gold or nickel)
    • Perfumes and chemicals in skincare and cosmetic products
  2. Irritant contact dermatitis: Contact between the skin and a toxic material causes Irritant contact dermatitis. They can be sub divided into two categories, namely chemical irritants and physical irritants. Some of the toxic substances that cause this condition include battery acid, drain cleaners, detergents, kerosene, pepper spray and bleach. The physical causes of Irritant contact dermatitis include low humidity from air conditioning, a variety of plants and so on so forth. The condition tends to become more painful, rather than itchy.

Symptoms 
Symptoms of contact dermatitis depend upon the causes of this condition. The symptoms of allergic contact dermatitis are:

  1. Dry or oozing blisters
  2. Dry, flaky and scaly skin
  3. Redness of the skin
  4. Extreme itching
  5. Burning of the skin with or without sores
  6. Leathery or dark appearance of the skin
  7. Hives
  8. Swelling, particularly on the face, in your eyes or groin regions

Symptoms of Irritant Contact dermatitis can be slightly different. They include:

  1. Blistering
  2. Swelling
  3. Cracking due to extreme dryness
  4. Ulcerations
  5. Stiff or tightened skin
  6. Open sores that form crusts

A breast lump is an enlargement, swell, protuberance or a bump that is different than the breast tissue surrounding it. Breast lumps can appear in men as well as women and they can be benign or malignant. Causes of benign breast lumps are as follows:

  1. Fibroadenosis: Also known as Fibro-cystic breast disease, it is a benign condition that affects the breast. It may affect either of the breasts or both. One of the major symptoms of Fibroadenosis is a breast lump. Usually, the lumpiness disappears after the menstrual cycle is over. Fibroadenosis occurs because the breast tissue doesn’t respond well to the hormonal changes during the menstrual cycle.
  2. Fibroadenomas: Fibroadenomas occur because of abnormal response to the hormone oestrogen. The lumps are usually well formed and round, around the milk ducts of the breast. Breast lumps due to Fibroadenomas are called breast mice, because they move around the breast. In some cases, they disappear and in some, they tend enlarge during pregnancy.

Other benign causes of breast lumps are breast abscesses, breast cysts, lipoma (lump of fat), mastitis (infection in the breast tissues), fat necrosis (another name for breast lumps) and intraductal papilloma (benign tumour of the milk ducts in the breast). A breast lump that is firm, well defined and doesn’t move around, i.e. it doesn’t disappear after your period or pregnancy, may be a result of breast cancer.

Treatment of breast lumps
The treatment of a breast lump depends on its cause. Benign breast lumps usually disappear over time. Otherwise following treatments can be considered as per the doctor’s discretion-

  1. Medication: If the breast lumps are painful or increase over time, it is advisable to take medication.
  2. Limiting the amount of fat in the diet or wearing well fitting bras also reduces the amount of pain due to breast lumps.
  3. Certain types of breast lumps are filled with fluid or pus. This fluid can be drained through a procedure called aspiration or drainage.

Delivering a baby is a painful, but also a pleasurable moment for the mother. But if the delivery is done through a C-section surgery or what is more commonly termed as Caesarean delivery, the pain is not felt during the actual delivery since the woman in under anaesthesia. However, the real challenge is to recover from the operation. The first week is particularly critical. Here are some things you can do to manage it with less discomfort:

  1. The first few hours, naturally, you will be under close observation till you fully regain conscience if it was a general anaesthesia. If only the lower portion was numbed for the surgery, then also the feelings in the legs etc. will return within a few hours.
  2. You will start breastfeeding you baby; wear comfortable clothes so that you can feed the baby without any discomfort. If this is the first experience, learn things that you need to know through books or from friends and relatives who would have had a C-section earlier.
  3. You may feel more comfortable in a recliner than a flat bed. Most hospitals should be able to provide you one.
  4. You might be better off with a heater pad of your own, since your back may be aching considerably and bringing down the pain is essential.
  5. Wearing a binder around your tummy is another suggestion given by many women who have had a C-section delivery; besides helping with easing the pain, it also prevents the tummy from bulging, which you would want to avoid.
  6. Follow the obstetrician’s advice on having a shower and also on resuming some of your physical activities without hurting the stitches or the sutures.
  7. By Day 3, you should be able to resume eating your normal diet; in many cases, it could be earlier than that.
  8. Do take care to avoid lifting anything heavy. The heaviest object you can lift is your baby. Make your husband or any other person to help you through this period to avoid any complications later.
  9. How about medicines? If you are already on any kind of medication and if you had stopped any of them due to the surgical procedure, check with the doctor and resume taking the medicines.
  10. Keep a close watch on the food you eat, particularly since you will also be nursing your baby. Don’t hesitate to consult the doctor on things you may be fond of, but have to avoid in order to protect the kid from being affected in any way.

Pregnancy and delivery are some of the happiest moments in any mother’s life; so remain cheerful throughout and enjoy the moments. So what is Vitiligo? Vitiligo is a pigmented disorder in which the pigment producing cells of the skin which is known as the Melanocytes are destroyed resulting in white patches all over the skin.

Now, what are the symptoms of Vitiligo? Vitiligo is usually noticeable white patches on the skin. These patches are more commonly found in the area where the body is exposed to the sun including the hands, feet, arms, face, and lips and these patches are usually symptomless. Vitiligo generally appears in one of three pattern-

First is the focal pattern, segmental pattern and the generalized pattern. In the focal pattern where the depigmentation limited to one or few areas.
In segmental pattern where the depigmentation develops on one side of the body.
The third is a generalized pattern. This is the most common pattern and in this depigmentation occurs symmetrically on both sides of the body.
Now, who are prone to Vitiligo? Although the course of Vitiligo is unpredictable but there are some certain factors also for the Vitiligo. Without proper treatment from a dermatologist, it can spread very rapidly and can be very extensively. So it is better to start treatment as early as possible before spreading of the disease. It may have negative impact on the social life of the patient also. It may have negative impact on psychological life of patients. So we are expert in treating Vitiligo. There are basically three types of treatments or therapies-

First is the medical therapy, second is the photo therapy, and third is the surgery. So in the medical therapy, just to stop the progression of the disease, this is given in the form of tablets or topical. And second is photo therapy, photo therapy is given in the form of UVA and UVB lights. It activates the pigment producing cells which is melanocytes, and helps to bring the color the color the black. Third type of therapy is surgery. In photo therapy Vitiligo surgery, usually it is done in the case of unstable Vitiligo. That is, if the area is not increasing its size and if it has been stopped last four to five months then only we can do the surgery. It is very effective on the patches of lips, hands, neck, or eyes, which is difficult to treat with the medical therapy.

There are three types of surgeries available-

Punch grafting which is very simple and economical procedure. The grafts are taken from the legs and it is put on the bigger areas but the thing is there that the cosmetic result is not as good as in case of other types of surgeries. Second is the blister grafting and third is the melanocyte transfer. In case of blister grafting, it is a very good type of grafting. This is meant for the smaller body surface areas. The results are very good and we never come to know that whether the patient has got surgery or not. The third type of surgery is the Melanocyte transfer technique. This is a highly specialized technique and good for the bigger areas and cosmetically having good results.

Types of Burn and its Treatment

Hello friends, I am Dr. Anubhav Gupta. I am a consultant plastic surgeon working at Sir Ganga Ram Hospital in New Delhi. Today I am going to discuss a very common topic that is burns and its sequelae, the commonest problems which are caused by burns that are contractures. I would answer the commonly and the frequently asked questions related to these topics.

Q. What is the first thing I should do when I am burned? Dr. See it s a very common thing that most people don t know. Wherever suppose some person is burned, the first thing is to stop the source of that fire. It can be done by many methods, by pouring sand, etc, the second thing is you should try and bring that part of the body under running tap water. It should be cool, it should not be cold or freezing, it should be just cool and let that part of the body be continuously under water. What it does is, it takes away a lot of heat and prevents the burn from getting deeper.

Q. What are the different degrees of burns?
Dr. Basically burns can be divided into three degrees, the first degree, second degree and third degree. First degree burns are the superficial ones wherein there is usually only redness which heals without any consequences. Second-degree burns are again of two types; second degree superficial and second degree deep. Second-degree superficial ones are burns with blisters, which may also heal on their own. It is the second degree deep and the third-degree burns which do not heal on its own and they require a treatment by a plastic surgeon.

Q. What burns came scaring and later contractures?
Dr. Basically second-degree burns and third-degree burns cause scarring and later contractures.

Q. Who should I approach for the treatment of contractures?
Dr. Contractures are treated by plastic surgeons.

Q. What surgery is done to treat contractures?
Dr. Basically contractures are when the body heals in burns it causes the part to come closer, leading to contractures, so there are a lack and deficiency of skin which causes contractures. What needs to be done is there, we put an incision on it, we cut it and we release it and then we take a graph, skin graph from the other part of the body and we put it on that released area and then after that we give a splint, so that the body is placed in the position it was before when there was a deficiency of tissue that is before burns. We tried to put the body or that joint in that position which is a normal position for that. And hence that is how we treat contractures.

Q. How long does it take for the healing to complete?
Dr. Basically healing of the contractures takes two-three weeks after we do the graph. After that, the patient is advised to take splinting.

Q. What is the advice to prevent contractures after burns?
Dr. See it is very important. After burns the patients, if it is deep burns should be given splinting. And they should be encouraged to do physiotherapy. Kayi baar male person intense itching ko ruk nhi pate hain and public place me bhi solve krne ki koshish krte hain. phimosis and paraphimosis young adults me rehti hai. Andar debris ikatha ho jata hai and itching hoti hai. Paraophimosis me skin piche aa jati hai. Is place me kafi swelling ho jati hai. Painfiul hone ke karan operation ki jrurat pad jati hai. Urine pass krne me patient ko problem hoti hai. Baar baar infection hota hai. Agar aise koi problem hai apko to doctor se consult kren. Iski ek surgery hoti hai, vo aap kra skte hain. Agar surgery ki jrurat nhi hai to pehle btaye gye lubricant ka use kren. Pain hota hai during intercourse, masturbation me intense pain or itching hoti hai. Kuch blisters ho jate hain.
