I have infertility issue from past 5 years. All the report s are next to normal according to doctor there is no such physical issue. Last doc to go for IUI but PCT was negative at that time and then IUI was not done. As we gone through IUI twice and IVF also was failure. Am completely stressful, what to do where to go. As one of my doc said there is hydro sulpinix so that embryo will not get attached to uterus and not growing so the IVF also failure. She suggest me to go for laparoscopy. 2 times the procedure withheld and the reason according to lapro surgeon was like not required in your case. Last month 3rd dy of my period I was suffering from severe pelvic pain and swollen lower abdomen. Again doc suggest me to go fr lapro and this month also the same pelvis pain with swollen abdomen. I cried like anything. Please help me out and please suggest me when lapro n IVF to be done. Which dy of my period. HSG, AMH is 6.0,PRL 12.43,TSH 3.80 ,progesterone1. 82 may vary monthly ,thyroid, USG report are also normal. Husband's report is also normal. Thank you.

Mam, srry to say, your hydroalpinx, is causing infertility, let me tell you bit clear IVF. Is the way you get conception, but untreated hydrosalpinx have lower conception rates than controls and it has been speculated that the tubal fluid that enters the endometrial cavity alters the local environment or affects the embryo in a detrimental way. Thus, many specialists advocate that prior to an IVF attempt, the hydrosalpinx should be removed. Lower abdominal pain, may be PID.

The diagnosis of menopause is made after periods have stopped for at least 1 year after the age of 40 due to reducing ovarian activity.The age span can vary between 45 to 55 and the median age is 52 years.Menopause occurring before the age of 40 is premature and one needs to consult a Gynaecologist.

The time span for several months to several years leading to menopause is called perimenopause.The periods during this time may become irregular and the patient may experience hot flushes, night sweats, anxiety, mood swings etc; 30-60% of women may experience emotional symptoms at this time. The major symptoms include:

  • Hot Flushes and Night seats
  • Vaginal dryness
  • Tiredness and sleep disturbances
  • Mood swings and lack of concentration
  • Loss of interest in having sex

Every woman experiences menopause differently. A few will have a combination of different mild symptoms while others will have more severe symptoms.Some women choose to go through menopause without any treatment whereas others prefer to have hormonal or non-hormonal medications to help control their symptoms.

However it is important to consult a Gynaecologist as irregular periods and other symptoms may be due to other pathologies and may need investigating.

A healthy lifestyle is very important during this time to maintain good bone health and general health.Regular aerobic exercises like swimming, jogging, brisk walking and in general maintaining an active lifestyle and maintaining an optimal body mass index is very important and helps in preventing osteoporosis, heart disease etc;

  • Low-intensity exercises such as yoga may help to reduce hot flushes and boost general well-being.
  • Reducing intake of tea/coffee/ caffeinated drinks will help reduce hot flushes and night sweats.
  • Vaginal lubricants and moisturisers help in reducing vaginal dryness during intercourse.
  • Also equally important is having a diet rich in Calcium, proteins and low in fat and refined carbohydrates like sweets and junk foods.
  • Soya-based foods like soya beans, chickpeas, peas are rich is phytoestrogens nad may help in reducing perimenopausal/ menopausal symptoms.
  • Getting an annual health check up done helps in identifying risks earlier and taking appropriate actions earlier on.
  • Women should have regular PAP smears, BP check, Mammograms, Lipid profile done and other tests if clinically indicated.

Finally, it is best to consult a Gynaecologist if a woman is having any perimenopausal or menopausal symptoms so that appropriate treatment can be instituted.

Ectopic Pregnancy: Know The Causes and Its Treatments

In a normal pregnancy, the egg that the ovaries release enters the fallopian tube. If a sperm fertilises it, the fertilised egg attaches itself inside the uterus. However, sometimes the fertilised egg can attach itself outside the uterus. This condition is called ectopic pregnancy.

Ectopic pregnancy can be detected in the first few weeks of the pregnancy itself. If your doctor does discover ectopic pregnancy, you would need immediate medical attention. Ectopic pregnancies can be sad and scary. Fortunately, one ectopic pregnancy doesn't mean you can never conceive again. Many women who lost their first baby to ectopic pregnancy have been able to have a healthy and normal pregnancy the second time around.

The causes of ectopic pregnancy include:

  1. An inflammation or infection of the fallopian tube can lead it to become entirely or partially blocked.
  2. Scar tissue from a surgery or an infection of the fallopian tube may also hinder the movement of the fertilised egg.
  3. Surgery in the tubes or pelvic areas in the past might cause adhesions.
  4. Birth defects or abnormal growths can cause anomalies in the shape of the tube.

These causes are usually followed by certain risk factors, such as:

  • Age (The age group of 35-44 especially)
  • An ectopic pregnancy in the past
  • Previous abdominal or pelvic surgery
  • Pelvic inflammatory disease
  • Several prompted abortions
  • Conceiving with an intrauterine device in place
  • Smoking
  • Endometriosis (growth of uterus lining tissues outside the uterus).
  • Fertility treatments.

The signs and symptoms of ectopic pregnancy include:

  1. Minimal vaginal bleeding
  2. Vomiting and nausea with pain
  3. Pain in the lower abdomen
  4. Sharp cramps in the abdomen
  5. Localised pain (Pain concentrated on one side of your body)
  6. Pain in your neck, rectum or shoulder
  7. Rupture of the fallopian tubes can cause fainting due to the bleeding and pain

The treatment of ectopic pregnancy can be any one of the following:

  1. If the pregnancy has not progressed too far, methotrexate will be administered. This absorbs the pregnancy tissue and can save the fallopian tubes.
  2. The tubes may be removed if they have ruptured or stretched, and have started bleeding.
  3. Laparoscopic surgery (operations performed by making minor incisions) may be performed to remove or repair the tubes and recover the ectopic pregnancy.

As I told you sir I have fibroid having size 10 cm x 14.2cm x 14.7 cm seen completely replacing fundus and body of uterus. Margin of fibroid smooth all round. Endometrial echoes are not identified. Only my tummy is size is increases. My menses heavy blood loss. No pain. And it comes after every 3 month.

It appears you will need surgery to remove the fibroid. There is no medical treatment for this I am afraid.

it is advisable to refrain frpm intercourse till three months or atleast the frequency should be reduced .but incase there is any bleeding or pain or a precious pregnancy one should refrin at lest till the problem solves

Why You Should Walk, Not Run, Your Way To Fat Loss

1. When you do any intense cardiovascular exercise like running, you burn calories from both muscle and fat. The body cannot pick which calories to burn, and thus by running you actually decrease your muscle mass which eventually makes you more metabolically inefficient.

2. The production of a stress hormone called cortisol increases after doing too much cardiovascular exercise, which leads to bigger food cravings since your body wants to compensate for all those burned calories. So you end up consuming more calories than you have previously burned.

**Too much cardio exercising makes you leptin-resistant, which has the same hormonal effect as sleep deprivation. This prevents your body from telling you when it?s full. You get very hungry quickly and due to the high amounts of cortisol you eventually develop insulin resistance and back to gaining weight.

**Cardiovascular exercises can have a negative impact on thyroid function. The thyroid produces a hormone called T4, and in order to have a properly functioning metabolism, it is then is converted to T3. But, the thyroid is unable to produce T4 if you are creating too much cortisol in your body. You end up with a dysfunctional metabolism, loss of energy and inability to burn fat.

3. Running is considered high impact and the further you run, the more impact. If you are overweight this can cause havoc on your joints and knees.
Summary: Running can cause hormonal imbalances, reduction of muscle mass, less efficient metabolism, which can lead to more weight, and if you are overweight, it can have a negative impact on your knees.

Walking on the other hand can be much more balanced and a healthier alternative, even though the amount of calorie burning effects are not as high as running. You decrease the amounts of cortisol with walking, which is much better in the long-term. Walking makes you more sensitive to insulin, enabling better calorie burn from other exercises and it doesn?t create unmanageable hunger and cravings. It also produces dopamine and serotonin at reasonably high levels which make you feel happier and motivated in your overall health and wellness.

So, start walking as much as possible! At least three days a week take 30 minutes to walk, and increase this gradually. Choose walking instead of running, especially if you are over 40 and not accustomed to an exercise regimen. If you want a tone and firm body, do some weekly resistance training exercises for 20 or 30 minutes. Combine these tips with a great diet and you won?t have to run ever again.

stomach pain occurs during periods is due to contraction of muscles of uterus. For take relief follow these methods: * take i/3 rd piece of ginger boil it in one glass of water, when half glass water remain then take it with one teaspoon of ajwayin. * use warm water bottle put it on your stomach area with wrap in the towel. * take rest properly do not do hard work. Do not do running and jogging before 7days from periods coming date . * Do massage with lukewarm mustard oil around area below stomach and back .

As you had menses which was heavy on first day you r not pregnant for sure. Even if menses are coming less how does it matter as your primary concern of not getting pregnant is not there any more.

Female Orgasmic Disorder - Tips to Improve it

Though the act of intercourse is portrayed as an act of pleasure, achieving an orgasm is an indication of a satisfying sexual act. Inability to achieve an orgasm is far more common in females than in males and this can be due to various reasons.

Read on to know some more about the causes and ways to manage this:
A mix of psychological, social, and physical conditions can lead to orgasmic disorders in female. Some of the common causes are listed below:

  1. History of sexual abuse
  2. Stress
  3. Poor self-esteem
  4. Relationship issues, lack of trust, conflicts, etc.
  5. Cultural or social beliefs
  6. Advancing age
  7. Chronic medical conditions like diabetes
  8. Medications like selective serotonin reuptake inhibitors (SSRIs), which are frequently used in depression (depression is common in women)
  9. Gynecologic issues like hysterectomy

It is a vicious cycle that is contributed by more than one of these factors, which leads to strengthening of the disorder in the future. Types of orgasmic disorders: There are four types of orgasmic disorders, which are as follows:

  1. Primary anorgasmia: Women with such disorders have never been able to achieve an orgasm.
  2. Secondary anorgasmia: Over a period of time, women who previously have had orgasms are unable to achieve an orgasm.
  3. Situational anorgasmia: Women suffering from it would be able to achieve an orgasm only during certain situations like oral sex or masturbation, but may not climax during regular sexual intercourse.
  4. General: These women could be highly stimulated and aroused, but would still be unable to achieve climax.

Management and diagnosis
The first step in management is diagnosis. A detailed discussion with the doctor about sexual history followed by physical examination can help identify the problem. Being open about discussing this private topic is very important in diagnosis and treatment.

Treatment would depend on the underlying cause, and include the following:

  1. Diagnose and treat/manage underlying medical conditions like diabetes.
  2. Switch to an alternate antidepressant if SSRIs are being used.
  3. Involve in sex therapy where cognitive behavioral therapy is a big component. Becoming aware of what can be is very important.
  4. Sexual counseling with the partner to resolve any conflicts, improves trust levels and the overall health of the relationship. This is often a major cause for this disorder.
  5. Involving the partner to help in achieving an orgasm by clitoral stimulation, etc.
  6. Hormone therapy in the form of a pill, patch or gel may be used to increase sensitization in the genital area. These help by improving blood flow to the genitals and increasing sexual desire.

It is important to understand that female anorgasmia is very common and though it can be frustrating, it can be managed effectively. But do not forget to admit the problem and seek help. If you wish to discuss about any specific problem, you can consult a gynaecologist and ask a free question.
