I am suffering from anxiety for the past 6 months. I used earlier paxidep cr 25 mg, then librium, then etizola 3 mg per day. Now I started with prodep 20 mg. But etizola and librium worked well for particular period. Now I am unable stand or walk.

Usually when we discontinue paxidep from 25 to suddenly 0 mg (that is stop )there is a withdrawal effect comprising of uncomfortable symptoms like you are currently having. So ideally it should have been tapered over 2 weeks, a duration by which prodep usually starts having its effects. About Etizola and librium, they are not cure for anxiety problem. They are just quick fix for short term relief .if used for long duration, people get dependent on them and they can cause memory problems along with gradual increase in their dose due to development of tolerance (meaning you will have to keep on increasing their dose gradually with time ). Kindly consider addressing the root of problem with counselling/psychotherapy.

I have anxiety disorder for last 1 year. I take prodep 20 once a day after lunch. I feel anxious some times in a day. Feeling like going to die, dizziness,stomach upset ,palpitations, shortness of breath, depersonalisation etc. I am very tensed about my anxiety. I have no problem as my doctor has said. May I live the life as previous?

Ideally paxidep or nexito is more beneficial in your kind of symptoms. Prodep is also a gud medicine but sometimes it is not a defective as other options .consider discussing these options with your psychiatrist.

I take describes medicine from 5 years continue due to some mental problems. Medicine described by the doctor. Now I take it without prior intimation of doctor .at present I am normal. Without these medicine I feel abnormal. Medicine are depran forte and zenoxa 150.Please suggest.

Zenoxa works in favour of a number of mental and physical problems. It is difficult to comment on need without having any idea about symptoms and their intensity. Depran forte is a combination of anti depressant and anti anxiety .and these medications should never be taken without supervision as they affect neurohormones. Kindly consider getting in touch with a psychiatrist online or offline and reconsider the possibilities of either continuing or stopping or augmenting the current medications depending on the need as assessed by the expert.

Usually there is no such restriction on sex during pregnancy as long as your partner too wants to participate. Forceful or violent sex is never advised.

Unprotected sex se pregnancy ke kaafi zyada chances hote hain. Ye iss par bhi depend karta hai ki aapke partner Apne menstrual cycle ke kis phase par hai, aapke sperm counts and quality normal hai ya nahi, uterus normal hai ya nahi aur usme hormones ka sahi effect hai ya nahi, .etc etc. Aisi kai sarey factors ka asar pregnancy ke honey ke liye zarrorat hain.

Brain Exercise! A Preventive Approach

The brain is not a stagnant realm fed with a granted quota of intelligence and memory at birth. It is always in a state of flux. Exercising your brain in various ways accelerates its ability to perform better. It is in your hands to have a sharper take on situations and occurrences. Playing games that require brainstorming, solving quizzes, crosswords or playing chess makes you feel rejuvenated and tired at the same time; the reason being activated brain cells. A healthy brain will affect your life decisions positively and you probably will think rightly before you leap.

Tips to exercise those brain cells:

1. Meditate and calm your mind: Ten minutes of meditation each day can help you get over anxiety. When your mind is at peace you think out pros and cons with enhanced insight. This composure reflects in each of your actions. Meditation demands your brain to be quiet when it is accustomed to work; you therefore have greater control over it.
     
2. Music can improve your brain abilities: Music is the best antidote for a sloppy brain. Music excites the neurons healing a bruised memory. It gifts you with clarity of thought. Apart from listening to music one must try to play a musical instrument. Teaching yourself things puts your mind through a strict regimen of constructive activities.
   
3. Take out time to learn a new language: We are paralyzed the minute our brain gives up. Due to several external and internal reasons each one of us is likely to suffer from cognitive disorders on being subjected to extreme pressure or shock. Trying to learn a new language motivate your brain cells to have a wider vocabulary, which further restricts your chances of brain damage.

4. Mental mathematics could better your intelligence: Calculators, computers and phones are ridding us of our ability to compute individual data. Computation and consolidation of data helps your mind to work faster with precision. So, you now know those lessons on mental mathematics in childhood were actually the key to a well-rounded brain. 

5. Think of something novel: The more the number of neurons the merrier is your brain. Take pains to direct your mind on a novel track. Thinking beyond what is given and expected helps your brain grow new neurons. Once in a lifetime all of us have had symptoms like heaviness in heart, increased heart beats, feeling that blood pressure will go up or fall down, chest pain, giddiness, heaviness of head, abdominal discomfort, dizziness, tremors in hands or shivering in body, etc which start suddenly without any stress or tension. They just peak in few minutes and are often accompanied by thoughts - 'I will die!' or 'Something will happen to me?'

'Everything around me has changed', 'my blood pressure will shoot up' A midst of the rapid climbing of the symptoms, the usual behaviour opted by most of us is rush to an emergency ward of a nearby hospital. All the check ups, blood investigations, ECG- electrocardiogram, Blood pressure assessment, etc done fast by the doctor. To our surprise 'Are tension kyu lete ho? sab normal hai !!' 

This makes us more worried that all the examinations and investigations are normal?? Then why this happened. Oh! this has to be something else which this doctor or hospital is not able to figure it out. So basic human psychology goes for a 2nd opinion and then 3rd, 4th and infinite, till we get a whole body check up. Sometimes same reports at different laboratories and wait for the diagnosis of what exactly is going on.

Everywhere the answer is 'Dont worry, You have nothing'. This hurts. Isn't it? More painful part, when it continues for few months, our family and near ones feel that this is nothing but stress and we should forget it. Some great advisors sympathize and advise for yoga and relaxation and go out of the station or spend time with friends, etc. But we know the agony it takes to have those symptoms and feeling blocked up everytime. Ahead fear enters in those places and spaces where this happens. We avoid them, like driving vehicle, queue, crowd, markets, travel, lift, theatre,etc. We feel that someone should accompany us and we will be safe.

This affects our work, time and major areas of life. What it is?? 

This is in simple words 'Panic disorder'. It is an anxiety group of disorders where mind misinterprets bodily symptoms and links it with situations or places and then the anxiety symptoms start. Why this happens? Neuro chemicals like serotonin, adrenaline and nor adrenaline and recently GABA have been implicated to be on a lower levels in brain which precipitate these symptoms. And most important part you are not at any fault in this low levels of chemicals. It is an individual vulnerability that differs and some people are more analytical or worrisome since childhood, harsh parenting or over pampering, high expectations, more body symptom concern, etc. are few factors which affect this vulnerability. 

What to do if I have panic? 

At 1st, panic disorder is NOT a serious psychiatric disorder. Yes, it is true. Websites may mention its catastrophic and worst prognosis, but it is not like that. Panic works on anxiety and fear cycle. This gets cleared off when treated in time. If you have panic, do not do anything. Sit and take deep breaths. Try to have a glass of water and relax. Most people become restless in order to get an immediate cure of panic symptoms. That is again a faulty behaviour. Going to doctors and emergency again and again will increase the chances of panic episodes.

Secondly, your mind needs to get a new form of learning everytime you have a panic. So be patient and let your mind know that this is anxiety and nothing life threatening. This will require two things - courage and patience.

Lastly, during panic episodes, do not take any decisions about your life or do not avoid any situations. This will aggravate the fear behind the episodes.

When to seek a Psychiatrist? 

When panic lasts for 30 days or more, affects your work and relationship, affects your concentration and memory; when your efforts fail to handle panic; when all the reports are normal and when you have some other symptoms with panic like depression, anger, addictions, medical conditions, etc. 

A psychiatrist can help you in both ways. Starting medications for quick relief of symptoms and if you and your psychiatrist feel then CBT- Cognitive Behaviour Therapy or other psychotherapy. 

Panic disorder is treatable. Panic disorder is non-serious. Panic disorder needs your patience and courage.

Obsessive-Compulsive Behaviour: Habits are not always normal?

Obsessive compulsive behaviour is form of anxiety disorder in which unreasonable thoughts and fears, which are obsessions that lead one to do repetitive compulsive behaviour. Root causes of obsessive compulsiveness are complex and often deep seated. The underlying emotional states may include the fear of unknown, not being in control, negative outcomes, failure, rejection, shame, annihilation or embarrassment

Signs and symptoms of OCD
Most people with obsessive-compulsive disorder have both obsessions and compulsions, but some people experience just one or the other.

Obsessive thoughts:

Common obsessive thoughts in OCD include:

  • Fear of being contaminated by germs or dirt or contaminating others
  • Fear of losing control and harming yourself or others
  • Intrusive sexually explicit or violent thoughts and images
  • Excessive focus on religious or moral ideas
  • Fear of losing or not having things you might need
  • Order and symmetry: the idea that everything must line up “just right”
  • Superstitions; excessive attention to something considered lucky or unlucky

Compulsive behaviors:

Common compulsive behaviors in OCD include:

  • Excessive double-checking of things, such as locks, appliances, and switches
  • Repeatedly checking in on loved ones to make sure they’re safe
  • Counting, tapping, repeating certain words, or doing other senseless things to reduce anxiety
  • Spending a lot of time washing or cleaning
  • Ordering or arranging things “just so”
  • Praying excessively or engaging in rituals triggered by religious fear

You can eliminate mild obsessive compulsion easily, whereas, severe obsessive compulsiveness, require support of medical and mental health professionals.

When you start pondering or obsessing over an activity, you must try to stop the negative pattern, and provide control and security. A good anchor code stops an obsessive thought pattern and provides fact-based security for new action. One can get it under control and recover from it, however at the present, there is no cure. It is a potential that will always be there in the background, even when one's life is no longer affected by it.

The treatment for obsessive compulsive behaviour depends on the how much the condition is affecting the daily life of the person. There are two main treatments, firstly Cognitive-behavioural therapy, which involves graded exposure and response prevention and the second being medication. The cognitive behaviour therapy emboldens one to face one's fear and let the obsessive thoughts occur without neutralising them with compulsions. In second case, treatment is by medication to control one's symptoms by altering the balance of chemicals in brain.

Obsessive compulsive behaviour that has a relatively minor impact on one's daily life is usually treated with a short course of cognitive behavioural therapy. Cognitive behavioural therapy involves exposure and response prevention that is used to help people with all severities. People with mild to moderate behaviour usually need about 10 hours of therapist treatment, combined with exercises to be done at home between sessions. Those with moderate to severe symptoms may need a more intensive course of therapy that lasts longer than 10 hours.

During the sessions, one works with therapist to break down the problems into their separate parts like physical feelings, thoughts and actions. The therapist encourages you to face your fear and let the obsessive thoughts occur without putting them right. It requires motivation and one should start with situations that cause the least anxiety first. These exposure exercises need to take place many times a day, and need to be done for one to two hours without engaging in compulsions to undo them. People with Obsessive compulsive behaviour find that when they confront their anxiety without carrying the compulsion, the anxiety goes away. Each time, the chances of anxiety reduces and last for a shorter period of time. 

Once you have one exposure task, you can move on to a more difficult task, until you have overcome all of the situations that make you anxious.  It is important to remember it can take several months before a treatment has a noticeable effect. It is extremely vital to remember that no one is perfect, nor can anyone recover perfectly.  Even in well maintained recoveries, people can occasionally mess up and forget what they are supposed to be doing. If you wish to discuss about any specific problem, you can consult a psychiatrist.

Dear Dr. , My son 12 year old in 7th standard was suffering from Epilepsy in his early years. After completing his course of 4 years of Epilepsy , he is absolutely fine from last 7 years after leaving the medicine. At present he is complaining of severe headache from last 3 or 4 days. We have consulted MBBS Doctor and he has given some medicine for this temporary phase. My question is that " Is this headache is related to that Epilepsy " What we should do. EEG is required or this may be another reason. Regards,

Hi there, Headache is something caused by over 30 reasons ranging from diminution of vision or ear related problem to simple stress.... The list may include a form of epilepsy but seems unlikely unless we can rule out more obvious and common causes for this problem.. I believe EEG is not the investigation needed at this point.... Rather going for eye testing be the first step in this.... More over a detailed history is very essential in ruling out most of the likely causes... Feel free to connect further... Plz take care.

Hi there..... The probability of transmission is not ruled out by changing a torn condom.... For confirmation and peace of ur mind.... Get urself tested for not just HIV but all STD.... And avoid unsafe or risky sexual behaviours
