I am Dr. Indu Taneja working as a senior consultant in Fortis Escort Hospital Faridabad. My special interests are high risk obstetrics and laparoscopic surgery. Today I will talk about contraception little about the normal menstrual cycle.

A normal cycle is of approximately 28 days so the ovulation means the egg formation occurs in the middle of the cycle that is approximately 14 days but this day is not fixed it can be one or two days prior or one or two days later. So let’s say if it happens on 14th day four or five days before the egg formation and four or five days after the egg formation is the period which is the danger zone that means the pregnancy can happen in this period if you have intercourse during this period so this is the danger zone. So if one happens to have the relation in this period one can use emergency contraception which is you must be all aware of pill 72 or unwanted pill which is to be taken 72 hours within this period the pregnancy is to be avoided.

Next will be other contraception like the barrier method which is normally condom. This is not the foolproof method. This has the maximum failure rate approximately 35 percent. This is mainly effective in the period when the patient is breastfeeding. At this period this is the common effective method. Then there are the other methods you have heard about the pills and the Copper T. The pills are the second commonest method in which every day one has to take the pill for 21 days. The main drawback of the pill is you have to remember you have to take it on a fixed time and fixed day. You should not miss even a single dose. If one misses a dose you can have breakthrough bleeding and the failure also happens. So this is the main drawback of the pills but otherwise the safety profile is very good.

Other method is the IUCD that is the Copper T which is inserted inside the uterus. This has again the same efficacy as the pills but the advantage over the pills is you are not supposed to remember this. This once inserted can be kept and can be used for three years to five years. Government has also provided the intra uterine device which is effective for five years it is as effective as the contraceptive pills but little drawback of this is sometime this can cause irregular bleeding or the heavy bleeding during the periods which can be taken care of giving some kind of medications.

The most common cause of this disease is the change in lifestyle of the young generation. Getting your periods for the very first time can be daunting. Especially if you have never been educated about it and you are far away from home and you have no clue about what is going on. The overbearing pain from stomach cramping does not make it any better. It is always good to be on the safe side and keep yourself educated about it.

What is your first period?
Periods are a very natural part of your life as a woman. It happens when the body of a girl undergoes changes and starts maturing into a woman. It happens when blood is expelled out of your uterus to set up your body for maturation and pregnancy. Since your body is transforming, you can get pregnant (and just before your first period as well).

Your period happens once per month and as a rule, lasts between 2-7 days, however, it might take a couple of months for your period to be consistent; the initial few might be light and sporadic. Trichomoniasis is an infection that is caused by a parasite and is transmitted sexually. This disorder is characterized by symptoms of itching in the genitalia, smelly discharge from the vagina and pain while urinating. This disorder may also result in premature birth in case of pregnant women.

The symptoms of this disorder are:

1. Redness around the genitals, which may cause intense itching

It is important to remember that the first trimester of your pregnancy is a crucial phase where you may not change that much physically, but will be prone to many emotional changes. Pregnancy and childbirth is one of the most important time in the life of a woman. It is a time to celebrate and yet be cautious as well, so that all goes well and the end of the pregnancy is a beginning of life for a baby. There are numerous ways of monitoring the health of the foetus during pregnancy. The non-stress test is one of the ways. Let us find out more about this: 

Heart rate monitoring: The non-stress test is basically concerned with the monitoring of the heart rate of the foetus. This is a prenatal test that is very commonly used in order to check the overall health and vitals of the baby while it is still in the womb. The test is essentially carried out to check how well the heart rate responds to the other movements carried out by the baby inside the womb. It is called a non-stress test because there is no external stimulation that may put pressure on either the heart rate or the movements of the baby.

Why: This test is usually recommended by the doctor when there is an increased risk of losing the foetus due to complications. This test is usually carried out when the pregnancy is in 26th to 28th week. This test will usually show whether or not the baby needs extra test, monitoring or care. This test is essentially conducted to check the oxygen supply and is a non-invasive procedure that does not pose any serious physical risks or harm.

Fetal Hypoxia: This test will also show whether or not the baby is suffering from fetal hypoxia, which is a condition where the heart beat is disrupted, if the passage of oxygen is blocked due to some reason.

Conditions for non-stress test: The non-stress test may be conducted if the lady in question has lost or had complication in previous pregnancy. Also, if the mother has any other medical condition like diabetes or cardiovascular conditions, then this test is imperative. Decreased movement of the foetus may also prompt the doctors to take action and conduct this test as a cautionary measure.

Procedure: During the procedure, you will be asked to lie down on a reclining chair and two belts with monitors will be attached to your abdomen. The heart rate of the baby, as well as the movements, will then be studied once the monitors are switched on. These monitors are in turn attached to a screen, in order to take the reading. This test will take no more than twenty minutes. However, if there is no movement in the baby, then the test may continue for a little longer until the baby becomes more active.

7 Benefits Of Kegel Exercises During Pregnancy!

Kegel exercises during pregnancy

Strong pelvic muscles are required to have a healthy pregnancy. These exercises are useful in preparing the muscles for labour and childbirth. Regularly exercising can help prevent complications occurring due to pregnancy.

How To Do Kegel Exercise During Pregnancy?

Try to stop the flow of urine when you are sitting on the toilet without tightening your abdominal, buttock, or thigh muscles. When you're able to successfully start and stop urinating, or you feel the vaginal muscle contract, you are using your pelvic floor muscle, the muscle you should be contracting during Kegel exercises.

You can do Kegel exercises two ways: either by holding or quickly contracting the pelvic floor muscle. To do slow Kegels, contract the pelvic floor muscle and hold for three to 10 seconds. Then relax and repeat up to 10 times. To do fast Kegels, quickly contract and relax your pelvic floor muscle 25 to 50 times. Relax for 5 seconds and repeat the set up to four times.

Squatting: Squatting is helpful during labor because it opens the pelvic outlet an extra quarter to half inch, allowing more room for the baby to descend. But squatting is tiring, so you should practice it frequently during pregnancy to strengthen the muscles needed.

How to do Squatting? An exercise called a wall slide is especially helpful. Stand with your back straight against a wall, place your feet shoulder width apart and about six inches from the wall, and keep your arms relaxed at your sides. Slowly and gently slide down the wall to a squatting position (keeping your back straight) until your thighs are parallel to the floor. Hold the position for five to 10 seconds, slowly slide back to a standing position. Repeat five or 10 times.

Pelvic Tilt: What is it? Pelvic tilts strengthen abdominal muscles, help relieve backache during pregnancy and labor, and ease delivery. This exercise can also improve the flexibility of your back, and ward off back pain.

How to do Pelvic Tilt? You can do pelvic tilts in various positions, but down on your hands and knees is the easiest way to learn it. Get comfortabe on your hands and knees, keeping your head in line with your back. Pull in your stomach and arch your back upward. Hold this position for several seconds. Then relax your stomach and back, keeping your back flat and not allowing your stomach to sag. Repeat this exercise three to five times. Gradually work your way up to 10 repetitions.

These exercises can yield great benefits with minimal effort. The exercises require no special equipment except comfortable clothes, and a little space to do them.

Do not worry if you are not close to the goal when you begin. Pelvic muscles are like any other muscles in your body. They will become stonger only with time, consistency and mindful work.

Beware: If you sense any pain in your back or abdomen after doing a Kegel set, it is a sign of you not doing them properly.

What Should You Know About Intrauterine Devices?

Intrauterine devices or IUD are T shaped contraceptive devices. These contain levonorgestrel or copper and are inserted into the uterus. IUDs are considered to be one of the most effective birth control measures and also they are reversible (meaning it can be removed whenever a need arises). The copper IUD has a failure rate of 0.8%, while the levonorgestrel IUD has only had 0.2% failures till now. As per a report in 2007, IUDs are used by more than 180 million users all over the world.

What you should know about Intrauterine devices:

  1. Types of IUDs: There are 2 types of IUDs Copper and Hormone. Copper IUD is considered to be the conventional form of the contraceptive. It is T shaped with strings and is inserted into the uterus by a doctor. It can remain in the body for three to five years. Minera is a known brand of the hormone IUD. It contains a low dose of hormone progestin and releases the same when inserted in the uterus. This is also T shaped and has strings. This can be left for three years.
  2. Pelvic infections are not caused by IUDs: There is a belief that IUDs cause pelvic infection but this is not true. The strings of the new IUDs are very thin and they do not cause any pelvic infection even if a person is suffering from a sexually transmitted infection (STI). Though there might be a slight risk of infection when the IUD is inserted and in the first 20 days, but that also dissolves soon.
  3. IUD can be used at any age: Earlier IUDs were not recommended for women who had never got pregnant. However, now anyone can use these irrespective of being pregnant or not. It is very safe for all age groups from teenagers to women involved in active sex to women post menopause.
  4. IUD is as effective as getting a tubal ligation: It is believed that Minera is 99.4% effective while sterilization (tubal ligation) is 99% effective in preventing a pregnancy. Ligation is considered to be a permanent birth control solution, while IUDs can be reversed.
  5. IUDs do not cause infertility: It was believed that a STI would creep into the pelvis through the string of the IUD and cause infertility but this is not so. Though a STI can cause infertility but the IUD has no role in the same and does not contribute or worsen the infection.
  6. Hormonal IUD have less side effects than birth control pills: Breast tenderness, acne, and bleeding for a short time may persist for the initial 3-4 months in case of hormonal IUDs. The copper IUD lead to cramping and heavy flow during menstruation.
