Hypopituitarism is defined as a condition in which the pituitary gland is unable to produce enough hormones. Under-secretion of pituitary hormones affects reproduction, body growth and blood pressure. It also results in pre-mature aging.

The pituitary gland is a small bean-shaped gland situated at the base of your brain, behind your nose and between your ears. Despite its size, this gland secretes hormones that influence nearly every part of your body.

In hypopituitarism, you have a short supply of one or more of these pituitary hormones. This deficiency can affect any number of your body's routine functions, such as growth, blood pressure and reproduction.

Few Causes:

Head injury

Tuberculosis

Radiation treatment

Stroke

Brain tumors

Brain surgery

Meningitis (brain infection)

Hypophysitis (autoimmune inflammatory disorder)

Sarcoidosis is an infiltrative (accumulation of cells and tissues in excess of the normal amount) disease which occurs in various organs.

Impaired hormone secretion by the pituitary gland is caused by genetic mutations.

Sheehan s syndrome: It is a condition in which the pituitary gland gets damaged due to severe blood loss during childbirth.

Hypothalamus (a part of the brain which is located above the pituitary gland) diseases wherein the pituitary activities are directly affected by the hypothalamus.

Symptoms:

Fatigue

Reduced sex drive

Decreased appetite

Anemia

Irregular menstruation, pubic hair loss, hot flashes (feverish heat), and inability to breast-feed properly because of low breast milk production).

Dwarfism in children

Weight loss

Increased sensitivity to cold temperature

Infertility

Facial puffiness

Reduction in body hair or facial hair

Treatment Options:

Hormone replacement drugs include:

Corticosteroids: Adrenocorticotropic hormone deficiency leads to low production of adrenal hormones (different kinds of hormones such as cortisol and adrenalin, produced in the endocrine glands). These drugs act as a replacement of adrenal hormones.

Growth hormone: Growth hormone, also known as somatropin, is injected beneath the skin. This treatment method promotes growth, which balances the height in children. Growth hormone deficiency in adults is also cured by these medications to some extent.

Surgery: Surgical procedure is needed if the condition of hypopituitarism is the result of a tumor. The doctor will remove the tumor through surgery, thus resulting in normal growth.

Radiation: Radiation therapy helps in reducing the size of the tumor which causes stunted growth or pre-mature aging.

Hyperthyroidism is a state of excess thyroid hormone production Pt has typical symptoms like profuse sweating ,palpitation, wt loss despite good appetite, increased bowel frequency, goitre and bulging eyes often. If one is suspected this condition they shd get a thyroid blood test ie. T4 andTSH done.

Sir/mam I am suffering with thyroid and bp main 4-5 month month se tablets le rhe hu mera bp to ab control me hai but mera thyroid (tsh) 4 month phele 9.3 ml tha or ab 13. 2 ml ho gya hai main thyrox 25 tablets le rhe hu but mera thyroid theek nhi hua balke bad gya hai. Mujhe kya karna chahiye ke mera thyroid jalde se jalde theek ho jaye.

First see that you are regular with medication and was taking it empty stomach keeping 45 minutes gap between thyroxine and breakfast. If everything was correct even then Tsh increased on thyroxine 25 ug, then increase dose to 50 ug daily especially if married and planning pregnancy.

No sensible doctors do that ;as artificial sweeteners increases insulin resistance it might worsen your glycemic control too. So regular use never endorsed. There is immense data on this. And a recent study published in nature on sweeteners whether caloric or noncaloric should not routinely be taken .it also alters gut microbiome (gud bacteria replaced with bad ones)

Respected doc. I am obese, and in April, I miss my cycle too. Doc prescribed me medroxyprogesterone acetate and it bleeds on 10 may to 15 may. Now, I do jogging, walking, running squatting regularly about 30 mins. I avoid junk food, soft drinks, sugars, chips, I eat only home food. I studied about thyroid effects on menses. Pls prescribe me way how to keep healthy thyroid. Do I avoid rice and potato too. Thankyou.

Firstly get urself checked for thyroid. Get t4 and tsh. It s blood test. If it deranged then medicine is started. If it is already normal you can relax. But there is nothing one could do to prevent it from not having in future.

She has diabetes nd she even takes medicine nd rarely she eats any sweet thng or sugar thn why it is so tht he blood sugar level is high like 215 or 169 nd so. Also he blood pressure goes down to 170.

Sugar control depends on optimum treatment and strict adherence to diabetic diet and regular walk. If anything not optimum outof above 3 things you will not achieve gud sugar control. Also regular follow up with your doctor with advised sugar reports mandatory for treatment revision. As diabetes advances in time your treatment need be adequate and may require optimisation.

Enough or not will depend on your sugar targets acheived or not, meaning your hba1c less than 7%and fasting and pp blood sugar within control.

This need to be evaluated. It appears you had some boil which has probably led to localised cellulitis. And cellulitis is basically infection which requires antibiotics. So show a physician for this.

I was on 50 mcg Thyronorm for 3 months and my current TSH is 6.94. It was 0.01 three months ago when I was on a 75 mcg course. Now what dosage should I switch to at 6.94? Back to 75 mcg? Or some other dosage pattern?

Dose depends on body weight and tsh depends on compliance to drug and correct dose. You should continue as thyronorm 50 ug daily monday to friday and 100 ug on saturday and sundays. But take medicines regularly empty stomach maintain 45 minutes gap between medicine and breakfast.

Do regular walk 45 minutes and follow 1400kcal diet chart for which consult dietitian. Your tsh seems normal so this cannot be held responsible for your weight gain. Its lack of exercise and probably dietary issues.

My tsh is increasing. I firstly tested my tsh it was 5.4. T3t4 was normal. Then started to take 50 mcg. Done test after 1 month. Tsh was 1.88. Then after 3 month. Tsh was. 2.99. Then after 6 month. The tsh has raised. It was 5.77. Why its happening.

If your first tsh was less than 10 uiu/ml you should not have started medicine altogether. At first report if tsh was 5.4 treatment was not required except in patients who are pregnant or infertility, galactorrhea with raised prl. Which was not your case. And for obesity thyroid replacement is not started. Once treatment is started your normal gland becomes lazy and if you miss few tablets or don't take it empty stomach or take anti acidity tablets, then these variable values of tsh comes. I would suggest you stop thyroxine and repeat tsh only after 3 months and if it remains less than 10 don't restart thyroxine. 15% population have tsh upto 7-8. No need of treatment in them ok.

Meri mother 70 years ki hai sugar test me 260 hai aur right leg me swelling bohot rehti aur left side hips me dard rehta aur tablets bhi khate ecosprin - Av, glenmark & insulin bhi lete & us insulin ka name biphasic isophane hai & abiek tablet ka name K - glim hai. Hypo thyroid TT3 / 84.26, TT4 / 7.45, TSH / 47.29 itna hai report date 09-04-2016. Haemoglobin 11.2, red blood cells 3.9, total white blood cells 6.900. Insulin morning ko 20 lete aur night ko 15 lete. Aur saans bhi bohot phoolti hai please kuch suggestions dijiye.

What is her hba1c level (average blood glucose test done 3 monthly) all diabetics should get it done every 3-4 months to keep an eye on their sugar control. Sugar 260 is whether fasting or pp or random have different meaning. You need to show your mom to a good specialist. As she has to follow strict diabetic diet from diet chart. Plus regular walk 30- 45 i'm daily. Plus correct insulin injection technique. Also if tsh is 47 then she need to be on thyroxine replacement. I don't know whether she is receiving that or not. Please see nice endocrinologist in your town for her problems.

If hyper thyroidism detected in first 3 months of pregnancy the first choice is ptu but subsequent 6 months pregnancy nmz (neomercazole) can be safely taken under close supervision of doctor as frequent 4-6 weekly thyroid testing needed so auto avoid under or overdosing.

See a dietitian for diet chart and do regular brisk walk 45 min /day. Or any other physical activity.

If this is reported first time and already not on treatment. And you are symptomatic -have palpitation, wt loss tremors. We need to rule out thyroiditis versus hyperthyroidism. Hyperthyroidism requires treatment while thyroiditis self recovers. So please get raiu (radioactive iodine uptake) scan 2 and 24 hrs done. If multi nodular goitre, also get 99mtc scan done.

Ones ideal weight is ht- 100 cm. So if you are 170 cm for example then ideal wt would be 70 kg. Losing wt is always difficult. Hypothyroidism may be responsible for 3-4 kgs but rest is due to usually sedentary life and eating more calories than burning. So please see a dietitian and do walking 30-45 min daily. If can't walk d/t arthritis, you can adopt swimming/ walking in pool. Any how burn calories.

Thyroxine does not interfere with being pregnant. Only caution is - thyroid profile should be within normal range during pregnancy to avoid fatal and maternal consequences. So more frequent testing and thyroxine titration required.

Your age is 13 years. And if you have diabetes. Then you need to be evaluated as in children type 1 dm is commonly seen and in this only insulin is prescribed while in type 2 tablets can be prescribed. Diabetic diet and regular walk is to be followed by all type of diabetics. Please see some nice endocrinologist or physician in your are area who can evaluate you and follow you up closely. As treatment will depend on ht, wt and multiple blood glucose values and tests.
