Hi, This is Dr Akshay from Fortis Hospital, New Delhi. If you have ACL injury i think it is best to get it reconstructed Arthroscopically to prevent joint degeneration and maintain stability of joint. Do not take too much stress Before sleeping do not take tea coffee or cold drinks Do relaxation therapy- close your eyes & contract & relax each joint of the body from head to toe Do meditation Listen to soft soothing music Take 1 cup of milk add 1/2 teaspoon turmeric powder to it & boil for 15min then keep it to cool & drink it before sleeping thrice a week

My blood test results are under under. Kindly suggest to me for better treatment. I am also gout patient for long time and take medicine febulas 40 & sobisis forte. These medicines discontinue for last one month. Blood test: - vldl 34.94, glucose pap 141.05, ast (got) 43, alt 41, uric acid 9.46, triglycerides 174.72, urea 18.46, creatinine 1.13, total bilirubin 0.6, direct bilirub. 0.20, cholesterol 170.64, direct hdl 37.98 & direct ldl chol 120.48. Heel pain as in your situation is most likely due to plantar fascitis. It is due to inflammation in the tough fibrous tissue called plantar fascia in sole extending from heel to base of toes. The treatment mostly is conservative and lifestyle modifications to achieve maximum functionality with minimum pain. Follow these lifestyle changes religiously to get benefit. 1. Reduce body weight as obesity will aggravate the stress on heel. 2. Avoid exercises like running / walking on hard surfaces. Stop such activities when you start developing pain. 3. Roll your feet on bottle of cold water for 20 mins 4 times a day or can apply ice pack. 4. Use shoes with silicone sole inserts. 5. Do calf muscle and heel stretches daily. 6. Avoid prolonged standing or sitting. 7. Use night splint to keep foot flexed at night. Stop using it once pain is relieved. 8. Use analgesics like Ibuprofen for decreasing pain and inflammation. With proper lifestyle changes as above 90 % patients have relief and functionality. Injections in plantar fascia are done only after conservative treatment as above has failed.Only 10% patients would require surgical intervention like plantar fascia release or gastrocnemius recession. There are three major possible explanations for morning back pain. 1.Inflammatory back pain: worrisome, relatively rare, and well known. 2.Myofascial pain syndrome: probably very common, but more mysterious and controversial. 3.Sleeping postures: although the most “obvious” reason for waking up with a kink in your back, it might be worse than you expect. So, sleep in proper position. Do proper exercise, take proper diet rich with calcium and multivitamins. Maintain proper posture.

I am 71 yrs 5ft 8 in weight 90 kg am suffering from diabetic for last 5 yrs lately I find that sole of both my feet become numb when I get up in the morning and also my right heel is very painful please advice what I should do ?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Your symptoms of numbness could be due to long standing diabetes in your case although other causes cannot be ruled out especially issues in lower back region. I would advise you for a very strict sugar control, you can take medications like pregabalin or gabapentin in low doses initially, take vitamin b12 and adequate calcium and vitamin d supplementation. Also heel pain could be due to inflammation in your heel fascia. Treatment modalities are aimed at relieving inflammation of the fascia, like ice packs, physical therapy, footwear modification and anti-inflammatory therapy. I will observe you with this management for around 2 weeks and then we will see if we require any investigations in your case. Do not hesitate to contact me if you need any further assistance.

Kindly have blood tests like bt, ct & prothrombin time. Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Do not ignore. It could be beginning of a serious problem.

It is quite difficult comment without actually examining the patient in such specific complaint. History of open heart surgery years ago and swelling in one leg now does not seem to have any obvious link. A Doppler examination of the affected leg is advisable to lok for evidence of DVT (Deep Vein Thrombosis ). Let a qualified physicianexamine her without wasting time and follow his advice accordingly.

Sleep on a hard bed with soft bedding on it. SPRING BEDS, FOLDING BEDS OR THICK MATRESS ARE HARMFUL Use no pillow under the head. DO HOT FOMANTATION. Paracetamol 250mg OD & SOS x 5days. Caldikind plus 1tab OD x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again.. Do not ignore .It could be beginning of a serious problem.

Do the cat/cow avoid bending in front. Postural correction- sit tall, walk tall. Apply hot fomentation twice daily. Extension exercises x 15 times x twice daily. Bhujang asana. Core strengthening exercises. Back stretchingstretch.

Your knees are one of the most vulnerable parts of your bodies. In case of strenuous or vigorous activities, knees are more prone to damage and injury than the other parts. Several of the knee tissues are fairly delicate and often are easily impaired. Among the other knee tissue injuries, meniscus tears are perhaps the most frequent. Meniscus is essentially a cartilage that protects and stabilizes the knee joints. However, an untoward twist or a strain can lead to a tear in the meniscus. Meniscus tears are a common phenomenon among athletes and sportsmen. They have been noted to be more recurrent in older individuals. 

Some of the early symptoms that indicate the onset of a meniscus tear are swelling and difficulty in bending or straightening of the leg. Sometimes a "locking sensation" in the knees also forebodes a torn meniscus. The inflammation potentially causes a lot of pain and irritation. While, it is fairly easy to ignore a meniscus tear for a normal knee injury, prolonged overlooking may only aggravate the pain and worsen the condition. The process of diagnosing a meniscus tear usually involves an MRI scan and an X-ray examination, over and above a detailed evaluation of the wound.

Treating meniscus tears vary according to the extent of the injury. Depending upon the location and the depth of the tear, various cures can be tried. The healing process, however, depends upon other allied factors such as age, level of physical activities and one's medical history. An external and minor injury is easily mendable. Whereas, graver injuries are resolved through surgical means. The other non-operative means include certain prescribed medicines and specific exercises. Some of the recommended ways of battling a meniscus tear can be enlisted as:

  1. Resting the knee by limiting the activities that involve excessive use of the legs or they may potentially strain the leg muscles.
  2. Applying ice pack can significantly bring relief to the swelling and the pain in the injured knee. Doctors advice regular use for a duration of at least 15 days.
  3. Compressing the knee using an elastic bandage or neoprene sleeve reduces the swelling in the knee.
  4. Prescribed medicines in the likes of non-steroidal anti-inflammatory drugs can be effective remedies for a meniscus tear. However, one must mandatorily consult a doctor before consuming such medicines.
