Palliative Care for Cancer Patients

With terminal illnesses like cancer, the disease is not the only thing that requires treatment. In addition to controlling the severity and spread of the main disease, the effect it has on the other body systems including emotional, social, psychological problems and also physical issues like pain and inflammation. Though the disease has reached an advanced stage and cannot be cured, the patient can be made to be pain-free and at peace to the extent possible.

The goal of palliative care is not to cure - in fact, palliative care is in place for diseases like cancer from the time it is diagnosed and treatment is begun but becomes the major component of treatment once the cancer is identified to be in terminal stages. Therapeutic care aimed at curing the cancer is gradually reduced and palliative care assumes a bigger role. This is also called as symptomatic care, supportive care, or comfort care.

Aspects of palliative care: once the disease is identified to be in terminal stages, then the following become considerations-
1. Stay at home or hospital
2. Withdraw chemotherapy and radiation?
3. Withdraw feeding tube
4. Spiritual discussions
5. Reduce anxiety
6. Good quality time with the family
7. Reduce pain and suffering
As is evident, there is a huge component of social/emotional/spiritual management in addition to reducing pain and suffering.

Physical: pain, fatigue, shortness of breath, sleeplessness, loss of appetite are some symptoms that need to be managed. In addition to pain killers, small exercises can be included to make them feel better, even if it is just getting a breath of fresh air.

Social: depression, anxiety, uncertainty, fear are all looming large and they are not sure how to manage these feelings. Talking to a counselor or some close family member or friend can be a big relief - the burden is off their chest.

Legal: there could be issues related to property, insurance, property that also need to be discussed. External help can be sought if required to provide financial counselling and legal advice.

Spiritual: looking into the spiritual needs and understanding the deeper meaning of life, restoring faith are some things that also can help them feel better" why did this happen to me" is a question most people keep asking and while there is no answer, some solace can be provided through talks.

It is to be noted that palliative care is not just for the patient. The caretakers (family and close friends) are equally in need of some support. So, once the patient is identified to be in terminal stages, all effort is made so the final leg of the journey is peaceful and as painless as possible.

For many women, their monthly period is nothing more than an annoyance; but other women may suffer some problems. Visit us at www.Malhotraayurveda.Com

I had missed my period and clinic test showed positive but I do not have nausea nor do I vomit nor gag to any smell. What usual things happen with people like vomiting is not happening with me. I am worried if everything is fine or not or am I really pregnant or not. Clinic test was positive when I was already 7weeks, till then I didn't realise I was pregnant.

Your question is not clear, still if you want to know that your pregnancy is the cause behind your hypothyroidism, its not the case but the vise versa can happen that is your hypothyroidism was one of the reason behind miscarriage, you should try to have normal thyroid levels before conceiving.

Totally wrong idea. Igg positive means she is immune to it- that means she had previous infection but she can never get this infection again. Moreover, pls get informed that rubella testing for miscarriage is an obsolete test as rubella can never give rise to repeated miscarriage. Rather try to look for other possible causes and get them corrected before pregnancy.

Techniques for Managing Hypertension During Pregnancy

Hypertension during pregnancy can be a problem for both baby and the mother. Thus it is of utmost importance that during pregnancy a good health should be maintained along with controlled blood pressure and cholesterol levels. With an increase in multiple births and women of older age the risk of hypertension during pregnancy has increased. But if proper care is taken it can be avoided.

Types of Pregnancy Hypertension:
There are three prominent forms of hypertension that can be seen during pregnancy. The pregnant ladies should be aware of the same. These are:

1. Preeclampsia: This is the most common and serious hypertension during pregnancy. This hypertension can only be controlled by delivering the fetus, which usually involves complications like death of the mother or child. This occurs 20 weeks after pregnancy.

2. Gestational Hypertension: This form is only prevalent during pregnancy and is not a problem for the mother or baby after delivery. This usually occurs in the last leg of the pregnancy

3. Chronic Hypertension: This form forms either prior to the pregnancy or before 20 weeks of the pregnancy.

Management of Pregnancy Hypertension:
Hypertension during pregnancy can be handled by the following:
1. In case of severe hypertension, blood pressure medication should be continued during pregnancy.
2. If you are on ACE inhibitor type medication, then the medication is changed to one that is even safe for the baby.
3. Your doctor might like to monitor you daily and can advise hospitalization for a few days
4. If medication is missed, it might lead to uncontrolled life threatening hypertension. Thus the medication should not be missed at any time.
5. In case of mild hypertension and absence of other diseases like diabetes and kidney disorders, the doctor might stop the medication or reduce the dose. Also, being off medicine does not cause any problem in mild hypertension.
6. Irrespective of the hypertension being mild or severe, the prenatal appointments should not be missed, so that the doctor can monitor you and the baby. So that problems, like rise in blood pressure, poor fetal growth, and signs of preeclampsia can be spotted and steps can be taken for the same.
7. In case there is some form of hypertension present the prenatal visits and lab tests will be more.
8. Apart from the usual second trimester ultrasound, there will be periodic ultrasounds in the third trimester to monitor the baby's growth and the amniotic fluid.
9. Also regular fetal tests and Doppler ultrasounds will be done to track the baby's growth.
10. Lifestyle changes should be made. Salt intake should be limited, fresh food instead of processed food should be consumed.
11. If blood pressure is high then doctor might ask you to avoid exercise especially if you never did before pregnancy.
