Renal Biopsy - When Should You Go For It?

MBBS, DM - Nephrology, MD-General Medicine
Nephrologist, Delhi
Renal Biopsy - When Should You Go For It?

Kidneys play an important metabolic role and are essential for balancing salt, minerals and water in the body. They also play a significant role in removing waste products from the body. They make urine, which contains all the waste materials that are eliminated from the body. They also play an important role in blood pressure regulation and in maintaining the balance of various minerals in the body. Any suspected kidney damage should be confirmed by a kidney biopsy, which will reveal the exact disease, thereby directing towards the appropriate treatment.

Why and when is it done?

A renal or kidney biopsy is done in the following situations:

  1. When there is abnormal protein in the blood or urine, which is indicative of a kidney disease, and the exact cause needs to be found out
  2. When kidney failure is suggested by blood tests but cause is not clear
  3. To find the cause of bleeding in the urine (haematuria)
  4. To identify and/or confirm diagnosis after a CT scan or ultrasound
  5. To check how well a transplanted kidney has been received

Know about the procedure

A renal biopsy is mostly done as an outpatient procedure and is a type of biopsy known as percutaneous biopsy (biopsy where a needle is inserted through the skin into the renal tissue). Very rarely, it may be combined with the CT scan or ultrasound and be done in the radiology department. This may be done on inpatients. The patient is made to lie on his/her back and a local anesthetic is used on the area of the injection. A thin, long needle is directed towards the area of the kidney from where some kidney tissue is removed for sampling.

In some cases, the direction of the needle may be decided by a CT scan or ultrasound. While this is a closed biopsy procedure, in some cases, as a part of the surgery, open biopsies may also be obtained, where a sample of tissue is extracted for analysis.

Recovering from a biopsy
The person would need some time to recover from the procedure, as there would be some discomfort at the site of a needle insertion. Vital signs would be monitored for the next couple of hours during which the person would also be monitored for internal bleeding. A pain reliever can be used if required. Haematuria or blood-tinged urine can be seen disappearing within the next 12 hours. Very rarely the bleeding can be severe and require angiography and further procedures. The person should also avoid strenuous activities for the next few days.

Hi Dr. you r very good. I feel pain on my left knee. Si please gv some advice and oblize me. Thanking you.

Hi Dr. you r very good. I feel pain on my left knee. Si please gv some advice and oblize me. Thanking you.
Knee joint is very complex joint made by 3 bones - femur, tibia, fibula and knee cap - patella. Several ligaments, cartilage and menisci are covered all around. So pain in knee joint may be due to several reasons. In your case the pain may be due to some internal injury like cartilage etc for which you are not aware or there may be knee arthritis as it is very common these days or may be due to deficiency of vitamins or uric acid. So in general I would advise you to please go for proper investigation so that the reason can be diagnosed. In the mean time to get relief from pain and stiffness please follow the physio measures: precautions- 1. Avoid prolong standing. Standing on one leg or running for time being if its painful. 2. Take care while bending your knees, it should not directly strain your joint. 3. Avoid heavy weight lifting. 4. Wear shoes with good arch support, do not wear high -healed shoes. 5. Use the correct positions during activities so that you do not strain your muscles. 6. Try to change your sitting position every 15 min so that it should not give strain on your knees. 7. Avoid sitting on floor with both knees bending. 8. Take diet rich in calcium like milk, dairy products, banana etc which will enhance your calcium and gives you strength. Treatment - 1. Use of cold or hot packs which ever suits you - apply ice or hot pack to the painful area and massage for near about 10 minutes. Make a habit for doing atleast twice a day initially. 2. After this using your thumb massage your knee area gently. Exercises include: 1. Quads isometrics. 2. Hamstrings isometrics. 3. Slr - straight leg raising 4. Bridging do not do single leg bridging if pain is very severe. 5. Knee bending lying or supine position. 6. Heel raising 7. Step ups 8. Lunge 9. Squatting you can do the above treatment plan it will give you pain relief. But if you face severe pain while doing the exercises then stop doing and apply some talcum over knee, with thumb massage your knee area slightly and gently. Get well soon. Good luck.

I was suffering pain from 2 days on both sides of testis and even I can't walk properly with that pain.

I was suffering pain from 2 days on both sides of testis and even I can't walk properly with that pain.
Over masturbation depletes the body of necessary neurochemicals, which help power the sex drive and keep the body in a balanced state. Men who masturbate in excess see a depletion in blood flow and an increase of the inflammatory responses. When the testicles experience a reduction in blood flow along with inflammatory signals, pain can ensue. Men who over masturbate see a decrease in testicular circulation and an increase in stagnation that leads to pain and testicle soreness. Meanwhile the testicles experience an increase in testicular interstitial tissue edema that creates groin pain.

This is a general low back ache and for this you can follow these measures: one keep a pillow right under the knee while sleeping, next is you can keep ice in the painful area for about 5--10 minutes, if pain still persists you can stretch your body by twisting the waist on both sides how we used to do in the school drill similarly you can try which will help you relax as well will reduce the pain. It looks like you are anaemic. If you have back pain after you sit for long hours then it is due to your haemoglobin levels as it is important to check that. Anaemia always leads to the symptoms of being tired and also having back / leg pain though there may not be any pathological reasons for back pain.

I am 30 year old. I have severe pain in my leg alternate. I am not getting any relief from medicine. The pain will go...
Most leg pain results from wear and tear, overuse, or injuries in joints or bones or in muscles, ligaments, tendons or other soft tissues. Leg pain can also be caused by blood clots, varicose veins or poor circulation. To relieve mild pain: stay off your leg as much as possible apply an ice pack or bag of frozen peas to the affected area for 15 to 20 minutes three times a day elevate your leg whenever you sit or lie down please go to a physiotherapist if: you have pain during or after walking you have swelling in both legs your pain gets worse your symptoms don't improve after a few days of home treatment you have painful varicose veins if you feel the pain is from muscle cramp then you need to drink a lot of water because poor fluid intake (dehydration) may predispose to cramps also low blood levels of either calcium or magnesium directly cause cramps. Best wishes.

I am 32 years old male having pain in my heel both feet. The pain would rise when I get up from my sleep that would make me difficult to walk. Eventually it would subside after an hour or 2. But the pain remains if you press your heel with your hands. At this point it is bearable. Also I have a pain in my lower back. Just 2 of the bones in the lower back. I am not sure which specialist should I consult because my friends say either visit a medicine Dr. Some has suggested an Orthopaedic and others a neurologist. Please suggest.

Kindly take rest and give your back the time to recover from the burden. As it is the best treatment for acute back pain. Take liquid diet like daliya, mooong daal soup, plantains, khichdi. Take warm water at least 3 liters a day. An take ashtavarga kshayam 30 ml twice a day with warm water, empty stomach.
