Hirsutism is a condition of unwanted hair growth in certain areas of a women's face and body, such as upper lip, beard area, chest and abdomen, that creates a "male pattern" of hair growth. It affects around 5-10% of the women and is a common presenting complaint among females for cosmetic reasons.

Hirsutism is attributed either to increased production or increased sensitivity of the hair follicles to the male hormone- testosterone. Polycystic ovary syndrome (PCOS), which causes irregular menstrual cycles, is the main cause of the same. However in some cases the reasons for the same can be unknown as all reports may seem normal. Such a type is called Idiopathic Hirsutism. It is important for women to not feel shy and embarrassed about the situation.  Often resorting to excessive bleaching and razers can harm the skin and the self image of a woman. You should come forward and talk to either a Gyneacologist or an Endocrinologist. The doctors after taking the complete history would advice certain tests that would measure the amount of certain hormones in the blood. If the androgen levels are elevated, doctor may recommend an ultrasound or a CT scan to check ovaries and adrenal glands for tumors or cysts.

The main causes of Hirsutism are:

  • Polycystic Ovary Syndrome: In 70% of the cases PCOS is the cause of Hirsutism. Women with PCOS often have hirsutism in combination with acne, balding near the front of the head, and menstrual irregularities. PCOS is also accompanied by infertility (due to menstrual irregularity), obesity, diabetes , High cholesterol and possible heart disease.
  • Idiopathic Hirsutism: Idiopathic hirsutism refers to hirsutism that has no identifiable cause. It is usually a mild variation of PCOS. A gradual increased growth of coarse body hair is typically the only symptom in women with this condition. Menstrual cycles are normal in women with idiopathic hirsutism.
  • Drug Induced: Certain drugs and steroids taken the past can trigger a hormonal imbalance and cause hirsutism
  • Adrenal or tumour causes: These are rare and are related to tumours or abnormal functioning of the adrenal gland.

Since Hirsutism can be a combination of medical issues self medication is not recommended. There are a variety of ways through which the same can be managed or cured. The treatment options are:

  • Medical Management: The doctors can recommend a combination of drugs such as oral contraceptives (contain estrogen & progestin that inhibits the male hormone), anti-androgens and insulin sensitive drugs.
  • Cosmetic Hair Removal: Electrolysis and the modern day safe Laser therapy are sought after cosmetic ways for removal of unwanted hair. The procedures destroy the hair follicles and prevent the hair from growing.
  • Surgical Therapy: Only in specific cases Laparoscopic ovarian drilling or removal of ovarian & adrenal tumours can help treat Hirsutism.

It is important to understand that this problem is medical and can be treated, managed and cured with expert advice & guidance. Women usually get depressed and have image issues. One should not get distressed; it may be a possibility that the reason may be just a little imbalance in your hormones. Eating healthy, weight loss and exercise are tried and tested natural ways to keep your body fit & balanced.

My wife has not returned to her regular periods since fourteen months after her first delivery. Also had put on more weight during these months. We thought of going for test thyroid. Kindly suggest if these conditions are common or if requires medical attention.

Yes, u should check thyroid hormones. Check s. Tsh & free t4 level. And also usg pelvis if still irregular.

FACTORS AFFECTING CALCIUM ABSORPTION
Incorporating healthy foods in the diet is not enough. One should understand factors affecting the absorption of nutrients from the food. Some factors are responsible for increased absorption of particular nutrient whereas some decreases its absorption. Calcium is the hardest mineral to get absorbed in the body. 30-80 % of the dietary calcium is not getting absorbed in the adult body.

Factors increasing absorption of calcium:
1. Vitamin d: vitamin d works in the digestive tract to absorb calcium into the blood stream from the walls of the duodenum. Vitamin d also helps maintain normal blood calcium level.

2. Parathyroid hormone: it increases calcium transport across the membrane of intestinal cells.

3. Acid environment: hydrochloric acid secreted in the stomach during the process of digestion is required for the absorption of calcium in the duodenum. Calcium supplements could be taken with magnesium at bedtime or between meals due to the acid environment needed in the stomach to assimilate the calcium. Always consult a physician before beginning a new supplement regimen.

4. Milk lactose: it favors absorption in infants. Lactose is acted upon by intestinal microbial flora to form acid which causes lowering of ph which makes calcium more soluble.

5. Amino acids: supplemental calcium is often chelated, or combined with protein molecules called amino acids, to help the body absorb them during digestion. Lysine and arginine increase calcium absorption.

6. Exercise: exercise along with vitamin d intake helps calcium absorption thereby making bones strong.

Factors decreasing absorption of calcium:
1. Oxalic acid& phytic acid: foods that are high in oxalic acid such as spinach, chard and chocolate, reduce calcium absorption. Oxalic acid binds with the calcium to form a salt crystal, calcium oxalate which cannot be absorbed. Phytic acid, which is found in whole-grain foods and high-fibre foods, effects calcium absorption the same way.

2. Phosphorus: too much phosphorus in the diet causes precipitation of calcium in the form of calcium phosphate.

3. Stress: stress can have a negative effect on hcl production in the stomach and on normal digestive behaviour in the body, and can therefore have a negative effect on calcium absorption.

4. Caffeine, drugs like anticoagulants, cortisone and thyroxine reduce calcium absorption.

5. Lack of exercise and vitamin d deficiency leads to decrease calcium absorption.

Hi! while you are feeding the baby, hormonal changes in the body can lead to missed periods. This time your pregnancy test is negative, so you should expect your periods after some time. If you don't want pregnancy now, please use some suitable birth control method because you can get pregnant even if you are not getting periods. Take care and let me know after 2 weeks if you got your periods or not.

Dr. I am 32yrs. & suffered PCOD; Nearly 1 yr. The fertility treatment was taken. For last four month I took the Lupigest inj. 150 mg - 2 per month, OOSURE-2 per day, Ovaashield caps & after 18th day cap. Lupigest200mg (10). But yet now not success. Is any side effect for continuations of inj. & hormone caps.

Effect of these injections are for very short duration and there is no lifelong effect or major side effect. As you r taking these injections it is better to go for iui rather than relying on natural intercourse specially when you r suffering from pcod.

This helps the doctor to see the internal organs, which is then used to remove the cyst. This method is less painful and has a quick recovery time.
  2. Laparotomy is the other method used for removing a cyst. In case the cyst is large, then one single large hole is made in the body to access the cyst. At times, the cyst and the ovaries are removed and sent for further laboratory inspection. This inspection is done to detect whether the samples are cancerous or not. This procedure is more complex than the previous one and might need the patient to stay in the hospital for a few days.
  3. In case you have a cancerous cyst, then your reproductive part itself would have to be removed rendering you barren for your life.

Ovarian cyst is very much treatable and is best to be detected at an earlier stage for better and faster treatment. Thus, it is best to go for regular check-ups and report in case of any disturbing symptoms.

Hello Savita, Yes, you can do normal work other than lifting heavy weights and excessive straining in the 1sttrimester. there is no need of bedrest unless indicated due to any complication in the pregnancy

Uterine prolapse is a condition which occurs due to a weakness in the muscles and ligaments of the pelvic floor which are responsible for supporting the uterus.   1. Severe pain in the lower back region along with significant problems during urination such as leakage of urine.
  2. Bulging out of tissues from the vagina.
  3. Recurrent instances of constipation and abnormal discharges from the vagina.
  4. Infection in the bladder and experiencing severe problems during sex intercourse.
