What Puts You at The Risk of Sinusitis?

Hello friends, I am Dr Rahul Bhargava, I am a consultant ENT at MGS hospital and have a clinic in Rohini sector 15, Delhi. Today I am going to discuss with you the Sinusitis topic. Sinusitis is a very common condition nowadays and it has been estimated that 20% of the people in India are suffering from Sinusitis and in children around 6 attacks of sinusitis happen in children every year. Sinusitis bahut hi common condition hai, iska ilaj bhi hai magar pehle hame yeh dekhna hai ki kis wajah se hai. Sinusitis kayi karno se hota hai sabse important karan hai ki hamari naak ke andar kuch banawati kharabi ho isme naak ki haddi ka tedapan hona, kuch cells extra hona ya sinus ki opening ke andar rukawat hona. Yeh sabse important karan hota hai Sinusitis ka or iske liye surgery ki zaroorat hoti hai. Toh jin cases mein structure problem hoti hai toh, we need to do surgeries in these cases and remove the problem. The other important cause Sinusitis ka hai woh hai allergy naak ke andar allergic hona, ya allergic rhinitis hona, naak ke jo sinuses hoti hain uski opening ko chota kar deta hai aur usse sinus ki problem hoti hai, kafi cases ke andar pehle patient ko allergy hoti hai aur uske baad wo sinus mein convert hota hai. Toh hum yeh chahte hain ki hum woh allergy ko wahi khatam kar de aur aage woh sinus ki problem mein na badle. Allergy kis wajah se hai, kis karan se hai, kis cheez se hai, wo janana hamare liye kafi avashyak hota hai, jis cheez se allergy hoti hai kuch cheeze aisi hoti hai jisko hum avoid kar sakte hain aur kuch ke liye immunotherapy deni padti hai. Lekin jaise bola jata hai prevention is better than cure, aise hi allergy ka sahi samay pe ilaj se wo aage sinus ki problem mein nahi badalta hai. Ek bahut aajkal common karan hai woh hai pollution, pollution ek irritant ka kaam karta hai sinus ke andar puri nak ki jo lining hoti hai usko usme sujan aa jati hai usse sinus ki problem ho jati hai. Toh uske liye aapko pollution ke area se bachna chahiye, jab jis din jyada pollution hai us din raat ko 9 baje ke baad aur morning mein 6 baje se pehle nahi nikalna chahiye ghar se bahar. Uske alawa mechanical barrier, koi bhi rukawat jo sinus ke raste mein aati hai woh bhi Sinusitis cause karti hai, jisme sabse important cost hota hai adenoid hypertrophy, adenoid ek mask hota hai jo naak ke piche hota hai aur sinus ka jo risha hota hai uske rasthe mein aata hai. Kisi bache mein agar adenoid bada hua hota hai toh kaan ki dikkat ke sath-sath uske nak ke andar sinus ki problem hoti hai. Aur aaj kal sinus ke andar chote baccho ke andar pehle adenoid ki hai surgery ki jati hai, taki woh sinus ki problem se nijat pa sake. Uske alawa jo sharir ki immunity hai wo bhi ek bahut important cause hai ki hamara sinus ki problem hone ka, immunity mein sabse jyada aaj kal Vitamin D ka role aa raha hai. Jin marijoo mein Vitamin D ki kami hoti hai unke under sinus ki problem bhi baar-baar hoti hai. Toh sinus ka ilaj jo hai woh sambhav hai kuch patients mein hamme surgery karni hoti hai, jiske andar koi banawati kharabi hui koi cell extra hai, jo ki hamme CT Scan se pata lagta hai usme hamme surgery ki zaroorat hoti hai. Jin baccho mein adenoid bada hua hai, hamme adenoid ki surgery karni padti hai, agar allergy ki problem hai toh allergy ka ilaj karna padta hai, toh isse tabhi hum is problems se completely nijat pa sakte hain. Earache: Causes, Symptoms, and Treatment

Today through this medium, I am going to talk about a very common problem in children and adults that is an earache or the pain in the ear. The pain in the ear can be due to local causes in the ear or it can be due to referred causes where the problem lies somewhere else and it is presented as pain in the ear. The common causes of an earache is especially in children is Otitis Media with effusion, which is water into the ear, otitis effusion which is the infection of the external part of the ear, Acute Suppurative Otitis Media which is the formation of pus behind the tympanic memory, a forum body or impacted earwax, these are few common problems which can lead to an earache and can be very troublesome in children. A forum body needs to remove as soon as it is detected, if you have impacted earwax and it is causing a problem then it needs to go. If you have acute suppurative otitis media there which is the inflammation and the pus behind tympanic memory then the usual cause lies in the nose. So we have to treat both the nose and the ear to solve the problem and prevent such frequent episode in the later on. Otitis media effusion is the very important cause of an earache in children it also leads to hearing difficulty and leads to developmental problems in children. The adenoid hypertrophy is a very usual cause which leads to otitis media with effusion and we need to tackle also along with an earache. The other important causes are the referred causes where the problem lies either in the temporomandibular joint which is the joint just in front of the ear or in the throat any infection in the throat or any problem in the throat can give rise to pain in the ear. Hi, Discharge of pus from the ears from childhood I take medicine after that they become right but from 2 to 3 years I am suffering from ear infection ear is eating some times pus will also come I use candibiotic ear drops when I use this drop ear become right but after 7 to 8 days same problem occurs please suggest me.

Hello lybrate-user, the ear infection or any infection should not be there in the body for such long time. As far as ear goes, there two major types of infection in the ear. You need to consult an ENT doctor for the same. The diagnosis is made after cleaning the ear and looking at the ear drum. Take care.

Why Is Tonsillectomy Performed?

The tonsils are soft tissue masses which are located at the back of your throat, usually at the pharynx. The tonsils are masses which are part of the lymphatic system that helps fight infections. However, tonsils can themselves become inflamed and cause a lot of pain. Tonsillectomy is a procedure to remove the tonsils and relieve pain.

Why is tonsillectomy performed?
A few decades back, tonsillectomy was a routine surgery performed to relieve any pain within the tonsils. This method of treatment however has changed over the past few decades as pain medications for inflammation have improved vastly. However, tonsillitis is still performed in many cases wherein it tends to cause acute problems. Some of the cases it is applicable in are:

  1. Recurrent sore throats caused by tonsillitis
  2. Upper airway being obstructed due to inflammation, thus causing difficulty with sleeping, snoring, or causing sleep apnea (irregular breathing during sleep)
  3. Chronic bad breath problems caused by recurrent inflammation of the tonsils
  4. It is also done along with dental surgery for correcting teeth with imperfect positioning
  5. Abscess formation in the mouth
  6. Ear infections being regularly caused by inflammation of the tonsils

Other reasons why it may be recommended
Tonsillectomy may be also be recommended when there are no indications of problems but tonsillitis and other infections keep cropping up too regularly. This affects children and can make them sick or keep them in pain regularly. Some of these other indications are mentioned below:

  1. Tonsil infections and inflammations happening too frequently within a year
  2. Tonsillitis is caused by bacterial infection wherein the antibiotics aren’t effective
  3. Formation of pus in the form of an abscess within or around the tonsils which keep forming even after repeated drainage or administering antibiotics

Possible complications and risks
Like any other form of surgery, tonsillectomy has certain risks associated with it as well. However, these are very rare. Some of these risks and complications are mentioned as follows –

  1. Swelling of the mouth: The tongue and the soft palate or the roof of the mouth may swell, resulting in breathing difficulties an few hours after the surgery.
  2. Infection: As is the case with any surgery, infections may occur in rare cases, which may require further treatment.
  3. Adverse reactions to anesthetics: Many patients exhibit an adverse reaction to anesthetics and may develop symptoms such as a headache, nausea, muscle soreness and frequent vomiting.
  4. Bleeding during surgery or healing: Although it is rare, bleeding during and after surgery has been reported after all sorts of procedures and it may occur with tonsillectomy as well.

Insect In Inside Ear - How To Deal With It?

It is that that time of the year when insect are all around. Suddenly one feels a buzzing sound or an itchy feel in the ear. To your horror, one feels a live insect in the ear which can be extremely annoying and a nightmare. Insects get stuck in the ear when enter our ears as they cannot fly or crawl out. They try their way out and their movements inside our ears can be very uncomfortable, painful, and itchy. Moreover, they can easily produce infection.

In this scenario, do not try to remove an insect with cotton swabs, tweezers or hair clips as it will make situation worse and can lodge the insect deeper into ear canal damaging the eardrum, leading to permanent hearing loss. If an individual is not certain of the potential for harm in regard to insect in the ear, one should seek medical care immediately. Insects in the ear are common reasons for visits to doctor's clinic, especially in children.

If one suspects an insect in ear, one may experience pain, swelling, blood or crackling. One may even feel biting, stinging, hearing loss or dizziness. It is best to stay calm in this situation as being active may lodge the bug further in ear or cause it to move further back, or cause serious damage to the sensitive eardrum

One way to try to remove a bug in ear is by tilting the ear toward the ground and attempt to wiggle the ear. Grasp the earlobe and give it a wiggle. If the bug is not too far into ear canal, it may fall out on its own. If the bug is still alive and is not too far inside of the ear canal, it may simply come out on its own. If one stays calm and keep objects including fingers away from ears, it is likely that the bug will find its way back out of ear.

One can also try to flush the ear with warm water with a dropper or a bulb syringe. This can be done by holding head upright and stretching the ear canal by pulling the outer ear and then putting a steady stream of warm water into ear. Tilt head to the side to drain out the ear. Do not try this if one suspects that ear drum has been ruptured to prevent additional damage.

To avoid stinging or eardrum rupture from scratching or biting , one may use a drop or two of mineral, baby, or olive oil inside your ear canal to kill the insect. Finally, visit an ENT specialist doctor as they can remove the insect by special suction devices. Post insect removal, one must look out for signs of infection as swelling, dizziness, hearing loss, fever, and pain. Prolonged Sore Throat - Is It Chronic?

Are you suffering from a sore throat that is prolonged in nature? Although, a sore throat is not very severe, being prolonged in nature indicates that it is chronic. You should not ignore a sore throat that does not heal over a long period of time. This might be an indication of a serious problem, and you will require proper diagnosis.

When is a sore throat termed as chronic?

  1. A sore throat may take various forms. It can be itchy, burning, scratchy in nature, and can also cause difficulty in swallowing food.
  2. It is likely for a sore throat to be accompanied by a form of pesky cough that leads to further irritation of the throat.
  3. Sore throats can also occur from common colds and several viruses. These usually resolve on their own.
  4. However, when your sore throat is persistent and you cannot find relief, it is referred to as a chronic sore throat. The primary cause of sore throat includes strep throat, allergies, tonsillitis or the infection of your tonsils, inhaling air pollutants, influenza, cigarette smoking, and inhaling via the mouth.
  5. Among these conditions, strep throat and tonsillitis may turn into severe problems, if not treated properly.
  6. Smoking is a major cause of chronic sore throat. You should control your smoking to eliminate these sore throat symptoms.
  7. Snoring is also a major cause for sore throat.

Relief from chronic sore throat
No matter what is the cause of your chronic throat pain, or sore throat is, you should follow some steps and undertake simple home remedies for soothing your scratchy, itchy and painful throat. Some of them include the following:

  1. Do hot water steam inhalation daily, which will help
  2. You must stay well hydrated and should moisten your throat by drinking lots of fluids throughout the day. You should add honey to warm drinks and hot tea.
  3. You may take over the counter pain relievers.
  4. You should install a humidifier in your home for preventing dry air that causes irritation to your throat.
  5. You may try gargling regularly with warm water and salt, at least twice a day.

It is difficult to diagnose the exact cause of a sore throat, but when it worsens or does not tend to leave, something serious is indicated. How To Treat A Nosebleed?

How To Treat A Nosebleed?

A nose bleed can take place due to a number of reasons, which usually all boil down to acute dryness of the nasal passages. The nosebleed and dryness can stem from sinus, allergic reactions, constant blowing of the nose, infections, and also during very harsh winter months when there is no lubrication in the nose. Nose bleed can also occur due due to conditions, such as high blood pressure and when a foreign object gets stuck inside the nose. Here are a few tips on treating nosebleeds.

  1. First step: The very first action when you experience a nose bleed should be to sit down. Once you are sitting, you should firmly pinch the spot that is soft. This lies just above the nostril. Do this for at least ten to fifteen minutes. The trick is to stay upright rather than lying down as this will bring down the blood pressure in the nasal blood vessels to stop further bleeding.
  2. Breathe: Breathe with your mouth as you lean forward to help in draining the blood into your nose, rather than out of it or towards the throat.
  3. Ice compress: Find a bag of frozen corn or peas in your freezer or an ice pack, and press this firmly against the nose to help in stemming the bleeding. As you are doing so, do not forget to cover the bridge of your nose with a towel so that it prevents you from catching a cold.
  4. Medication and side effects: If you are on any kind of anticoagulants that end up thinning the blood, then you may immediately want to see your doctor about a possible side effects and a change in the medication. This could also be an effect of haemophilia where the bleeding does not stop.
  5. Children: If a child who is under the age of two years suffers from such nosebleeds, then you will need to rush him or her to the doctor immediately as it could point at a more serious problem.
  6. Anaemia: Nose bleeds could also suggest the presence of anaemia, so you may want to see your doctor about lab tests to determine and possibly rule out this cause. You can also start medication accordingly for this kind of an ailment.
  7. Nasal Packing: The doctor can carry out this procedure with the help of gauze that can seal the spot where the bleeding is happening.
  8. Cautery: This is a minor procedure that involves burning the blood vessel that is causing the persistent bleeding with a stick of silver nitrate. The stick will be inserted after local anaesthesia is administered to the patient.

Sinus problem is quite common with the allergy in the nose. First we have to rule out any structural problems in the nose, if there are none then we treat the allergy to cure the sinus problem.

Hello, the test which can differentiate is called sleep study or polysomnography. This can be done both At home or at an hospital. The Hospital one has more parameters and is better but many patients have difficulty in sleeping at a hospital. The one done at home has less parameter but easily done.
