How Knee Replacement Procedure Is Performed?

Knee replacement is a procedure where the weight carrying surface of knee joint is replaced surgically to ease the pain or any disability. People suffering from osteoarthritis, rheumatoid arthritis or psoriatic arthritis undergo knee replacement. All these conditions revolve around stiffness and painful knee. This surgery is usually performed on people aged over 50.

Surgery Types

Knee replacement is mainly of two main types:

  1. Total knee replacement where both the sides of knee joints are replaced
  2. Partial knee replacement where only single side of the joint is replaced

Procedure

In case of partial knee replacement with minimal invasion, a smaller incision, which is 3 to 5 inches, is required. This leads to minimal tissue damage, and the surgeon can work between the fibres of the quadriceps muscles. Here, an incision through the tendon is not required. This may result in less pain, recovery time is reduced, and motion is better as scar tissue formation is less.

In total knee replacement, four steps are performed:

  1. Removal of damaged cartilage surfaces, which is at the ends of the femur and tibia, with a small quantity of underlying bone.
  2. Replacement with metal components, which help as a recreated surface of the joint
  3. Incision of knee cap with a resurface made of a plastic button, which is optional based on the case
  4. Insertion of a medical-grade plastic spacer amid the metal components. This creates an effortless gliding surface.

After general or spinal anesthesia, an incision of 8-12inches is made in the front part of the knee. The joint part which is damaged is removed from the surface of the bones. The surfaces are then formed in a way to hold a metal or plastic artificial joint. The thigh bone shin, as well as knee cap, is attached to the artificial joint with either cement or a special material.

After Effects of the Procedure

After the surgery, patients may stay in a hospital for three to five days. Post surgery, notable improvement can be seen after a month or later. The patient is gradually relieved from pain with the construction of new gliding surface during surgery.

There will be slow progress in the movement. In the beginning, one may walk with a support of parallel bars and then with the help of crutches, walker, or cane. After full recovery in about six weeks, people can enjoy normal activities except running or jumping.

Presently, over 90% of total knee replacements function well for 20 to 22 years of surgery. Hence, knee problem is no problem at all!

The Rehabilitation After Total Knee Replacement: 

  1. Full weight-bearing walking, knee bending, exercise next day after surgery 
  2. Commode use, sitting on chair 2nd day 
  3. 3rd day: Stair climbing, start cross leg sitting 
  4. 2 weeks: Suture removal and walking to outdoors

Patients whose knee joints have been damaged by either trauma or progressive arthritis should consider total knee replacement surgery. The post-surgical hospital stay after knee joint replacement is usually three to five days. The surgery has a very high success rate and shows dramatic improvement after a month. This improvement is most notable one month or more after surgery. The pain caused by the damaged joint is reduced significantly when the new gliding surface is constructed during surgery. Initially, the patient will walk with the help of a walking aid until the knee is able to support full body weight. After six weeks, the patient can walk comfortably with minimal assistance. Patients with artificial joints are prescribed to take antibiotics during the course of any elective invasive procedures including dental work. Physiotherapy is an essential part of rehabilitation and it will increase the muscle strength and patient can enjoy most activities, except running and jumping. If you wish to discuss about any specific problem, you can consult a Orthopedist.

4 Causes Of Anterior Cruciate Ligament Tear And Its Treatment!

The Anterior Cruciate Ligament or ACL tear is a very common problem with athletes and with people who have an active lifestyle. In fact, this can also be caused by any accident due to sudden movement.

Importance of ACL-

The ACL is the supporting ligament, which runs diagonally from back to front right in the middle of the knee, connecting the femur bone and the tibia, and helps the tibia not to come before the femur. Thus the balance of the knee during back and forth movements and the entire stability of the knee joint is secured by this very important piece of ligament.

What leads to an injury to the ACL?

Here are a few actions that may result in a ligament tear.

  1. Sudden stopping while running or slowing down from a fast speed

  2. Sudden change in direction

  3. Uncontrolled jumping from a height

  4. Direct hitting with an object or person

What happens when the ACL is torn?

If the ACL is torn, the stability of your knee is gone, and you feel as if you have no control over the joint. Moreover, it causes extreme pain and instant swelling. Swelling usually appears within 24 hours of the injury.

Treatment of ACL Injury-

Recovery from an ACL tear is possible only after a surgery. Without surgery, the affected will not be able to return to sports or athletics, or may not be able to run and make swift movements again as before. However, to avoid surgery, there are non-invasive treatments for the aged and people who require moving less.

  • Non-invasive Treatment: Bracing is one treatment where braces are given to support the knee joint during movement and also to avoid further injury. This is effective for the elderly who need minimum movement and may manage daily activities with a braced knee. Also, crutches add to the support and let the weaker knee get rest. This is followed by physical therapy. But the swelling has to reduce before starting it. Many exercises and controlled movements over a long time help in getting back agility in movement gradually.

  • Surgical Treatment: Once the swelling subsides, doctors would plan for an Arthroscopic (Key-Hole Surgery) ACL reconstruction. In this Doctor make a graft of a tendon. This graft is inserted into the joint and fixed in anatomical position in femur and tibia Arthroscopically so that the newly created ACL functions like the natural one. This is the only way to get back the ligament in whole as the torn ligament cannot be stitched. Surgery is done through arthroscope and recovery time is 15 days for resuming day to day activities and 3 to 6 month for resuming sports activities with continued physical therapy after the surgery. My mother (age 51, weight 70) has Rheumatoid Arthritis and she recently (12/6/2017) undergone for TKR of right knee. She has CRP =40, ESR =100 and anti CCP = >200. As ashwagandha is known to reduce the inflammation and can help in reducing pain. Can I give Ashwagandha powder or capsule to my mother after surgery with other medicines like cetil 500, gabapentin, ultracet, metformin 500, stamlo 5 mg, calcium and iron tablets.

It could be that your knee must have been injured during the workout. Kindly apply ice pack for 2 3 days if it does not subside you need to get yourself assessed for any ligament sprain.

Rheumatoid Arthritis - 3 Symptoms Of It!

Rheumatoid arthritis is a chronic disorder in which the small joints of the hands and feet of the affected person get inflamed. It affects the lining of the joints and causes pain and swelling that may lead to joint deformity and erosion if left untreated.

This disorder of the autoimmunological system occurs when the body's immune system attacks the body's own tissues.

Although this medical condition usually causes joint problems only, sometimes it may go on to affect other body organs like skin, lungs, eyes and blood vessels as well.

Some of the signs and symptoms that are indicative of rheumatoid arthritis are:

1. Problems in the joints

One of the most common symptoms of rheumatoid arthritis is swelling of the joints that become tender and painful. During the early days, only the smaller joints are affected, especially the finger and toe joints. However as the disease worsens, the symptoms spread to other body parts like knees, wrists, elbows, ankles etc. Joint redness and warmth are also characteristic features of rheumatoid arthritis. It is also recognized by joint deformity and stiff joints.

2. Fever, fatigue, and loss of weight

Fatigue is a common symptom of rheumatoid arthritis. It may be a result of the body's reaction to insomniaanemia, and inflammation. It may also be caused due to loss of appetite and the consequent and unintended weight loss. Sometimes rheumatoid arthritis is also accompanied by fever.

3. Rheumatoid nodules

Another symptom of rheumatoid arthritis is the presence of firm bumps of tissues under the skin on the arms. These bumps are known as rheumatoid nodules and may be painful. The nodules may come and go depending on the severity of the disease at a particular time. Hi, I'm 55 years female from kolkata. I'm a diabetic (take Glycomet GP1 FORTE (GLIMEPIRIDE 1 mg + METFORMIN 1000 mg) twice & 10 iu insulin overnight ,fasting 80, PP 254 measured yesterday) & pancreatic (taking Panlipase 25000 after food thrice every day for last 5 years) patient. For the last 1 year, I'm feeling pain in fingers of both hands and in shoulder joints. Pain is unbearable at times. In afternoon often we see middle fingers of the hands get deformed. Visited a doctor. And he told this is a rheumatic arthritis. He gave Wysolone 10 mg for 45 days followed by wysolone 5 mg for 30 days. HCQ (Hydroxychloroquine) 300 mg, ARAVA (Leflunomide) 20 mg, INJ. Folitrax 20 mg once on sundays. Folvite 5 mg mondays to saturdays. Wysolone is now completely stopped. When I take wysolone there is no pain in fingers. Here is the test reports at a glance, Uric Acid 3.69 (range 2.6-6.0), RF 7.15 (<14), CRP 3.02 (<5) vit D (25 oh D) 36.12 (30 -100. AntiCCP Anti Body 5.61 (<20). CBC, Haemoglobin 11.8 gm/dl (range 12.5 - 15.3), MCHC (Mean corpuscular Hemoglobin concentration) 32.9 (range 33.4-35.5),Total RBC 4.0 million cells/cu mm (range 4.5-5.1), lymphocytes 52% (range 20-40) basophil 00 (range 01-02), ESR 16 (range <30) SGPT 13.8 U/L (range <35, Lipase 32.5 U/L (range 30-300), CRP 1.20 (range <10), amylase 78.2 (range 30 -110) Test was done in SERUM, Kolkata in 15th june 2017. Many doctors are telling me that this is rheumatic arthritis, But after 6 months of taking medicines for the same pain the still there, only fingers does not get deformed in afternoon. Please tell me is the diagnosis is right? Am I on right treatment? Or there is any other problem? Is there any bad action for taking panlipase 25000 for last 5 years? One more question for RA CRP & RF should be positive, but for my case it is negative, so how can it be RA?

RA is a autoimmune disease which is related to diet which needs to be radically changed for reversal of disease. Rheumatologist Drs in India only know to give pills for any illness .They do not know that there is natural treatment available because nutrition is not taught in the medical curriculum. They do not think outside the box. Change your diet and reduce the medicines. I think you were getting too many drugs for RA .Were they all given at the same time or at different periods. What was the dietary advise given by your Dr. for RA A detailed history is needed to advise you. Take a consult. Remember you cannot fix your health without fixing your diet.

Hello ,as you have mentioned you had a fracture in proximal bone of tibia ,I would suggest to strengthen your thigh and leg muscles. Because cramps sometimes occurs due to weakness in muscles as due to fracture and immbilisation in plaster cast muscles get weak and may be stiffness in joints so I would suggest to go to a physiotherapy doctor first of all. Learn some exercises and proper rehabilitate your muscles, thank you.

Hii. Need more information like from when do you have this. It can be of various reasons. Uneven growth, cartilage defect etc. An orthopedician can tell the exact cause. 82 Kgs is not too much. A good regimen of walking 1 hour a day and diet control will help reduce weight. The only way to save the knees is through exercise. See a physio and start Quads strengthening exercises for the knee. Ask mum to avoid squats and reduce stair climbing.

Am suffering from Lt knee Lock since last 2 months MRI taken on May 12 revealed there is Multiple Ligaments problem of which 2 Partial Tear & 2 sprained (Tear in ACL & LCL) presently wearing Hinges Brace & undergoing physiotherapy but it started paining while walking & lting on bed keeping leg straight. kindly suggest d how it will get unlocked.

Locked knee since 2 month is a serious problem. It's most likely the torn meniscus which has caused the locking. Unfortunately there is no medicines which can heal the torn meniscus. I would strongly recommend that you undergo arthroscopy meniscectomy. I would not be much worried about ligaments at your age, but locked Knee if it remains locked will quickly cause degenerative arthritis in the joint.

Avoid sitting Cross legged. Avoid Squatting- Quadriceps Exercises- Lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core Strengthening Exercise- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Hams Stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. repeat 10 times, twice a day. Sports Taping- stretch the tape from both ends and apply on the affected area Contrast Fomentation (Hot and Cold).
