MBBS, MS - Obstetrics and Gynaecology

Polycystic ovary disorder (PCOD)

PCOD (Polycystic Ovary Disorder) is an issue in which a woman’s hormones are out of control. It can bring about issues with your periods and make it hard to get pregnant. PCOD or PCOS additionally may bring about undesirable changes in the way you look. In case that it is not dealt with, after some time it can cause serious medical issues, for example, diabetes and heart diseases. In PCOD, the hormones change rapidly.

The reason for PCOD is not completely seen, but rather hereditary qualities might be a reason. PCOD appears to keep running in families, so your possibility of having it is higher if other women in your family have it or have diabetes. PCOD can be passed down from either of your families’ side.

The most widely recognized clinical features are:

  1. Skin inflammation.
  2. Weight gain and problem getting thinner.
  3. Additional hair on the face and body. Frequently women get thicker and darker facial hair and more hair on the abdomen, stomach, and back.
  4. Diminishing hair on the scalp.
  5. Unpredictable periods. Frequently women with PCOD have less than nine periods a year. A few women have no periods. Others go through extreme bleeding.
  6. Infertility issues. Many women who have PCOD experience difficulty getting pregnant.

Some of the most effective and long-term treatments include:

  1. Normal work out and weight control are the key medicines for PCOD. Treatment can lessen uncontrollable symptoms and stop the long-term medical issues that come along with it.
  2. Try to fit in moderate movements and additionally outdoor activities regularly. Taking a walk is a great exercise that a great many people can do.
  3. Eating healthy foods is very important. This includes heaps of vegetables, nuts, beans, and whole grains. It limits foods that are high in soaked fat, for example, meats and cheeses.
  4. Most women who have PCOD can benefit by shedding pounds. This can mean losing the hormones that are uncontrollable and having regular periods.
  5. In case that you smoke, consider stopping. Women who smoke have higher androgen levels that may add to PCOD symptoms.
  6. Your specialist may recommend anti-conception medication pills to decrease side effects and help you have consistent menstrual cycles or infertility medication in the case that you are experiencing difficulty getting pregnant.
  7. Aerobic exercises, jogging, walking for 1 hr. daily to be added.

It is important to consult a specialist for the follow-up to ensure that treatment is working and to change it if necessary. You may likewise require normal tests to check for diabetes, hypertension, and other conceivable issues. It might take a while for medicines to help with the symptoms, for example, facial hair or skin break out. You can use over-the-counter or physician endorsed pharmaceuticals for skin break out.

My first baby age is 1 year 3 months. Can we plan for another baby now? Note-first baby was through operation Thanks.

MD - Obstetrtics & Gynaecology, MBBS
Gynaecologist, Gurgaon
Hi lybrate-user. I would suggest you to wait and plan for next baby only once your 1st child is of 3 years. Before that if she has another pregnancy then there would be high chances of uterine rupture which can be fatal for the baby as well as the mother.

Sir what is the best procedure to avoid pregnancy I and my wife got first baby with scissoring and baby is now 45 day old we want to have 5 years gap or even may be permanent solution without any surgery please suggest thanks.

While choosing a method of contraception, it is important that it not interfere with lactation or have negative effects on the infant. Iucd or copper t is more effective than condoms and progesterone only pills. Oc pills suppresses milk production. Estrogen containing contraceptive pill either it is standard dose or low dose, it reduces the volume of breast milk. Progestin-only contraceptives like cerazette for 28 days are safe for use by breast-feeding mothers if copper t is not acceptable.

How Ayurveda can Help you with Postnatal Care

Bachelor of Ayurveda, Medicine and Surgery (BAMS)
How Ayurveda can Help you with Postnatal Care

For a mother, the first few months after delivery are crucial since the uterus after expanding due to delivery, shrinks back to its normal size. Moreover the tendons, ligaments and muscles connected to the uterus begin to regain the power and strength lost during labor. A lot of emphasis is put on postnatal care in Ayurveda.

Ayurveda suggested diet tips:
According to your digestive system, you must take Sesame oil i.e. Til oil and ghee i.e. clarified butter after mixing it with panchkola. Once this mixture has been digested, you must consume rice gruel and milk that is medicated with Vidharigandhadi gana or panchkola. Apart from these, you must consume jaggery water for about a week. After a week, you must eat foods that are easily digestible to help you regain your strength.

Do's and Don'ts:
1. Do massage yourself with Vatahara Oil
2. Do bathe with warm water
3. Do keep your perineum and breasts clean
4. Do urinate every 5 hours
5. Do breastfeed properly
6. Don't consume food irregularly
7. Don't have stale or cold food
8. Don't retain urine or feces
9. Don't indulge in sex or exercise

Ayurvedic medicines and herbs:
All the medicines and herbs mentioned below are never advised together. An ayurvedic specialist will usually give you a combination of a few medicines depending on your symptoms and condition.
1. Dashamoolarishtam: Dashamoolarishtam is extremely effective in boosting your immunity and keeping infections at bay. It is usually prescribed for the first few months as it also helps reduce inflammation and pain.
2. Jeerakarishtam: Jeerakarishtam is well known for inducing normal lactation, and is usually prescribed to mothers during the monsoon or winter seasons. This is because it keeps the body warm owing to its hot nature.
3. Shatavari Gulam: Improve lactation and achieve overall hormonal balance by consuming Shatavari Gulam.
4. Pulileham: Pulileham is administered to new mothers from the third week onwards for about a month and half.
5. Shatavari: Shatavari herb is mixed with boiling milk to improve the production of breast milk and avoid post partum depression. It is also useful for normal involution of the uterus.
6. Musta: Apart from improving breast milk production, musta herbs also relieves clogs in milk ducts and inflammation.

All About Vaginal Rashes

MBBS, Diploma in Venerology & Dermatology (DVD)

All About Vaginal Rashes

Rashes in the vulva, which might be caused due to various health problems or due to irritation on skin, usually constitute all the vaginal rashes. The vaginal rashes might be the result of non infectious conditions (it might be an allergic reaction) or it might also be the result of a sexually transmitted infection. The vulva may become red and might have sores and blisters. Many times, the vulva doesn't change color at all, but visible bumps are present in the area.

The symptoms of vaginal rashes are as follows:

  1. Blisters or sores
  2. Reddening of the vulva
  3. Visible bumps
  4. Vaginal discharge
  5. Pain in the pelvic region
  6. Painful urination with a burning feeling
  7. Itchiness
  8. Pain while having sex
  9. Lice or insects in the pubic hair
  10. Fever
  11. Enlarged lymph nodes
  12. Ulcers on the groin

Although the symptoms are not usually fatal, sometimes the conditions might become life threatening. If your body temperature rises above 101oF and if you are suffering from a severe abdominal and pelvic pain, the best option is to consult a doctor immediately.

Vaginal rashes can be due both noninfectious causes as well as due to sexually transmitted infections. Some of the infectious causes are:

  1. Genital herpes
  2. Granuloma inguinale
  3. Human papilloma virus
  4. Molluscum contagiosum
  5. Scabies
  6. Pubic lice
  7. Yeast infections
  8. Syphilis

Some of the noninfectious causes are:

  1. Allergic reactions
  2. Eczema
  3. Chemical irritants like soap, lubricants, creams, etc
  4. Lichen Simplex Chronicus
  5. Psoriasis

Although the vaginal rashes aren't very serious in nature, you might face certain complications like infertility, secondary or tertiary syphilis, and transmitting the infection to your partner.

Vaginal rashes are usually treated with home remedies. Here are a few treatments you could try out if you are afflicted with vaginal rashes:

  • Apple Cider Vinegar: This particular substance is known for having antifungal and antibacterial properties. It will help you get over the itching which takes place when you are suffering with a vaginal rash. If the rash is due to a fungal or bacterial infection, then apple cider vinegar will help to clear out the infection. Wash your vagina with apple cider vinegar and warm water mixture.
  • Anti-bacterial Talc: Anti-bacterial talc is extremely mild to the skin. It helps to cure fungal infections but this method might take some time to cure the rash.

Consult your doctor if the rash becomes serious and requires immediate attention.

I've got abnormal vaginal bleeding since last 3 days. Though it is very less in amount without any pain. I also want to mention here that my periods was just over last week. I am married and had few normal intercourses after that. But I am worried about this abnormal bleeding. Please tell me why it is happening.

MD - Obstetrtics & Gynaecology, MBBS
Once a menstrual bleeding had finished, very few women bleed again till next menses. But in some slight bleeding may occur at ovulation time. This is perhaps what you are experiencing. But without a thorough examination, one will not be able to rule out other causes, like cervical erosion, polyp or infection or even a vaginal tear. So best to go and see a gynecologist.

My mother aged 45 had a problem of heavy period bleeding after which she consulted a gynae and she gave her primolut n medicine which controlled the condition. Few days back consulted another gynaecologist. She wrote some tests such as pelvic ultrasound, pap smear, endometrial biopsy .the results of the tests came clear. Only ultrasound shown bulky uterus with small fibroids and level of prolactin serum high .after which she suggested some vitamin medicines and said to continue primolut n only from 5th day of month. She stopped taking primolut. And after 2-3 days again the problem of abdomen pain and bleeding is happening. Please help and suggest some solution immediately.

MD-Ayurveda, Bachelor of Ayurveda, Medicine and Surgery (BAMS)

Hello- These kinds of symptoms are faced by most women in their late fourties due to menopausal changes in the body physiology. Improper dietary intake and faulty schedule done up till 40 years result in complicated menopausal condition after 40. Bulky uterus and uterine fibroid suggest that the normal physiology of the uterus is affected up to a greater extent. More over hormonal imbalance in the later stage has worsen the condition of your mother. If not treated it will cause blood deficiency and the doctor will force you towards hysterectomy (removal of uterus) which will be the most wrong step taken by you up till now.Still I can assure you that menopausal menstrual complications can be cured effectively with ayurveda. Consult if interested.

You should get a basic set of investigations if you are having unprotected sexual intercourse since one year. Your age is 27 and if married for more than 1 year, a set of basic tests is no harm done.
