How to Diagnose and Manage Acute Coronary Syndrome

The word acute coronary syndrome refers to a group of symptoms that are caused by blockage of the blood flow to the heart muscles.  The most common result of this is myocardial infarction or heart attack as it is popularly called. Reduced blood flow leads to death of some portion of the heart muscle wall. While the word heart attack sounds almost fatal, it need not be the case. Knowing how to identify an attack and being aware of some simple measures can help save lives. 

Symptoms
The tell tale signs of a heart attack are as follows:

  • Chest pain and discomfort usually described as a tightness or burning in the chest region
  • Pain along the left side of the shoulder and neck, going up into the jaw, down to the arm
  • Nausea and vomiting 
  • Profuse sweating 
  • Difficulty breathing 
  • Dizzy or fuzzy feeling 
  • Tired, extreme fatigue
  • Anxious, apprehensive feeling 

However, be also aware that there are a lot of people who experience a silent heart attack. Women, obese, elderly, and diabetic patients can have silent attacks and depending on severity, either they go on with life as usual or can have a fatal attack.

Diagnosis
Once you are doubtful of a heart attack, the next step is to reach the closest medical facility for a diagnosis.  In addition to a detailed examination and history, the following two tests will be performed.

  1. Electrocardiogram (ECG): A 12-lead ECG will measure electrical activity of the heart and identify irregular electrical activity, which is indicative of a myocardial infarction.
  2. Blood tests:  Presence of certain enzymes in the blood, CK-MB and troponin are indicative of a heart attack.  A complete electrolyte profile also will be done, and increase or decrease of some electrolytes is helpful in diagnosing a heart attack.
  3. In addition to these two, chest radiography, cardiac angiography, echocardiogram, stress test, and computed coronary tomography may also be required to confirm the diagnosis.

Management
Once diagnosed, the first step would be to relieve the symptoms, negate the effects of reduced blood flow, and restore cardiac function. 

  1. Dissolve the clot: Using thrombolytics like clopidogrel 
  2. Nitroglycerin: To dilate the blood vessels and improve blood flow, especially to the heart muscles
  3. Anticoagulant therapy: Blood thinners are usually used to avoid blood clot formation; aspirin and heparin are the most commonly used agents.
  4. Drug therapy: Blood pressure maintaining drugs like beta blockers and/or angiotensin-converting enzyme (ACE) inhibitors are also used
  5. Use of Statins: Statins are used to reduce the amount of cholesterol in the blood and stabilize plaque deposits.

In very severe cases, angioplasty and stenting or coronary bypass surgery may be required. Educating people on how to identify a heart attack and manage it is very useful and can help save lives.

Stages and Symptoms of HIV Infections

HIV or the human immunodeficiency virus is the lentivirus which causes HIV infection and leads to acquired immunodeficiency syndrome or AIDS.

The symptoms of HIV vary from person to person. There are three main stages of HIV infections, where each stage has different symptoms.

Here are some facts about the three stages of HIV infections along with the accompanying symptoms.

Acute HIV Infection Stage: This is the first stage of HIV infection and after three to four weeks of becoming HIV infected, people experience symptoms, which are similar to flu. This flu usually does not last more than two weeks.

Symptoms include:

  1. Fever
  2. Rashes on the body
  3. A sore throat
  4. Swelling of different glands
  5. Headache
  6. Joint and muscle pain
  7. Improper digestion

These symptoms appear and indicate that the body is reacting to the HIV. Infected cells circulate throughout the blood, and the immune system produces HIV antibodies in order to attack the viruses. This process is termed as seroconversion, and it takes place within 45 days of getting infected. The levels of virus in your blood are quite high during this stage.

Clinical Latency Stage: This is the second stage of HIV infection, which follows the early stage. This stage is also known as chronic HIV stage. During this stage, HIV is active, but is reproduced at a very low level. People in this stage may not receive symptoms related to HIV or may get mild indications.

In case of people who do not take medicines for HIV treatment, this stage lasts for a long period. Some people, however, progress faster through this stage. Medicines should be taken to keep the virus in check. During this stage, people can transmit HIV to others very easily in spite of not experiencing any symptoms. People who are on medication stay suppressed virally and have a low level of HIV in their blood, and the risk of transmission is less.

Symptomic HIV infection or AIDS stage: This is the third stage of HIV infection, which is characterized by severe damage to the immune system of an HIV-virus affected person. A patient is likely to have serious infections and gets bacterial or fungal diseases. The infections are termed as opportunistic infections. The patient is now said to be having AIDS.

The symptoms of this stage are:

  1. Loss of weight
  2. Diarrhoea
  3. Sweating at night
  4. Fevers and persistent coughing
  5. Problems in the mouth and skin
  6. Infections on a regular basis
  7. Illness and development of other diseases.

HIV infection affects the body via three stages and leads to AIDS in the third stage. Each stage is accompanied by several symptoms.

Measure your temperature and tell me reading tell me if you are having other symptoms like cough/ throat pain/ burning urine/ stomach pain so that we can confirm cause of fever the most common symptoms of dengue fever are: 1. High fever 2. Severe headache 3. Pain behind the eyes 4. Nausea and vomiting 5. Swollen lymph node 6. Muscle and joint pain 7. Exhaustion 8. Few things related to headache so that we can plan the treatment accordingly - if pain is associated with blurred or decreased vision than check your eye sight for refractive error - if the pain is associated with cold and fever than we have to rule out sinusitis as the cause and if the pain is pulsatile in nature and with a throbbing pain usually with an aura than we have to rule out migrane as the cause - if the pain is associated with neck problems than we have to rule out cervical spondylosis as the cause - if the pain is associated with either low or high bp or due to hyperacidity than we have to treat the cause after examination.

You should go for tests stool r/m, stool c/s, Lft . blood sugar , Examination of the rectum and lower, or sigmoid, colon (sigmoidoscopy).Examination of the rectum and entire colon (colonoscopy). An X-ray of the rectum during defecation (defecography). Avoid smoking/alcohol if you take. Eat green veg. More fiber foods. Avoid fatty foods,junk foos,oily meal,avoid stress,non-veg.,. drink plenty of water.take digestive meal ,salaad,green veg,whole grain foods,oats.go for walk. You can take tab. Charcol at night. Syp. Aristozyme 10 ml thrice a day.syp. Livoluk 10 ml at night. These can help cure in constipation.

Cranial nerve palsy is one of the common culprits of causing double vision. This is all the more true for people above the age of 60. A person is more prone to cranial nerve palsy if he is suffering from high blood pressure and diabetes. This is the reason why this disease is more commonly known as diabetes palsy. Eventually, though they get better and the problem of double vision disappears.

Symptoms:

One weak cranial nerve leads to the weakness of other cranial nerves as well. In case the eyes fail to move together, chances are that the patient will have double or blurred vision. In case the sixth nerve gets affected, side to side double vision is encountered. For the 3rd and 4th nerve disturbance, there could be a vertical double vision. The lack of blood flow causes pain to the eye.

Possible Causes of Cranial Nerve Palsy?

Cranial nerve palsy can result from traumatic or congenital factors. They can also result from vascular diseases such as strokes, hypertension, diabetes, and aneurysms. It can also arise out of infections, increased intracranial pressure, migraines, and tumours. A patient's age and clinical findings help a doctor decide the right course of treatment.

Treatment:

There are no particular treatments which can speed up the recovery of cranial nerve palsy. In a case of pupil pain, doctors may prescribe anti-inflammatory drugs such as Motrin and Advil. They key to natural recovery is to ensure that the blood sugar and the blood pressure is diligently controlled. By patching the eyes, the problem of double vision can be resolved. They are typically worn over both the eyes. They are however known to slow down the process of recovery and strain the eye. There is no known exercise to fix this condition. Botulinum injections are sometimes used by doctors to straighten the eyes but the result of such a treatment greatly varies from person to person. Doctors mostly rely on watchful waiting until the phase passes within 6-8 weeks. Pain medications are recommended on a case to case basis.

Lifestyle Changes:

Managing cranial nerve palsy can be difficult for a working person. It involves many lifestyle changes till the condition fully cares. Some of the suggestions given by an ophthalmologist include resting the eye as long as one can. Reading or working on a computer should be eschewed since it puts great pressure on the eyes. The eye should not be contacted with forehands in case there is too much pain. Other basic hygiene issues include taking the medicine time to time, cleaning the eye with fresh water two to three times a day, putting the eye drops on time as prescribed by the doctor. If you wish to discuss about any specific problem, you can consult a ophthalmologist and ask a free question.

Clear nasal passages. If you have a stuffy nose, rinse sinuses with saline before bed. Using a Neti pot, nasal decongestant, or nasal strips can also help you breathe more easily while sleeping. If you have allergies, reduce dust mites and pet dander in your bedroom or use an allergy medication. Keep bedroom air moist. Dry air can irritate membranes in the nose and throat, so if swollen nasal tissues are the problem, a humidifier may help. Change your sleeping position. Elevating your head four inches may ease breathing and encourage your tongue and jaw to move forward. There are specially designed pillows available to help prevent snoring by making sure your neck muscles are not crimped. Sleep on your side instead of your back.

Uterine fibroids are referred to as benign, abnormal growths which tend to develop in the uterine walls of a woman. The size of such growths can range from a few centimeters to even excess of a few inches. As such, they can cause the uterus to increase to the size of a five month pregnancy. Although, the symptoms of fibroids are not always apparent, they often cause heavy bleeding and pain in women. A recent research concluded that around 60 to 75 percent women contract such fibroids by the age of 50, at least once in their life.

Depending on the site of formation, uterine fibroids are distinguished into different types. Intramural fibroids in the lining of the uterus and subserosal fibroids which develop outside the uterus are the most commonly observed fibroids.

What causes Uterine Fibroids?
Although, the exact reason for the formation of fibroids are obscure, medical professionals have determined certain factors that may affect their formation. Some of them are:
1) Hormones: Progesterone and estrogen, produced by the ovaries regenerate the uterine lining during each menstrual cycle and trigger the growth of fibroids.
2) Family history: If you have had a family history of uterine fibroids, then you're likely to develop the condition yourself as well.
3) Pregnancy: The production of progesterone and estrogen increases during pregnancy which increases the likelihood of fibroids.

What are the signs of the condition?
Depending on the location and size of the tumors, symptoms of such fibroids include:
1) Heavy bleeding and blood clots during periods
2) Pain in the pelvis
3) Frequent menstrual cramps
4) Pressure and pain in the lower abdomen
5) Swelling in the abdomen
6) Pain while intercourse

What is the procedure of the treatment?
Ultrasound and pelvic MRI are common diagnostic procedures to check for uterine fibroids. After diagnosis, depending on your age, size of the fibroid and your comprehensive health, the doctor would prescribe you with appropriate medications. Malaria is a life-threatening disease transmitted through bite of anopheles mosquito infected with a plasmodium parasite. The parasites travel to the liver, where they mature and infect red blood cells. Symptoms of malaria develop within 1-4 weeks following the infection. Like: high fever with shaking chills profuse sweating headache nausea/vomiting diarrhea/bloody stools anemia muscle pain seizures coma dragging heavy sensation in abdomen (enlarged spleen). Symptoms may appear in cycles. Episodes of symptoms may occur every 48 hours in infection with p. Vivax or p. Ovale. This cycle of symptoms is also one of the major signs that you are infected with malaria.
